Role of encapsulated anaerobic bacteria in synergistic infections.

The effect of encapsulation on the virulence, survival, and protection of anaerobic bacteria from phagocytosis is reviewed. Support for the importance of encapsulated Bacteroides sp. and anaerobic and facultative Gram-positive cocci (AFGPC) was provided by their higher recovery rate in oropharyngeal infections, compared to their number in the normal oral flora. Studies of the pathogenicity of anaerobic bacteria of the Bacteroides, Fusobacterium, and Clostridium genera and AFGPC are also presented. The organisms were inoculated into mice and their ability to induce subcutaneous abscesses was de…

Metronidazole therapy of anaerobic bacteremia, meningitis, and brain abscess.

Four patients with Bacteroides fragilis bacteremia, one patient with a brain abscess due to Bacteroides species, Fusobacterium naviforme, and Peptostreptococcus species, and an infant with Bacteroides species ventriculitis and meningitis were treated with metronidazole. In all cases the anaerobic pathogens were eradicated. Five of the six patients recovered. One patient with leukemia in whom B fragilis bacteremia was eradicated by metronidazole treatment subsequently died of Pseudomonas aeruginosa bacteremia. Ventricular fluid and serum concentrations of metronidazole were determined in the ca…

Effect of aspoxicillin on anaerobic bacteria.

Aspoxicillin (ASPC), a semisynthetic penicillin has a broad spectrum of antibacterial activities against Gram-positive and Gram-negative anaerobic bacteria. Its in vitro antibacterial activity was less than those of cefoxitin against Peptostreptococcus and Veillonella, but was significantly high against Bacteroides fragilis, one of the most clinically important anaerobe. The therapeutic and/or protective effect of ASPC in experimental subcutaneous abscess or experimental intraabdominal mixed infection due to beta-lactamase producing B. fragilis and non-producing Escherichia coli were much stro…

Time to reconsider moxifloxacin anti-anaerobic activity?

Large data on moxifloxacin activity on anaerobes are old. Moxifloxacin is often used for empiric therapy therefore the knowledge of current epidemiologic data is fundamental. We analyzed 69 anaerobic strains, from a recent multicenter Italian study, for moxifloxacin susceptibility. Using EUCAST criteria 81% of Bacteroides spp. and 48% of anaerobes other than Bacteroides were resistant to moxifloxacin. Using CLSI criteria moxifloxacin resistance rates decrease to 35% for all anaerobes, and to 41% for Bacteroides spp. We reported an alarming increase in moxifloxacin resistance among anaerobes in…

In vitro activity of azithromycin and erythromycin against organisms associated with bacterial vaginosis and chancroid.

The in vitro activity of the new macrolide azithromycin and erythromycin against Haemophilus ducreyi, Gardnerella vaginalis, Bacteroides spp. and Mobiluncus spp. was compared. Gardnerella vaginalis, Mobiluncus spp. and Haemophilus ducreyi were highly susceptible to both agents but azithromycin was more effective overall against all three groups of organisms. The activity of both antibiotics against Bacteroides spp. varied greatly, but azithromycin was again more active. Collectively, 62% of all Bacteroides strains were susceptible to erythromycin (MIC 1 mg/1) and 74% were susceptible to azithr…

Use of bacteriology in anorectal sepsis as an indicator of anal fistula: experience in a distinct general hospital.

The bacteriology of anorectal sepsis in a district general hospital has been reviewed to see whether the information gained helped patient management. Forty-six patients with anorectal sepsis were reviewed, underlying fistulas were identified in nine patients (19.5%). Bacteroides species were not routinely subtyped in the department. It was found that isolation of unspecified Bacteroides species was not helpful in identifying those patients with underlying fistulas. Unless subtyping of Bacteroides species is specifically requested, pus from anorectal sepsis should not routinely be sent for bac…

Iron: an essential micronutrient for the legume-rhizobium symbiosis.

Legumes, which develop a symbiosis with nitrogen-fixing bacteria, have an increased demand for iron. Iron is required for the synthesis of iron-containing proteins in the host, including the highly abundant leghemoglobin, and in bacteroids for nitrogenase and cytochromes of the electron transport chain. Deficiencies in iron can affect initiation and development of the nodule. Within root cells, iron is chelated with organic acids such as citrate and nicotianamine and distributed to other parts of the plant. Transport to the nitrogen-fixing bacteroids in infected cells of nodules is more compli

Antimicrobial resistance in anaerobes.

