Prevalence of Voice Disorders and Correlation Between Hypernasality and Acoustic Characteristics in Children With Cleft Lip and Palate.

OBJECTIVE: To determine the prevalence of voice disorders, relationships between voice disorders and hypernasality, patterns of compensatory articulation disorders (CAD), acoustic characteristics, and associated factors in children with cleft palate with/without (CP ± L). METHOD: The study consisted of 60 children with CP ± L, aged 7-12 years, who had undergone palate repair with/without cleft lip repair. Data were collected by evaluating voice quality using the GIRBAS scale (grade, instability, roughness, breathiness, asthenia, and strain). The Computerized Speech Lab (CSL) was used to evalu…

[Progressive anarthria: one case without lingual apraxia].

Progressive anarthria is a focal cortical degenerative disorder characterized by a profound, progressive alteration in speech without impairment in other cognitive domains. The first symptoms consist of an alteration in the articulation of speech producing telegraphic speech. The disorder invariably progress towards anarthria by deprogramming of the phonation and orolingual movements (bucophonetic apraxia). This type of apraxia is usually associated with orolinguofacial apraxia and it has been anatomofunctionally correlated with frontal opercular involvement of left predominance. The patient p

Integrated neurobiology of bipolar disorder.

From a neurobiological perspective there is no such thing as bipolar disorder. Rather, it is almost certainly the case that many somewhat similar, but subtly different, pathological conditions produce a disease state that we currently diagnose as bipolarity. This heterogeneity - reflected in the lack of synergy between our current diagnostic schema and our rapidly advancing scientific understanding of the condition - limits attempts to articulate an integrated perspective on bipolar disorder. However, despite these challenges, scientific findings in recent years are beginning to offer a provis

Four new speech and prosody-voice measures for genetics research and other studies in developmental phonological disorders.

Research in developmental phonological disorders, particularly emerging subgroup studies using behavioral and molecular genetics, requires qualitative and continuous measurement systems that meet a variety of substantive and psychometric assumptions. This paper reviews relevant issues underlying such needs and presents four measurement proposals developed expressly for causal-correlates research. The primary qualitative system is the Speech Disorders Classification System (SDCS), a 10-category nosology for dichotomous and hierarchical polychotomous classification of speech disorders from 2 yea

Fetal alcohol syndrome: a description of oral motor, articulatory, short-term memory, grammatical, and semantic abilities.

Formal communication skills assessments were completed on eight children with diagnosed forms of fetal alcohol syndrome (FAS) ages four and one-half to nine and one-half years. Non-FAS children ages three and one-half to six and one-half years matched for ethnic background, living situation, and nonverbal cognitive ability served as controls. All but one of the FAS subjects demonstrated abnormalities of the speech mechanism within at least one major valve along the vocal tract. Mental age inconsistent articulation abilities were found in three FAS subjects when matched for nonverbal cognitive

Naming disorders anomic aphasia.

Naming disorders, as a symptom, is always found in all aphasic patient. The patient although has a clear understanding of what he is trying to name or what he is told to write, but needs longer time or can not articulate it. Sometimes he substitutes with other words or even produces jargon words. Assessment and intervention are complicate and delicated the most important functions of the clinician. The goal of word-retrieval activities in treatment is to improve the patient's cognitive performance and to teach method to circumvent the vocabularly when needed.

Trauma-related altered states of consciousness: exploring the 4-D model.

Frewen and Lanius (in press) recently articulated a 4-D model as a framework for classifying symptoms of posttraumatic stress into those that potentially occur within normal waking consciousness (NWC) versus those that intrinsically represent dissociative experiences of trauma-related altered states of consciousness (TRASC). Four dimensions were specified: time-memory, thought, body, and emotion. The 4-D model further hypothesizes that in traumatized persons, symptoms of TRASC, compared with NWC forms of distress, will be (a) observed less frequently; (b) less intercorrelated, especially as me

Apraxia of speech: an overview.

Apraxia of speech (AOS) is a motor speech disorder that can occur in the absence of aphasia or dysarthria. AOS has been the subject of some controversy since the disorder was first named and described by Darley and his Mayo Clinic colleagues in the 1960s. A recent revival of interest in AOS is due in part to the fact that it is often the first symptom of neurodegenerative diseases, such as primary progressive aphasia and corticobasal degeneration. This article will provide a brief review of terminology associated with AOS, its clinical hallmarks and neuroanatomical correlates. Current models o

The buccal flap--a useful technique in cleft palate repair?

