A case report: hemothorax caused by rupture of the left atrial appendage.

Cardiac rupture is defined as a full-thickness myocardial tear; this injury after blunt chest trauma is rare, and is associated with high mortality. Blunt cardiac rupture typically presents with either cardiac tamponade or massive hemothorax, and is often unrecognized in the context of blunt chest trauma. It is a little known fact that pericardial effusions can decrease due to pericardial lacerations. Hence, cardiac rupture with pericardial lacerations may be easily overlooked especially by chest surgeons. We herein report a case of hemothorax caused by rupture of the left atrial appendage. An…

Prevention and Management of Complications of Left Atrial Appendage Closure Devices.

Atrial fibrillation is associated with an ongoing risk of thromboembolic stroke and systemic embolism due to stasis and thrombus formation within the left atrial appendage (LAA). Transcatheter occlusion or ligation of the LAA represents a potential paradigm shift in the management of stroke prevention in at-risk patients with atrial fibrillation. This review summarizes the types and rates of procedural complications that have been observed with LAA occlusion and ligation; describes strategies that can be implemented to minimize these complications; and discusses management approaches that may …

Hemangioendothelioma of the right atrial appendage associated with pericardial effusion in an infant.

BACKGROUND: Cardiac hemangioma is a rare cause for pericardial effusion. We present a case of hemangioendothelioma of the right atrial appendage associated with pericardial effusion in an infant. METHODS: The patient was a 49-day-old infant transferred to our department because of the presence of pericardial effusion. Two-dimensional transthoracic echocardiography revealed moderate pericardial effusion and a 24 mm × 16 mm hypoechoic mass located on the right side of the right ventricular outflow tract and in front of the aorta. RESULTS: The infant underwent an exploratory median sternotomy. …

Treatment of congenital aneurysms of the left atrium and left atrial appendage.

We report 2 cases of congenital aneurysm, 1 of the left atrium and 1 of the left atrial appendage. The patients were 14 and 27 years of age, respectively. Their common symptoms were paroxysmal palpitations and dyspnea. Diagnoses were suggested by chest radiographic films that revealed prominent convexity at the upper left border of the heart and were confirmed by echocardiography, which demonstrated a large cystic mass attached to the left atrium in each case. Aneurysmectomy was performed through a median sternotomy or a left thoracotomy, with cardiopulmonary bypass in 1 patient and without it…

Thoracoscopic obliteration of the left atrial appendage: potential for stroke reduction?

BACKGROUND: In a review of all relevant articles describing the site of left atrial thrombus in patients with atrial fibrillation, the thrombus was localized to the left atrial appendage in 43% of patients with rheumatic heart disease and in 91% of patients with nonrheumatic atrial fibrillation. This study was designed to test the feasibility of thoracoscopic obliteration of the left atrial appendage as a means of reducing thromboembolic stroke. METHODS: Thoracoscopic obliteration of the left atrial appendage was undertaken in 10 dogs, 5 with staples and 5 with an endoloop. Obliteration also …

Left atrial appendage occlusion in patients with non-valvular atrial fibrillation and cerebral amyloid angiopathy: insights from the LOGIC (Left atrial appendage Occlusion in patients with Gastrointestinal or IntraCranial bleeding) international multicenter registry.

OBJECTIVES: Oral anticoagulation therapy (OAC) is often contraindicated in patients with atrial fibrillation (AF) and cerebral amyloid angiopathy (CAA) because of the high hemorrhagic risk. Left atrial appendage occlusion (LAAO) can prevent thromboembolic events while avoiding long-term anticoagulation. However, a short period of antithrombotic therapy (AT) is still recommended after LAAO, and, therefore, it is unclear whether patients with CAA can be candidates for LAAO. The aim of the study was to investigate the safety and efficacy of LAAO in patients with CAA and AF. METHODS: In this sub-…

Left Atrial Appendage Occlusion vs Oral Anticoagulation for Stroke Prevention in Atrial Fibrillation: A Meta-Analysis.

BACKGROUND: Left atrial appendage occlusion (LAAO) has emerged as a non-pharmacological alternative to oral anticoagulation (OAC) for stroke prevention in patients with atrial fibrillation (AF). Following the publication of recent CHAMPION-AF and CLOSURE-AF trials, an updated synthesis of evidence is warranted to clarify the long-term comparative efficacy and safety of LAAO versus OAC in patients with AF. METHODS: We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) comparing LAAO with OAC in adults with AF. A comprehensive search of major databases was pe…

Prevalence of left atrial appendage thrombi and associated risks in patients referred for atrial flutter ablation.

INTRODUCTION AND OBJECTIVES: Atrial flutter (AFL) is a common supraventricular arrhythmia, often associated with an increased risk of thromboembolic events. Current guidelines recommend catheter ablation of the cavotricuspid isthmus as first-line therapy. However, thrombotic risk assessment remains essential, particularly through transesophageal echocardiography (TEE) to detect left atrial appendage (LAA) thrombi prior to ablation. This study aimed to determine the prevalence of LAA thrombi in patients with AFL undergoing pre-ablation TEE and to identify clinical, laboratory, and echocardiogra…

Cardiac MRI for Preprocedural Assessment in Participants with Nonvalvular Atrial Fibrillation Undergoing Left Atrial Appendage Closure.

