Air crash survival: injuries and evacuation toxic hazards.

Carbon monoxide and cyanide gas have resulted from inflight or post-crash fires which have pyrolyzed certain cabin interior materials. Three air carrier accidents have been studied in depth from the standpoint of carbon monoxide effects on passengers. Post-crash hydrogen cyanide in victim blood levels has similarly been investigated and certain laboratory synergistic studies of carbon monoxide and hydrogen cyanide have been conducted. The latter investigations indicate that a combination of carbon monoxide and hydrogen cyanide has a pharmacologic effect which exceeds that of either alone. The

Cardiomyopathy--the frequently forgotten mimic: clinical and open-chest myocardial biopsy studies.

Attention is drawn to the cardiomyopathies, 25 idiopathic cases being presented and the features noted which may mimic other conditions, especially coronary heart disease. Subtle clinical manifestations may be misinterpreted unless these disorders are kept in mind. The diagnosis can usually be suspected at the bedside but specialized tests may be required to confirm it. Open-chest myocardial biopsy has yielded exciting, but not yet apparently useful, results in six instances. Recognition of cardiomyopathy in aviation cardiology is important because of the high incidence of cardiac arrhythmia a…

Status and cockpit dynamics: a review and empirical study.

One of the most troublesome dynamics evident in the airplane cockpit is related to patterns of authority relations between the captain and the first officer: Too often, captains fail to listen and first officers fail to speak. The authors propose that many instances of superordinate and subordinate behavior in the cockpit--the captain's tendency to reject input from other team members and the first officer's hesitancy to question the captain--represent cases of status generalization. First, the authors describe the theory of status generalization and show support for the operation of the theor

Cases from the aerospace medicine residents' teaching file. Case #39. Ménière's disease.

A case of Ménière's disease in an Air Force pilot is presented. The disease is reviewed in a question and answer format. A record review of all aviators evaluated at USAFSAM during the last 10 years revealed: 1. Only 11 cases of possible Ménière's disease were referred to USAFSAM in the years 1979-88. 2. 100% of fliers with Meniere's disease were recommended to be permanently disqualified from flying status regardless of the treatment or the results. 3. A single case of cochlear hydrops was returned to flying status after one year of observation.

[Patient safety and root cause analysis].

Safety in the field of transfusion medicine has greatly improved in France. The risk of viral transmission has decreased by a factor greater than 1500 within the last 20 years. In comparison, the risk related to ABO error has decreased only by half. The reporting of critical incidents, which occur at any step of the transfusion procedure is now mandatory in France and is subject to an in-depth analysis, using methods close to that used in aviation safety. The goal of these analyses is to better understand human factors in order to implement more adequate prevention measures.

Are your engineers talking to one another when they should?

Communication may not be on managers' minds at companies that design complex, highly engineered products, but it should be. When mistakes take place, it's often because product-component teams fail to talk. The consequences can be huge: Ford and Bridgestone Firestone lost billions by not coordinating the design of the Explorer with the design of its tires. The major delays and cost overruns involved in the development of Airbus's A380 "superjumbo"--which most likely led to the CEO's exit--were a result of unforeseen design incompatibilities. To help managers mitigate such problems, the authors

The ejection experience of the Royal Australian Air Force: 1951-92.

This paper reviews the ejection experience of the Royal Australian Air Force (RAAF) between 1951 and 1992. During this period 84 aircrew ejected from their aircraft, with 77 survivors and 7 fatalities. The causes of these ejections were examined, and the injuries received by the ejectees were classified into four categories: fatal, major, minor, and no injuries. Vertebral fractures were the most common type of major injury (35%). Most ejectees received at least one minor injury. There were 7 reported cases in which the ejectee received no injuries. The RAAF ejection experience was found to be

[Back ache in helicopter pilots].

Due to low back pain (LBP) and harmful effects of flying, questionnaires were sent to 71 helicopter pilots of the experimental group, 22 mechanics helicopter flyers and to the control group of 28 air-traffic controllers. The prevalence of LBP was the highest in helicopter pilots, then in helicomechanics and air-traffic controllers (53%, 50% and 36%). Effects of exposure to vibration, body posture and working load have not contributed significantly to the occurrence of LBP. LBP has not lead to an important difference in the strength of the back musculature, body mass index and spondylosis, that

The 'problem' with automation: inappropriate feedback and interaction, not 'over-automation'.

As automation increasingly takes its place in industry, especially high risk industry, it is often blamed for causing harm and increasing the chance of human error when failures do occur. I propose that the problem is not the presence of automation, but rather its inappropriate design. The problem is that the operations under normal operating conditions are performed appropriately, but there is inadequate feedback and interaction with the humans who must control the overall conduct of the task. When the situations exceed the capabilities of the automatic equipment, then the inadequate feedback

Transportation noise exposure and incident type 2 diabetes: A retrospective cohort study in a large U.S. healthcare system.

OBJECTIVE: Transportation noise is increasingly recognized as a cardiometabolic stressor, but its relationship with type 2 diabetes mellitus … Transportation National Transportation Noise Map. Five noise categories were examined: Road, Rail, Aviation, Road plus Aviation and Total. Cox proportional hazards models estimated associations across predefined categories, Quiet (≤45 dB), Moderate (45-54 dB) and Loud (≥55 dB), and per 10 dB increase, adjusting for demographics, cardiometabolic risk facto …