OBJECTIVE: To evaluate speech quality and oronasal fistula after primary palate repair using a buccal mucosal flap. DESIGN: Retrospective study cohort of patients with cleft palate. SETTING: Primary care center for treatment of craniofacial congenital anomalies. PATIENTS AND METHODS: One hundred fifty-six nonsyndromic patients underwent palatoplasty with the buccal myomucosal flap by the senior surgeon between 1989 and 2002. The preoperative workup, surgical technique, and other factors that might affect the outcome were identical in every case. Oronasal fistula and variables affecting spee

Automated analysis of connected speech reveals early biomarkers of Parkinson's disease in patients with rapid eye movement sleep behaviour disorder.

For generations, the evaluation of speech abnormalities in neurodegenerative disorders such as Parkinson's disease (PD) has been limited to perceptual tests or user-controlled laboratory analysis based upon rather small samples of human vocalizations. Our study introduces a fully automated method that yields significant features related to respiratory deficits, dysphonia, imprecise articulation and dysrhythmia from acoustic microphone data of natural connected speech for predicting early and distinctive patterns of neurodegeneration. We compared speech recordings of 50 subjects with rapid eye

A cognitive morphology of psychopathology.

In contrast to contemporary approaches to psychopathology that establish diagnostic taxonomies derived primarily from differences in manifest symptoms, this paper, based on an integration of cognitive and psychoanalytic developmental theory, proposes a structural cognitive morphology for understanding and assessing differences among various forms of psychopathology, from schizophrenia to the neuroses. A theoretical model based on the development of cognitive schema, consistent with clinical and research data, considers schizophrenia and paranoid schizophrenia as involving fundamental disturban

Echolalic speech in childhood autism. Consideration of possible underlying loci of brain damage.

The speech of echolalic autistic children is (1) specifically lacking in appropriate use of expressive-intonational features, but (2) the echolalic child's clear articulation of words and phrases indicates that discrimination of phonemic features is intact. The impairment in aphasic disorders is just the reverse. Failure to attend to auditory stimuli and the characteristic language disorder are among the most consistent findings in autistic children; they could be related. Discrimination of differential stress emphasis is the way the normal young child extracts major morphemic word stems and s

Five New Principles to Govern Intersex Pediatric Surgery: A Reply to Kevin G. Behrens.

Individuals with intersex conditions are biological males and females who have, through various disorders/differences of sexual development, an (epistemically) unclear biological sex. Intersex pediatric surgery (IPS) is clarification surgery which seeks to restoratively, functionally, and aesthetically clarify or disambiguate the sex of a child with intersex conditions. IPS is performed in healthcare institutions by a urologist, ideally one with technical certification in pediatric urology. The pediatric urologist is (or may be) part of the multidisciplinary healthcare team assigned to the chi

Quantifying changes in ultrasound tongue-shape pre- and post-intervention in speakers with submucous cleft palate: an illustrative case study.

Ultrasound Tongue Imaging is increasingly used during assessment and treatment of speech sound disorders. Recent literature has shown that ultrasound is also useful for the quantitative analysis of a wide range of speech errors. So far, the compensatory articulations of speakers with cleft palate have only been analysed qualitatively. This study provides a pilot quantitative ultrasound analysis, drawing on longitudinal intervention data from a child with submucous cleft palate. Two key ultrasound metrics were used: 1. articulatory t-tests were used to compare tongue-shapes for perceptually col

Motor and speech disorders in classic galactosemia.

Purpose To test the hypothesis that children with classic galactosemia and speech disorders are at risk for co-occurring strength and coordination disorders. Method This is a case-control study of 32 children (66% male) with galactosemia and neurologic speech disorders and 130 controls (50% male) ages 4-16 years. Speech was assessed using the Percentage of Consonants Correct (PCC) metric from responses to the Goldman-Fristoe Test of Articulation-2 and from a 5-min recorded speech sample, hand and tongue strength using the Iowa Oral Performance Instrument, and coordination using the Movement As

Evidence for a specific impairment of serial order short-term memory in dyslexic children.

In order to better understand the nature of verbal short-term memory (STM) deficits in dyslexic children, the present study used the distinction between item and serial order retention capacities in STM tasks. According to recent STM models, storage of verbal item information depends very directly upon the richness of underlying phonological and semantic representations. On the other hand, storage of serial order information appears to reflect a language-independent system. Hence, if there is a fundamental STM deficit in dyslexia that is not to be explained only by the poor phonological proces

Treating children with phonological problems: does an eclectic approach to therapy work?

BACKGROUND: A survey of clinicians made by Joffe and Pring in 2008 revealed that different approaches exist between researchers and clinicians in the treatment of children with phonological problems. Researchers have examined specific approaches to treatment often giving substantial amounts of therapy and have obtained encouraging results; clinicians, with less time available, often use an eclectic approach mixing different treatment methods. The reasons for this difference are discussed. AIMS: We examined the effectiveness of an eclectic approach giving amounts of therapy more consistent wit