Purpose To evaluate the feasibility of cardiac MRI for preprocedural left atrial appendage closure (LAAC) planning and to compare cardiac MRI- and transesophageal echocardiography (TEE)-derived measurements. Materials and Methods Patients with nonvalvular atrial fibrillation who were suitable candidates for LAAC between October 2019 and July 2023 were included in this prospective single-center study. Participants underwent preprocedural TEE and cardiac MRI for the measurement of LAA ostium diameter and depth at four angles (0°, 45°, 90°, and 135°), and measurements were compared between modali…

Left Atrial Appendage Thrombus in Patients With Hypertrophic Cardiomyopathy and Atrial Fibrillation: Prevalence and Risk Factors.

BACKGROUND: Atrial fibrillation (AF) is common in hypertrophic cardiomyopathy (HCM) and increases stroke risk, primarily due to thromboembolism from the left atrial appendage (LAA). Oral anticoagulation (OAC) is recommended, but data on LAA thrombus (LAAT) in HCM and AF are limited. The current study aimed to assess LAAT prevalence in patients with HCM and AF. METHODS: We retrospectively analyzed 170 patients with HCM and AF (mean age, 67.7±12.4 years; 57.1% men) who underwent at least one transesophageal echocardiogram (TEE) during a median follow-up of 41.5 months (interquartile range, 15-7…

Left Atrial Appendage Occlusion Versus Medical Therapy in Atrial Fibrillation: A Systematic Review and Meta-Analysis.

Background: Percutaneous left atrial appendage occlusion (LAAO) is an established nonpharmacologic strategy for stroke prevention in atrial fibrillation (AF). Its comparative effectiveness and safety relative to contemporary medical therapy, including direct oral anticoagulants (DOACs), remain uncertain following recent large randomized controlled trials (RCTs). Methods: We performed a systematic review and meta-analysis of RCTs comparing catheter-based LAAO with medical therapy in AF patients. PubMed, CENTRAL, and ScienceDirect were searched from inception through May 2026. Dichotomous outcom…

Left Atrial Appendage Closure vs Direct Oral Anticoagulants in Atrial Fibrillation: Meta-Analysis of Randomized Trials.

BACKGROUND: Left atrial appendage closure (LAAC) is a potential alternative to oral anticoagulation for stroke prevention in atrial fibrillation. OBJECTIVES: We performed an updated meta-analysis of randomized controlled trials comparing LAAC with direct oral anticoagulants (DOACs) for efficacy and safety. METHODS: We systematically searched from January 1, 2012, to April 3, 2026, for randomized controlled trials comparing percutaneous LAAC with DOACs in patients with atrial fibrillation. Using the longest available follow-up from each trial, we pooled efficacy and bleeding outcomes, includi…

Atrial fibrillation and coexisting cerebral amyloid angiopathy: safety and efficacy of transcatheter left atrial appendage occlusion.

BACKGROUND: To investigate the safety and efficacy of transcatheter left atrial appendage occlusion (LAAO) in patients with concomitant atrial fibrillation (AF) and cerebral amyloid angiopathy (CAA) and high bleeding risk that precludes long-term oral anticoagulation. METHODS: Consecutive patients with non-valvular AF and a CHA2DS2-VASc score ≥ 2 and concomitant CAA who underwent transcatheter LAAO between 2014 and 2022 at Parma University Hospital were included in the analysis. The primary endpoint was a composite of any ischemic/hemorrhagic events. The observed rates of ischemic and bleeding…

Comparative analysis of left atrial size and appendage morphology in paroxysmal and persistent atrial fibrillation patients.

PURPOSE: Pulmonary vein isolation (PVI) is effective in treating atrial fibrillation (AF), but outcomes are worse for persistent AF (persAF) patients than paroxysmal AF (PAF) patients. The study aimed to identify differences in left atrial (LA) and left atrial appendage (LAA) anatomy in different AF types. METHODS: In a single-center observational study, a blinded retrospective analysis of preprocedural cardiac computed tomography angiography (CCTA) images was performed. The study evaluated the dimensions of the LA and pulmonary veins (PV), as well as the size and morphology of the LAA using …

Outcomes of Left Atrial Appendage Closure in Hemodialysis Patients With Atrial Fibrillation: The OCEAN-LAAC Registry.

BACKGROUND: Left atrial appendage closure (LAAC) has demonstrated favorable outcomes as an alternative to permanent anticoagulation in patients with nonvalvular atrial fibrillation (NVAF). In hemodialysis (HD) patients with NVAF, anticoagulation increases bleeding complications, with inconsistent benefits in stroke prevention. OBJECTIVES: This study aimed to clarify the benefit of LAAC for HD patients. METHODS: Consecutive patients who underwent LAAC, as extracted from the Japanese multicenter registry, were eligible. When comparing HD and non-HD patients, perioperative events, including LAA…