The development of antibiotic resistance in anaerobic bacteria has a tremendous impact on the selection of antimicrobial agents for empirical therapy. Susceptibility studies have documented the emergence of antimicrobial resistance and indicate distinct differences in resistance patterns related to individual hospitals, geographic regions, and antibiotic-prescribing regimens. Resistance to beta-lactam drugs, clindamycin, tetracyclines, and 5-nitroimidazoles (metronidazole) has been observed. The prime mechanism for resistance to beta-lactam agents is the production of beta-lactamases. Resistan

The in vitro and in vivo activity of taurolin against anaerobic pathogenic organisms.

An antibacterial agent, taurolin, formed by the combination of 2 molecules of taurine with 3 molecules of formaldehyde has been tested against anaerobes in vitro and in vivo. In vitro, taurolin is highly active against Bacteroides species and anaerobic Streptococci as measured by the minimum inhibitory concentration. In vivo, small dosages of taurolin given intraperitoneally increase the mortality in mice with Bacteroides peritonitis. However, by increasing the dosage of taurolin, survival is prolonged and mortality reduced. Intravenously administered taurolin protects rabbits from the effects…

[The effect of nitrate and nitrite on the respiration and cytochrome system of Rhizobium lupini].

Differential spectrophotometry and oxygen electrode were used to study the effect of nitrate and nitrite on the cytochrome system and respiration of Rhizobium lupini cultivated in the conditions of different aeration, and bacteroids of this strain isolated from lupine nodules. In the anaerobic conditions, nitrate (10(-3) M) accepts electrons from the cytochrome system in suspensions of bacteroids and bacterial cells grown in the microaerophilic conditions (12 muM O2), but does not oxidize cytochromes of the cells cultivated in the aerobic conditions (240 muM 32). Nitrate (10(-3) M) oxidizes ac

Reaction of the vaginal flora to ornidazol in patients with cervicitis.

The cervical and high vaginal flora of 76 patients with cervicitis were studied before and after therapy with Ornidazol by quantitative culture methods. Lactobacilli were the predominant organisms, but Peptostreptococci, Bacteroides and Trichomonas were encountered in 17, respectively 32 and 81% of all specimens. During and after therapy Trichomonas disappeared completely, the bacterial flora normalized and became comparable to that of healthy women with incidences for Bacteroides of 8-13% and Peptostreptococci of 4-5%. The in vitro susceptibility (MIC and MBC) of 50 strains of Bacteroides to …

Epidemiology and antimicrobial susceptibilities of wound isolates of obligate anaerobes from combat casualties.

Data from recent conflicts related to war wounds and obligate anaerobes are limited. We define the epidemiology and antimicrobial susceptibility of obligate anaerobes from Iraq and Afghanistan casualties (6/2009-12/2013), as well as their association with clinical outcomes. Susceptibility against eleven antibiotics (7 classes) was tested. Overall, 59 patients had 119 obligate anaerobes identified (83 were first isolates). Obligate anaerobes were isolated 7-13 days post-injury, primarily from lower extremity wounds (43%), and were largely Bacteroides spp. (42%) and Clostridium spp. (19%). Patie…

Mechanism of antimicrobial resistance and resistance transfer in anaerobic bacteria.

The antimicrobial susceptibility pattern of anaerobic bacteria has been changing over the past decade. This paper reviews the mechanisms by which these organisms have become resistant to selected antibiotics and reviews data demonstrating that Bacteroides fragilis and Clostridium perfringens possess systems for transferring resistance determinants. Within bacteroides there is widespread resistance to penicillins, cephalosporins and tetracycline compounds while there have been reports of resistance to clindamycin and cefoxitin, and there is rare resistance reported for chloramphenicol and metro…

IgG antibodies against anaerobic bacteria in blood donor sera.

Fifty-six blood donor sera were examined by indirect immunofluorescence for IgG antibodies to a selection of anaerobic bacterial strains, often isolated from wound cultures. Another 25 sera were examined by IgG antibodies to six NCTC Bacteroides strains. A wide range of IgG antibody titres were found against Fusobacterium, Clostridium, and anaerobic streptococcal species. Very low titres were found against the Bacteroides strains. It is suggested that the testing of a single serum specimen for IgG antibodies against a representative collection of Bacteroides fragilis strains could be helpful i…

Comparison of in vivo and in vitro efficacy of ceftizoxime.

Two hundred obligately anaerobic bacterial isolates from human clinical sources were tested for susceptibility to ceftizoxime using a standard National Committee for Clinical Laboratory Standards microwell system. In general, the minimal inhibitory concentrations (MIC50) ranged from less than 0.125 to 32 microgram/ml; however, the MIC50 for Bacteroides fragilis averaged greater than 128 microgram/ml. This finding is inconsistent with the results of in vivo testing of ceftizoxime in an animal model of intra-abdominal sepsis (B fragilis is a major contributor to the development of intra-abdomina…
