Management of children with thrombocytopenia-absent radius syndrome: an institutional experience.

AIM: Thrombocytopenia-absent radius (TAR) syndrome is characterised by bilateral absence of the radii in the presence of both thumbs and hypomegakaryocytic thrombocytopenia. We report our experience of three patients affected by TAR syndrome presenting with different clinical manifestations focusing on the management of these patients. METHODS AND RESULTS: We propose the use of long-term central venous catheters to reduce discomfort because of repeated vein punctures in search for a venous access, rendered difficult by upper limbs abnormalities. CONCLUSIONS: It is important to improve the qu

[A new method of age determination by age-related changes in the hand bone].

An anthropological method designed for determining the age is under discussion; it is based on the point-evaluation of age-related changes observed in the hand bones by X-Ray, i.e. osteophytes, osteoporosis, osteosclerosis and joint deformations. The method was made use of to elaborate a new modified point-based assessment of the above changes in the hand bones with respect to their severity.

Metabolic bone disease risk factors strongly contributing to long bone and rib fractures during early infancy: A population register study.

BACKGROUND: The aim of this study was to assess the incidence of fractures in infancy, overall and by type of fracture, its association with accidents, metabolic bone disease risk factors, and abuse diagnosis. METHODS: The design was a population-based register study in Sweden. Participants: Children born 1997-2014, 0-1 years of age diagnosed with fracture-diagnosis according to International Classification of Diseases (ICD10) were retrieved from the National Patient Register and linked to the Swedish Medical Birth Register and the Death Cause Register. Main outcome measures were fractures of

Large Plasmacytoma Occupying the Upper Limb in a Myeloma Patient.

Extramedullary disease (EMD) is an issue for patients with multiple myeloma (MM), since extramedullary spread of MM is associated with an aggressive course and a poor prognosis. Moreover, the mechanism of EMD development is uncertain. Here, we present extensive extramedullary plasmacytoma occupying the left upper limb of a 66-year-old female patient with MM with an extremely aggressive course and multiple visceral organ involvement without bone marrow infiltration or plasma cell leukemia. EMD of this large size is extremely rare and this case may provide a clue for better understanding of clin

[Short rib-polydactyly syndromes].

The authors report on a newborn which died only a few hours after birth. It manifested generalized dropsy and ascites, a central upper lip cleft, narrow thorax, severe dysraphia, short limbs but normal-length trunk, and brachydactyly. Particularly striking radiologic findings were the extremely short, horizontal ribs and a shortening of the long bones, with rounded calcification zones. On the basis of comparison with other cases of short rib-polydactyly syndromes (SRPS) and other chondrodysplasias, this case was classified as Majewski's syndrome, even though there was no polydactyly and the sh

Pseudotumour of the temporal bone: an unusual [corrected] cause of otorrhoea and facial palsy.

An inflammatory pseudotumour of the temporal bone is a rare, idiopathic, fibro-inflammatory lesion which mimics malignancy. Although such a lesion is known to occur elsewhere in the body such as the liver, abdominal viscera, pelvis, thorax and the upper respiratory tract, the involvement of the temporal bone is extremely unusual. We present such a case in a 60-year-old man, which provided a great diagnostic challenge. We review the key radiological and histological results that confirmed the diagnosis and demonstrated for the first time that oral steroids alone may completely resolve the condi

Surgical intervention and customised dressings in an extremity wound caused by necrotising fasciitis: a case study.

Necrotising fasciitis can result in extensive loss of soft tissue and limb function. Therefore, surgical intervention requires a balance between extensive tissue removal and saving the limb. A 56-year-old male presented with necrotising fasciitis of the upper extremity. Debridement resulted in a full-thickness wound of the forearm and upper arm, an exposed olecranon and two proximally-based skin flaps. Reconstruction required multiple debridements to optimise the wound for skin grafting and to obtain flap adherence to underlying tissues of the upper arm. Conventional dressings over the flaps p

Long term clinical outcome of dental implants placed in a patient with Singleton-Merten syndrome.

PATIENTS: Singleton-Merten syndrome is an extremely rare autosomal dominant condition with less than 10 reported cases in the literature. It is characterized by abnormal aortic calcifications and dental abnormalities. The goal of this case report is to discuss the abnormal oral clinical features and the modified treatment protocol that was used in order to achieve osseointegration of dental implants in a patient having abnormal bone density and bone turnover associated with Singleton-Merten Syndrome. DISCUSSION: Following extraction of the remaining teeth, titanium implants (Friadent GmbH, Ma

Adverse cardiac events in 56,000 orthopaedic trauma patients: Does anatomic area make a difference?

PURPOSE: Postoperative cardiac events in orthopaedic trauma patients constitute severe morbidity and mortality. It is therefore increasingly important to determine patient risk factors that are predictive of postoperative myocardial infarctions and cardiac arrests. This study sought to assess if there is an association between anatomic area and cardiac complications in the orthopaedic trauma patient. PATIENTS AND METHODS: From 2006-2013, a total of 361,402 orthopaedic patients were identified in the NSQIP database using Current Procedural Terminology (CPT) codes. Of these, 56,336 (15.6%) pati

Surgical and microbiological characteristics that influence the probability of reoperation to promote bone healing after fracture-related infection diagnosis.

PURPOSE: To compare the surgical and microbiological characteristics of fracture related infection (FRI) that had union and those requiring additional surgery to promote bone healing. We hypothesized that FRIs with MRSA will have higher risk for reoperation to promote bone healing. METHODS: This is a retrospective study on 247 patients over 18 who underwent bone fixation for pelvis, upper, and lower extremities fractures between 2013 and 2021 at a level I trauma center. All cases had an FRI and at least 6 months of follow-up after diagnosis. Patients with pathologic fracture due to underlying

Aseptic (avascular) bone necrosis in the foot and ankle.

