The bicuspid aortic valve (BAV) is the most common congenital cardiac malformation. A literature search was performed using the key words 'bicuspid aortic valve', 'pathophysiology', 'exercise' and 'training'. BAV is the result of a complex developmental process where several genes seem to lead to abnormal valvulogenesis. Complications associated with BAV include aortic stenosis (AS) and regurgitation, infective endocarditis and aortic dilation and dissection. Moreover, BAV may be associated with other cardiovascular anomalies, mainly aortic coarctation. There is greater awareness of BAV in the…

OBJECTIVE: In general, classification of a disease has proven to be advantageous for disease management. This may also be valid for the bicuspid aortic valve, because the term "bicuspid aortic valve" stands for a common congenital aortic valve malformation with heterogeneous morphologic phenotypes and function resulting in different treatment strategies. We attempted to establish a classification system based on a 5-year data collection of surgical specimens.
METHODS: Between 1999 and 2003 a precise description of valve pathology was obtained from operative reports of 304 patients with a dise…

BACKGROUND: There is increasing interest in the role of valve repair for patients with isolated severe aortic regurgitation. Those with bicuspid aortic valves are suggested as most suitable for repair. Morphologic features of these valves that suggest feasibility of repair are not well defined.
METHODS: Perioperative echocardiograms on 132 consecutive patients (mean age 42 +/- 12 years; 94% male), with bicuspid valves and isolated aortic regurgitation undergoing surgery at our institution were reviewed. Seventy-five patients (57%) underwent successful valve repair. Repair was attempted but un…

OBJECTIVES: Aortic dilatation occurs in many patients with bicuspid aortic valves. We have added root replacement using the remodeling technique originally designed for tricuspid aortic valves to bicuspid aortic valve repair for treatment of the dilated root. We compared the results of remodeling in bicuspid aortic valves with those in tricuspid aortic valves.
METHODS: From October 1995 through January 2004, 60 patients underwent root remodeling for bicuspid aortic valves (group A), and 130 patients underwent root remodeling for tricuspid aortic valves (group B). Correction of cusp prolapse w…

We weighed each cusp separately in 200 operatively excised stenotic congenitally bicuspid aortic valves. It was found in 152 patients (76%) that the cusps differed (by > 0.2 g) in weight and in 48 patients (24%) that the cusps were of similar (a < or = 0.2 g difference) weights. In 161 of the 200 patients (80%), raphes were present in 1 cusp; the raphe and nonraphe cusps differed in weight in 120 patients (74%), with the raphe cusps being larger in 89 patients (74%), and were of similar weights in 41 patients (26%). Of the 39 patients with raphes in neither cusp, 32 cusps (82%) differed in wei…

BACKGROUND: Prosthodontic and orthodontic procedures have been described for the treatment of patients with traumatic loss of teeth.
PATIENT AND METHODS: Both upper central incisors and the upper right lateral incisor had to be extracted in an 8-year-old patient after failure of conservative procedures following a sports accident. The patient was treated on a temporary basis with a space retainer as a substitute for the teeth. Definitive therapy was carried out by transplanting a lower bicuspid germ into the upper arch with subsequent orthodontic gap closure.
RESULTS: 8 years after the inter…

Pure aortic regurgitation is usually caused by a congenital bicuspid aortic valve (BAV). In this condition, there are a few cases accompanied by an abnormal fibrous band stretched from the center of the conjoined cusp to the aortic wall. This report describes two cases of congenital bicuspid aortic valve (BAV) with an abnormal fibrous band. The first case of BAV with an abnormal fibrous band developed aortic regurgitation and the second case exhibited aortic stenosis. These two abnormal fibrous bands were confirmed to be congenital by the microscopic findings. According to the literature, the …

Cardiac anomalies are usually diagnosed early in life, which is particularly true for their various combinations. The diagnosis in adulthood is rare. Here we report the case of a young man with an aortic coarctation corrected at the age of 16, however the associated stenotic bicuspid aortic valve and cor triatriatum sinistrum were corrected after Streptococcus viridans endocarditis 7 years later.

From January, 1978 to December, 1990, 85 patients with congenital bicuspid aortic valve underwent aortic valve replacements (AVR) with St. Jude Medical valve prosthesis. We classified congenital bicuspid aortic valve into four types. Type I (44.7%): Two cusps are situated right and left, a coronary artery arises from each related sinus of valsalva. Type II (22.4%): Type I + raphe in the right cusp. Type III (3.5%): one cusp is located anteriorly, the other posteriorly and both coronary arteries arise from anterior cusp. Type IV (29.4%): Type III + raphe in the anterior cusp. Regarding to preop…

Twelve patients (age 18 to 69 years old) with surgical treatment for congenital bicuspid aortic valve and one (age 26 years old) for quadricuspid aortic valve were reported. These 13 patients occupied 12.3% of the total number of patients with aortic valve replacement during the same period. Among the patients with bicuspid aortic valve, 10 patients were male (83%) and 2 were female (17%). Seven patients exhibited aortic stenosis and 5 showed predominant aortic regurgitation. The mean age of patients with aortic stenosis was higher than that of patients with aortic regurgitation. (63.3 years v…

A patient with an aneurysm of the ascending aorta and calcific stenosis of a congenital bicuspid aortic valve, whose brother also had a stenotic congenital bicuspid aortic valve, is described. Predominant aortic stenosis at cardiac catheterization and the presence of an aneurysm distal to and not including the aortic valvular ring made the initial diagnosis of Marfan's syndrome unlikely. Cystic medial necrosis present in the aneurysmal wall probably arose as a consequence of poststenotic dilation. Adequate noninvasive evaluation of the ascending aorta requires echocardiographic studies, as wel…

BACKGROUND: Bicuspid aortic valve (BAV) is a common congenital condition associated with progressive aortopathy and remains a frequent justification for sporting restriction despite limited data on exercise-related aortic events.
CASE SUMMARY: An asymptomatic 18-year-old athlete undergoing evaluation for military service was discovered to possess a type-1 BAV, accompanied by a dilated aortic root (4.2-4.3 cm), with trivial to mild aortic regurgitation. Serial imaging demonstrated rapid progression of aortic dilatation to 4.7 cm. Such findings posed significant implications for military eligib…

Bicuspidization is a valid option for unicuspid aortic valve repair, in which creating symmetrical commissural orientation is essential for improved outcomes. However, the right coronary orifice often interferes with symmetrical attachment of the neocommissure. In a paediatric patient without aortic root dilation, we rotated the right coronary orifice clockwise by cutting out a triangular piece of the non-coronary sinus wall and augmenting it between the left and right coronary sinuses. A neocommissure with patching was sewn to the left side of the right coronary orifice, and the symmetrical b…
