Debulking procedures: potential device specific indications.

During the past decade, the endovascular treatment of peripheral arterial disease has become more widespread due to the impressive extension of the endovascular toolbox to include different atherectomy technologies. The promise of atherectomy is to avoid the "barotraumas" defined as complications of traditional angioplasty such as dissection, elastic recoil, and disruption of the internal elastic lamina resulting in overwhelming neointima and smooth muscle cell proliferation. The oldest atherectomy technologies are the excimer laser and the Rotablator. The excimer laser technology is based on …

Biological effect of orbital atherectomy and adjunctive paclitaxel-coated balloon therapy on vascular healing and drug retention: early experimental insights into the familial hypercholesterolaemic swine model of femoral artery stenosis.

AIMS: The efficacy of paclitaxel-coated balloons (PCB) for the treatment of superficial femoral artery (SFA) disease has been demonstrated in the clinical setting. Due to the high frequency of arterial calcification found in this vascular territory, the adjunctive use of atherectomy plus PCB has been proposed. In this study, we aimed to evaluate the biological effect on vascular healing and drug retention of this combination approach in the familial hypercholesterolaemic swine (FHS) model of femoral artery stenosis. METHODS AND RESULTS: Eleven femoral arteries (six superficial and five profun…

When Do Microbubbles Become Macrobubbles in Laser PCI?

During laser atherectomy, the use of contrast can lead to the formation of microbubbles, a common but potentially problematic occurrence. This can result in distal embolization and significant hemodynamic compromise. We would like to share a case of a 64-year-old female who had severe calcified in-stent restenosis with stent underexpansion, which was treated with excimer laser coronary atherectomy. The procedure was complicated by the formation of macrobubbles, which caused slow reflow with temporary hypotension and bradycardia. Laser atherectomy can cause macrobubble formation with distal emb…

Plaque debulking for femoro-popliteal occlusions: techniques and results.

Although currently there is a trend of using percutaneous transluminal angioplasty (PTA) and stenting for the treatment of long occlusions of superficial femoral artery, many studies reported comparable results in terms of mid- and long-term patency between PTA and stenting and plaque debulking techniques such as remote endarterectomy, directional atherectomy catheter atherectomy and laser guided atherectomy. A successful debulking procedure is strongly associated with patients comorbidities, length of lesions and clinical presentation. In the last decade many new devices have been proposed to…
