The two-toed Wadoma--familial ectrodactyly in Zimbabwe.

Ectrodactyly has been referred to as being present among the Wadoma people of Zimbabwe for many years and has given rise in popular imagination to the legend of 'the two-toed people'. An investigation into this situation is reported in which three members of one Wadoma family are described. Reference is made to reports of a similar condition occurring in a group of people who may be ethnically related to the Wadoma living on either side of the Botswana/Zimbabwe border, who have not, as far as is known, been scientifically assessed.

Benefits of continuous isoniazid preventive therapy may outweigh resistance risks in a declining tuberculosis/HIV coepidemic.

OBJECTIVE: Extending the duration of isoniazid preventive therapy (IPT) among people living with HIV (PLHIV) may improve its effectiveness a … among PLHIV. We evaluated the sensitivity of our baseline model, which was calibrated to data from Botswana, to different assumptions about the future trajectory of the TB epidemic. RESULTS: Our model suggests that, in the context of a declining TB epidemic such as that currently observed in Botswana, the incidence and mortality benefits of continuous IP …

Barriers to good consultations in public primary healthcare clinics.

STUDY AIM: To explore the barriers to good consultations in government primary health care (PHC) clinics, in Botswana. BACKGROUND: Good consultations improve adherence to treatment, treatment outcomes, and health seeking behaviour while bad consultations do the opposite. We explored the barriers to good consultations (doctor patient interactions) in public PHC clinics in Botswana. Knowledge of these barriers may help in crafting strategies to eliminate the barriers and improve health care. METHODS: This was a descriptive qualitative study in which patients and doctors were interviewed. A sem…

Late registration for antenatal care by pregnant women with previous history of caesarean section.

BACKGROUND: Despite good access to antenatal care (ANC) services for most women, and regular training of healthcare workers in obstetrics and gynaecology, many pregnant women with a previous history of caesarean section (C/S) still presented late for ANC services at Letsholathebe II Memorial Hospital (LIIMH) in Maun, Botswana. This may increase morbidity and mortality in women with previous C/S delivery and neonates. Knowing why women with previous C/S present late for ANC may help in the formulation of interventions that decrease morbidity and mortality amongst these women and neonates. AIM:…

Orb-web spider Argiope (Araneidae) as indigenous arrow poison of G/ui and G//ana San hunters in the Kalahari.

Hunting has been crucial in early human evolution. Some San (Bushmen) of southern Africa still practice their indigenous hunting. The use of poisons is one remarkable aspect of their bow-and-arrow hunting but the sources, taxonomic identifications of species used, and recipes, are not well documented. This study reports on fieldwork to investigate recent indigenous hunting practices of G/ui and G//ana San communities in the Central Kalahari Game Reserve (CKGR), Botswana. Here we discuss their use of spider poison. The hunters use the contents of the opisthosoma ('abdomen') of a spider as sole …

Global Measures of HIV Care Accessibility Across Urban, Suburban, and Rural Areas.

Despite advances in HIV treatment, disparities in healthcare access remain across low-income (LIC), middle-income (MIC), and high-income (HIC) countries, limiting access to clinical care, antiretroviral therapy, and viral load testing. This study developed the HIV Care Access Index (HIV-CAI) to measure and compare HIV care service access across countries and within geographic regions and examined associations with economic and health indicators. A cross-sectional survey was conducted between August 2021 and June 2023 across nine countries: Botswana, China, Colombia, Kenya, Nigeria, Puerto Rico…

The effects of war on the control of diseases of livestock in Rhodesia (Zimbabwe).

The disruption of veterinary services in the tribal areas of Rhodesia (now Zimbabwe) during seven years of conflict resulted in serious epidemics of disease. The cessation of dipping was followed by the death of an estimated one million cattle from tick-borne disease. Heavy mortality followed the disruption of control measures for trypanosomiasis. Foot-and-mouth disease and anthrax spread widely in the tribal areas. Rabies, normally restricted to areas bordering Botswana and Mocambique, became widespread. A marked increase in human deaths from anthrax and rabies occurred.

Cost-effectiveness of single, high-dose, liposomal amphotericin regimen for HIV-associated cryptococcal meningitis in five countries in sub-Saharan Africa: an economic analysis of the AMBITION-cm trial.

