Choledochoduodenal fistula (CDF) is an abnormal communication between the choledochus and the duodenum, accounts for 5-25% of all internal biliary fistulas. Here, we report a case of CDF secondary to chronic duodenal ulcer who presented with cholangitis. CDF is suspected in case of pneumobilia, and surgery is recommended for refractory cases.

Injuries of the biliary tract remain an important concomitant lesion following liver trauma. Intra- and extrahepatic bile duct injuries differ substantially in etiology and choice of surgical procedure. Intrahepatic bile duct injuries gain clinical relevance postoperatively following primary surgical liver treatment. Characteristic consequences of intrahepatic bile duct injuries are: infected necrosis, abscess, bilious fistula, bilhemia, hemobilia and biliary duct stricture. In contrast, concomitant organ or vessel injury are of primary importance in extrahepatic biliary duct injury. Lesions o

Under analysis were 235 repeated operations upon the external bile-excretory ducts, fulfilled in 218 patients. The main indications for repeated operations on the biliary ducts were choledocholithiasis, stenosis of Vater's papilla, stricture of biliary ducts, pancreatitis, external biliary fistula. The pathological conditions were most frequently observed in combinations. During repeated interventions choledochotomy and revision of the biliary ducts were made in 182 patients. Biliodigestive anastomosis was performed in 97 patients, in 54 patients choledochotomy was completed by external draina…

Pneumobilia, or air within the biliary tree of the liver, suggests an abnormal communication between the biliary tract and the intestines, or infection by gas-forming bacteria. Pneumobilia usually can be distinguished from air in the portal venous system by its appearance on computed tomography (CT) scan. The most common conditions associated with pneumobilia include: 1) a biliary-enteric surgical anastamosis, 2) an incompetent sphincter of Oddi, or 3) a spontaneous biliary-enteric fistula. Three cases of pneumobilia associated with its most common causes are presented and further differential

A 65-year-old female with a history of chronic gastritis presented with repeat epigastric pain and heartburn after meals. A physical examination was unremarkable, other than a mild fever and epigastric tenderness. Laboratory data revealed leucocytosis of 11,340/μl with 86.8 % of neutrophils, elevated γ-glutamyltransferase at 282.1 IU/l and mildly abnormal AST 40.5 IU/l, ALT 74.9 IU/l, total bilirubin 30.4 μmol/l (direct bilirubin 9.4 μmol/l) and amylase 202 IU/l. A gastroscopy showed a 4 mm fistula on the anterior wall of the proximal duodenal bulb, without ulcerations and the patient was admi

BACKGROUND: Congenital bronchobiliary fistula (CBBF) is a developmental structural abnormality between the trachea or bronchus and the biliary tree, with non-specific clinical manifestations that mainly present as repeated cough and dyspnea, but it is often misdiagnosed as pneumonia. If not promptly diagnosed and effectively treated, CBBF can be fatal. Currently, radical surgery is the only treatment option for CBBF.
CASE PRESENTATION: An 8-day-old full-term Han Chinese female infant presented with coughing accompanied by sputum and shortness of breath with worsening dyspnea for 4 days. Yello…

The introduction and popularization of laparoscopic cholecystectomy has been accompanied with a considerable increase in perforation of gallbladder during this procedure (10% - 32%), with the occurrence of intraperitoneal bile spillage and the consequent increase in the incidence of lost gallstones (0.2% - 20%). Recently the complications associated with these stones have been documented in the literature. We report a rare complication occurring in an 81-year-old woman who underwent laparoscopic cholecystectomy and developed cutaneous fistula to the umbilicus and elimination of biliary stones

Pneumobilia is the finding of air in the biliary tree. Most cases are iatrogenic in origin, especially after sphincterotomy and after hepaticojejunostomy or choledochojejunostomy. In patients without such history, the presence of pneumobilia needs further investigation. Most patients are likely to have an enterobiliary fistula. Although patients may be asymptomatic, possible complications include gallstone ileus, Bouveret syndrome or recurrent episodes of cholangitis. We present a case of a 38-year-old man presenting with obstructive jaundice and pneumobilia in whom choledochoduodenal fistula …

KEY CLINICAL MESSAGE: Biliopleural fistula is a rare but serious complication after liver transplantation that should be managed nonoperatively with antibiotics, pleural drainage, decompression of high-pressure biliary tract, or ultimately surgery in unresponsive cases.
ABSTRACT: Bilious pleural effusion is a rare entity often iatrogenic, following hepatobiliary surgeries and biliary interventions, and has been reported only in a limited number of patients after liver transplantation. A 5-year-old girl underwent living donor liver transplantation due to progressive familial intrahepatic chole

In order to achieve this goal, we have to perforate the biliary tree into the abdomen. We report a patient with spontaneous cholecystocolonis fistula secondary to cholelithiasis. A 93 year-old woman was admitted because of weight loss, diarrhoea and upper abdominal pain. Ultrasound examination revealed air in the biliary tract and cholescientigraphy revealed a fistula between the gallbladder and right colon. Using endoscopic retrograde cholangiopancreatography a calculus was extracted from the bile duct and the symptoms disappeared.

RATIONALE: Intraductal papillary mucinous neoplasms (IPMNs) may perforate into adjacent organs, but pancreaticobiliary fistula caused by IPMN is rare. We present a rare case of pancreaticobiliary fistula due to a branch-duct IPMN, accelerated by a simultaneous ampullary carcinoma.
PATIENT CONCERNS: A woman in her 70s developed obstructive jaundice and cholangitis after a year of follow-up for IPMN.
DIAGNOSES: Imaging revealed a branch-duct IPMN with a mural nodule and dilation of the main pancreatic duct, along with a dilated distal bile duct. Perforation into the bile duct and lymphadenopat…

Spontaneous cholecystocutaneous fistula (SCF) is a rare complication of neglected calculous biliary disease and also an extremely rare complication of gallbladder neoplasm. This pathology has become even rarer because of prompt diagnosis and expedient surgical intervention for gallstones. So far, there is one published report of a SCF due to gallbladder adenocarcinoma. We present the case of a woman aged 87 years, admitted to the Vth Department of Surgery, Clinical Municipal Hospital of Cluj-Napoca (Romania) for a tumoral mass located in the epigastrium. In the epigastrium, the patient had thr
