[A biomechanical study of stability of atlantoaxial junction fixation with anterior approach screw fixation through C2 vertebral body to C1 lateral mass and Gallie's technique].

OBJECTIVE: To determine the three-dimensional stability of atlantoaxial reconstruction with anterior approach screw fixation through C2 vertebral body to C1 lateral mass and Gallie's technique (ASMG) for C1,2 instability. METHODS: Twenty-five human cadaveric specimens (C0-3 ) were divided randomly into 5 groups (n = 5). The three-dimensional ranges of motion C relative to C2 were measured under the five different conditions: the intact state group (group A), type II odontoid fracture group (group B), posterior C1,2 transarticular screw fixation group (group C), ASM group (group D) and ASMG gr

How to diagnose axial spondyloarthritis early.

BACKGROUND: Chronic low back pain (LBP), the leading symptom of ankylosing spondylitis (AS) and undifferentiated axial spondyloarthritis (SpA), precedes the development of radiographic sacroiliitis, sometimes by many years. OBJECTIVE: To assign disease probabilities and to develop an algorithm to help in the early diagnosis of axial SpA. METHODS: Axial SpA comprises AS and undifferentiated SpA with predominant axial involvement. Clinical features include inflammatory back pain (IBP), alternating buttock pain, enthesitis, arthritis, dactylitis, acute anterior uveitis, a positive family histor

Congenital spondylolysis of the axis with associated myelopathy. Case report.

Cervical spondylolysis is a rare clinical entity and occurs predominantly at the C-6 level. The authors describe a patient with congenital spondylolysis of the axis that caused myelopathy. The patient was a 57-year-old woman with long-standing gait disturbance. Plain cervical radiography revealed a radiolucent defect across the pedicle of the axis. Magnetic resonance imaging of the cervical spine in the neutral, flexion, and extension neck positions as well as a computerized tomography myelography in the neutral neck position failed to demonstrate any spinal cord compression. When she rotated

Anomaly of the axis causing cervical myelopathy. Case report.

Although the craniovertebral junction is one of the most common sites at which anomalies develop, spina bifida occulta of the axis (C-2) associated with cervical myelopathy is extremely rare. The authors present the case of a 46-year-old man who developed progressive tetraparesis caused by a cervical canal stenosis at the level of the axis. The spinal cord was compressed by an invaginated bifid lamina of the axis. The patient made a remarkable recovery after undergoing decompressive laminectomy of C-3 and removal of the bifid posterior arch of the axis.

[Atloaxoid dislocation with basilar pressure and spinal cord symptoms in rheumatoid arthritis. A case of occipitocervical arthrodesis followed by resection of the odontoid].

The authors report a case of atlanto-axial dislocation in a patient with rheumatoid arthritis. There was some neurological impairment with upper limb pain and hyperreflexia. As a first stage, a cranio-cervical fusion was performed through a posterior approach. Three weeks later the axis was excised through an antero-lateral approach, together with its odontoid process. The patient's condition was much improved and the neurological lesions disappeared.

Evaluating differences in the upper instrumented vertebra (UIV) tilt angle between adolescent idiopathic scoliosis patients with Lenke 1-ve (flexible) and Lenke 1 +ve (stiff) curves.

PURPOSE: To analyse the mean optimal upper instrumented vertebra (UIV) tilt angle in Lenke 1 subgroups with UIV at T2, T3, and T4, as derived from preoperative supervised cervical supine side-bending radiographs. METHODS: This retrospective study included 288 Lenke 1 adolescent idiopathic scoliosis (AIS) patients who underwent posterior spinal fusion between 2020 and 2023, with UIV selected at T2, T3, or T4 levels. All patients were categorised into two subgroups: Lenke 1-ve (flexible) (proximal thoracic side bending (PTSB) Cobb angle: <15°) and Lenke 1 + ve (stiff) (PTSB Cobb angle: 15°-24.9

Vertebral Arteriovenous Fistula: A Rare Complication Following Transpedicular Occipitocervical Fixation in a Patient with Atlantoaxial Dislocation.

Vertebral arteriovenous fistula (AVF), a complication of a vertebral artery injury (VAI), is a rare but serious complication of upper cervical spine fixation surgery. We report a case of a 59-year-old female patient who had a vertebral AVF following transpedicular occipitocervical fixation surgery. Endovascular embolization of the AVF was successfully performed using ethylene vinyl alcohol. From this case we learned that preoperative evaluation of the course of the vertebral artery is necessary, and vertebral artery embolism is an effective and safe method to treat vertebral AVF after proof of

Extracranial aneurysms of the posterior inferior cerebellar artery. Literature review and report of a new case.

Aneurysms arising from the extracranial portion of the posterior-inferior cerebellar artery (PICA) are considered extremely rare. To date, only ten cases have been reported in the literature. The authors report a case of a 29 year-old male patient admitted comatose in Hunt-Hess grade IV because of an subarachnoid haemorrhage predominantly involving the left cranio-vertebral junction up to C2 rather than the posterior fossa and the fourth ventricle. Angiography showed an extracranial aneurysm at left vertebral artery-PICA junction a few millimetres superior to the dural entry point of the verte

Fractures of the odontoid: a laboratory and clinical study of mechanisms.