Aseptic necrosis may be defined as a group of diseases that have bone necrosis as a common denominator. They usually appear in the epiphyses and in the carpal and tarsal bones. They generally appear during a growth period and principally at those skeletal points subjected to particular stress. In Müller-Weiss disease in the advanced stages, talonavicular-cuneiform arthrodesis, with or without back foot correction, is the best surgical option. In Freiberg-Kohler disease, treatment can be conservative and we can maintain the head of the metatarsal by performing a joint debridement of the metatarso

Upper-extremity phocomelia reexamined: a longitudinal dysplasia.

BACKGROUND: In contrast to longitudinal deficiencies, phocomelia is considered a transverse, intercalated segmental dysplasia. Most patients demonstrate severe, but not otherwise classifiable, upper-extremity deformities, which usually cannot be placed into one of three previously described phocomelia groups. Additionally, these phocomelic extremities do not demonstrate true segmental deficits; the limb is also abnormal proximal and distal to the segmental defect. The purpose of this investigation was to present evidence that upper-extremity abnormalities in patients previously diagnosed as ha

Bone Health in Handball Players: The Role of Exercise.

Exercise has an indirect but significant effect on bone mass by generating mechanical stress on the skeleton. It is widely recommended to enhance bone quality and counteract the loss of bone and muscle mass, particularly in the elderly. Dual-energy X-ray absorptiometry (DEXA) is a relatively inexpensive and effective method for assessing bone mineral density (BMD) and bone mineral content (BMC), both of which are strong predictors of fracture risk. Team handball is a high-impact sport, which is characterized by frequent jumping, landing, and torsional strains on the upper and lower limbs, as w

Segmental body composition transitions in stroke patients: Trunks are different from extremities and strokes are as important as hemiparesis.

BACKGROUND & AIMS: Loss of muscle and bone mass is prevalent in the stroke population. Few studies have investigated the difference between having a stroke and hemiplegia and their influence on segmental body composition. This study aimed to evaluate the changes of body composition in the extremities and trunk of stroke patients in comparison with those of the healthy controls. METHODS: Stroke patients with an onset of longer than 6 months and healthy participants matched by age and gender were recruited. Body weight, height, grip strength, and gait speed were measured, and a dual-energy x-ra

Diffuse Large B-Cell Lymphoma Infiltrating Limb Bones With a Few Lymphadenopathy Revealed on Whole-Body 18F-FDG PET/CT.

A 74-year-old man presented to our hospital for a 2-month history of worsening feet and left hand swelling pain. Computed tomography reconstruction image and sagittal images of the left foot demonstrated multiple and irregular bone destruction. Orthopedic surgeon suspected tuberculosis or primary skeletal malignancy. ¹⁸F-FDG PET/CT revealed bone destruction and abnormal activity mainly in bones of both upper and lower extremities and a few lymphadenopathy at the region of left axilla and groin. Diagnosis of diffuse large B-cell lymphoma was eventually made. Subsequently, the patient was transf

Upper-limb physical activity after distal radius fractures treated operatively or nonoperatively: a secondary analysis of a multicenter randomized trial.

BACKGROUND AND PURPOSE: While most patients recover well from distal radius fracture, some experience prolonged deficits. We aimed to primarily compare high-intensity upper-limb activity 12 months after operative vs nonoperative treatment of malaligned distal radius fractures and secondarily to assess the impact of fracture alignment and compare activity levels with healthy older adults. This is a secondary analysis of the Distal Radius Fracture Trial. METHODS: The Distal Radius Fracture Trial was a multicenter, randomized controlled trial. Patients with malaligned distal radius fractures wer

Reevaluating the Need for Antibiotic Prophylaxis in Adult Upper Extremity Surgery With Hardware.

BACKGROUND: Although it is well established that antibiotic prophylaxis is not needed in soft tissue upper extremity cases, there is still no definitive consensus when hardware implantation is involved. We hypothesize that antibiotic prophylaxis is not necessary and there is no difference in postoperative surgical site infection rates regardless of preoperative antibiotic administration. METHODS: A retrospective cohort analysis was performed on upper extremity surgical cases with hardware implantation performed at a single institution amongst 5 hand surgeons between November 2021 and November

Microbiology of surgical site infection in closed upper limb fractures: Data from a prospective trauma registry.

The current study aims to determine the rate of surgical site infection, causal microorganism, and antibiotic sensitivity pattern in operated upper limb closed fractures at the Aga Khan University Hospital, Karachi. Cases presenting between June 2015 to October 2019, were selected from a single-centre, longitudinal, prospective orthopaedic trauma registry. Infection rate, causal microorganism, and antibiotic sensitivity pattern were determined up to six months after surgery. From among a total of 376 closed fractures, 12 encountered surgical site infection with some having late onset, giving a

Effect of shoulder model complexity in upper-body kinematics analysis of the golf swing.

The golf swing is a complex full body movement during which the spine and shoulders are highly involved. In order to determine shoulder kinematics during this movement, multibody kinematics optimization (MKO) can be recommended to limit the effect of the soft tissue artifact and to avoid joint dislocations or bone penetration in reconstructed kinematics. Classically, in golf biomechanics research, the shoulder is represented by a 3 degrees-of-freedom model representing the glenohumeral joint. More complex and physiological models are already provided in the scientific literature. Particularly,