BACKGROUND: HIV-associated cryptococcal meningitis is a leading cause of AIDS-related mortality. The AMBITION-cm trial showed that a regimen based on a single high dose of liposomal amphotericin B deoxycholate (AmBisome group) was non-inferior to the WHO-recommended treatment of seven daily doses of amphotericin B deoxycholate (control group) and was associated with fewer adverse events. We present a five-country cost-effectiveness analysis. METHODS: The AMBITION-cm trial enrolled patients with HIV-associated cryptococcal meningitis from eight hospitals in Botswana, Malawi, South Africa, Ugan…

Modest reduction in adverse birth outcomes following the COVID-19 lockdown.

BACKGROUND: Widespread lockdowns imposed during the coronavirus disease 2019 crisis may impact birth outcomes. OBJECTIVE: This study aimed to evaluate the association between the COVID-19 lockdown and the risk of adverse birth outcomes in Botswana. STUDY DESIGN: In response to the coronavirus disease 2019 crisis, Botswana enforced a lockdown that restricted movement within the country. We used data from an ongoing nationwide birth outcomes surveillance study to evaluate adverse outcomes (stillbirth, preterm birth, small-for-gestational-age fetuses, and neonatal death) and severe adverse outc…

Metabolic capabilities are highly conserved among human nasal-associated Corynebacterium species in pangenomic analyses.

Corynebacterium species are globally ubiquitous in human nasal microbiota across the lifespan. Moreover, nasal microbiota profiles typified … mated the metabolic capabilities of 87 distinct human nasal Corynebacterium strain genomes: 31 from Botswana and 56 from the USA. C. pseudodiphtheriticum had geographically distinct clades consistent with localized strain circulation, whereas some strains from the other species had wide geographic distribution spanning Africa and North America. All species …

Dolutegravir-induced neural tube defects in mice are folate responsive.

OBJECTIVES: In 2018, the Botswana Tsepamo Study reported a nine-fold increased risk of neural tube defects in infants whose mothers were treated with dolutegravir (DTG) from the time of conception. As maternal folate supplementation and status is a well known modifier of neural tube defect (NTD) risk, we sought to evaluate birth outcomes in mice fed normal and low folic acid diets treated with DTG during pregnancy. DESIGN: DTG was evaluated for developmental toxicity using pregnant mice fed normal or low folic acid diet. METHODS: CD-1 mice were provided diet with normal (3 mg/kg) or low (0.3…

Exploring Sedentary Time as a Moderator in the Relationship Between COVID-19 Symptom Burden and Mental Health Outcomes in 4 African Countries.

BACKGROUND: Physical inactivity and mental distress were prevalent across African populations during COVID-19. However, limited research has explored the role of sedentary time in shaping mental health outcomes during the pandemic. In this study, sedentary time during COVID-19 and its moderating role in the relationship between COVID-19 symptom burden and mental health outcomes were examined in Botswana, Ghana, Nigeria, and Tanzania. METHODS: Cross-sectional data (April 2020 to November 2022) from 3125 adults (male = 52.3%, Mage = 27.05, SD = 8.91) were analyzed. Participants completed self-r…

A severe case of contagious ecthyma in Tswana goats.

The first severe case of caprine contagious ecthyma (parapox) in Tswana goats is described from Botswana. Affected animals were indigenous adult Tswana goats. The case involved a flock of 12 goats of which 4 (33.3%) were very severely affected but none died. The lesions were confined to the head and included swollen lips, swollen submandibular lymph nodes, gingivitis, glossitis, ulceration on lip and gum mucosae and scab formation on ulcerated areas. No lesions were found on other parts of the body. This case was clinically indistinguishable from bluetongue.

Prevalence of clinical isolates of Cryptococcus gattii serotype C among patients with AIDS in Sub-Saharan Africa.

Cryptococcus gattii is a group of exogenous, neurotropic yeasts that possess the capsular serotype B or C. Isolates of serotype C are extremely rare and, until recently, were known to infect only immunocompetent individuals. We genotyped 176 isolates of Cryptococcus from patients in sub-Saharan Africa who had AIDS; 22 (13.7%) of 161 isolates from Botswana and 2 (13.3%) of 15 isolates from Malawi were C. gattii serotype C strains. All of these serotype C strains belong to the rare VGIV genotype, possess the MAT alpha mating-type allele, and exhibit little genetic diversity.