Previous reports and a study of the mechanism of injury in 25 cases of fracture of the odontoid suggest that the fracture usually results from a high velocity force directed to the head, producing flexion, extension, or rotation of the upper cervical spine. In a laboratory biomechanical study using cadaver preparations, fractures of the odontoid were produced in 20 specimens from the upper cervical spine, which were loaded in failure in forward and lateral modes.

Fracture of the odontoid process.

A review of 26 cases of odontoid fractures has been carried out. Late diagnosis and consequent delay in treatment were found to be important causes of non-union. Myelopathy in odontoid fractures is of two types. One type is that present immediately after injury, while the other is delayed. In treatment of long-standing cases, only fusion without posterior decompression should be done. If the latter procedure is carried out, it can produce medullospinal haematomyelia with respiratory arrest and tetraplegia. In the performance of atlanto-axial fusion, the technique of bone grafting and the use o

Surgical management of remote, isolated type II odontoid fractures with atlantoaxial dislocation causing cervical compressive myelopathy.

OBJECTIVE: The remote (more than 6 mo after injury) and isolated (not associated with any other cervical spinal fractures) Type II fractures of the odontoid (RI IIO) are unique in being inherently unstable and prone to malunion or nonunion, leading to cervical compressive myelopathy. The present study discusses their surgical management. METHODS: Nineteen patients with RI IIO with atlantoaxial dislocation (AAD) causing compressive myelopathy were treated. Their preoperative disability was graded as Grade I: neurologically intact (presented with hyperreflexia and mild spasticity; n = 3); Grade

Is the chin-brow vertical angle always a reliable parameter for surgical planning in the correction of thoracolumbar kyphosis secondary to ankylosing spondylitis?

OBJECTIVE: The aim of this retrospective study was to investigate the correlation between chin-brow vertical angle (CBVA) and osteotomized vertebra angle (OVA) across diverse cervical ranges of motion (CROMs) and to examine the reliability of CBVA in surgical correction design. METHODS: The authors analyzed all ankylosing spondylitis (AS) patients with thoracolumbar kyphosis who had undergone single-level lumbar or thoracic pedicle subtraction osteotomy between January 2015 and December 2019 and had at least 2 years of follow-up. The patients were categorized into 3 groups based on their CROM

Trends in Outcomes of a Prospective Consecutively Enrolled Single-Center Adult Cervical Deformity Series.

STUDY DESIGN: Retrospective cohort. OBJECTIVE: The aim was to describe the two-year outcomes for patients undergoing surgical correction of cervical deformity (CD). BACKGROUND: Adult CD has been shown to compromise health-related quality of life. While advances in spinal realignment have shown promising short-term clinical results in this parameter, the long-term outcomes of CD corrective surgery remain unclear. MATERIALS AND METHODS: Operative CD patients >18 years with two-year (2Y) health-related quality of life/radiographic data were included. Improvement in radiographic, neurologic, an

C2-fractures: part I. Quantitative morphology of the C2 vertebra is a prerequisite for the radiographic assessment of posttraumatic C2-alignment and the investigation of clinical outcomes.

Pertinent literature exists concerning indications, techniques, complications of treatment, and risk factors for nonunion in axis and odontoid fractures; however, there are scarce data regarding the incidence and definition of malunion in these fractures. As a prerequisite for the study of anatomical alignment following surgical and nonsurgical treatment of C2-fractures, an understanding of normal C2 anatomy is essential. Therefore, the authors intended to evaluate morphometrical dimensions of the C2 vertebra. The purpose was to provide normalized quantitative data to enable assessment of mala

Machine learning analysis of cervical balance in early-onset scoliosis post-growing rod surgery: a case-control study.

We aimed to analyze the cervical sagittal alignment change following the growing rod treatment in early-onset scoliosis (EOS) and identify the risk factors of sagittal cervical imbalance after growing-rod surgery of machine learning. EOS patients from our centre between 2007 and 2019 were retrospectively reviewed. Radiographic parameters include the cervical lordosis (CL), T1 slope, C2-C7 sagittal vertical axis (C2-7 SVA), primary curve Cobb angle, thoracic kyphosis (TK), C7-S1 sagittal vertical axis (C7-S1 SVA) and proximal junctional angle (PJA) were evaluated preoperatively, postoperatively

Epidural hematoma after immobilization of a "hangman's" fracture: case report and review of the literature.

BACKGROUND CONTEXT: Neurologic deterioration after immobilization of traumatic spondylolisthesis of the axis rarely occurs because of the decompressive nature of the injury itself and the large amount of space available for the cord in the upper cervical spine. PURPOSE: To document neurologic deterioration after reduction (without the use of traction) and halo immobilization of a Type IIA traumatic spondylolisthesis of the axis (hangman's fracture) secondary to an epidural hematoma. STUDY DESIGN/SETTING: Case report. PATIENT SAMPLE AND OUTCOME MEASURES: The patient population consisted of on
