Algorithm-based remote monitoring of heart failure risk data in people with cardiac implantable electronic devices: a systematic review and cost-effectiveness analysis.

BACKGROUND: Heart failure is a clinical syndrome caused by any structural or functional cardiac disorder that impairs the heart's ability to function efficiently and pump blood around the body. Function can also be monitored using cardiac implantable electronic devices, some of which may also deliver a therapeutic benefit (e.g. pacemakers), while others only monitor metrics over time. Implantable devices can include algorithms that aim to predict the occurrence of a heart failure event. They are intended to be used alongside clinical judgement and make treatment decisions. OBJECTIVES: To dete

The underrepresented minority STEMM pipeline is a pipe dream: The pipes rarely connect.

Building and sustaining a diverse health care workforce including members of underrepresented groups (URG) requires overcoming an intricate web of structural barriers and their downstream sequelae that conspire to perpetuate underrepresentation of many groups of Americans in science, technology, engineering, math, and medicine (STEMM). These include students from low income, marginalized racial and ethnic minority, gender minority, disabled, female, rural and other groups. Several recent efforts to better recognize and work through these barriers and capture talented URG youth in STEMM have ta

Sex reconfigured: DIY hormone therapy and vernacular endocrinology in transfeminine communities in China.

Faced with a restrictive institutional medical landscape, trans people in China turn to DIY (Do-It-Yourself) hormone therapy. While existing health literature has studied the risks and impacts of informal hormone therapy, little is known about the practical strategies and the embedded meaning-making processes around hormone-taking on the ground. Building on digital and in-person participant observation conducted in two cities in China and semi-structured interviews with eight transfeminine individuals between 2021 and 2022, this article examines the embodied practices and community knowledge o

Paclitaxel-Coated Balloon Dilation for Adults With Recurrent Bulbar Urethral Stricture: A Health Technology Assessment.

BACKGROUND: Bulbar urethral stricture is the narrowing of the bulbar segment of the urethra, which causes urinary symptoms and difficulty in voiding. Surgical urethroplasty is the gold standard treatment, but usually the first-line treatment is using either a simple (uncoated) balloon, a rigid dilator, or performing direct vision internal urethrotomy, which uses a blade or laser to make a cut in the stricture. Treatment with a balloon that is coated with paclitaxel has been offered as a second-line treatment when the stricture recurs. This health technology assessment looked at how safe, effec

Composite endpoints in health technology assessment: Part 2 - expanding the evidence base with a framework for best modeling practice.

The first paper of this two-part series critically examined the role of composite endpoints in health technology assessments (HTAs) and outlined strategies for determining whether to employ the composite estimate of treatment effect or disaggregate into the component endpoints of the composite and apply separate treatment effects within a modeling framework. In this second paper, we expand the discussion beyond a pivotal trial and consider the way in which additional evidence from the same indication for different drugs in the same class, or the same drug for different indications, could be em
