Nocardia farcinica brain abscess in an immunocompetent patient with isolated hemianopsia.

BACKGROUND: Nocardia farcinica is an uncommon cause of brain abscess, most often in immunocompromised hosts. Increasingly, cases in immunocompetent individuals are reported, but diagnosis remains challenging and mortality is high. CASE PRESENTATION: We report a 63-year-old diabetic male with sudden altered mental state and right temporal hemianopsia. Initial CT and FDG-PET/CT suggested a tumour, delaying suspicion of infection. MRI later revealed a cerebral abscess, and stereotactic aspiration yielded purulent material. Direct microscopy showed branching Gram-positive rods; culture identified…

Group B Streptococcus Brain Abscess in a Neonate with Bilateral Otorrhea.

Introduction  Group B streptococcus (GBS) is the leading cause of bacterial sepsis and meningitis in neonates worldwide, but brain abscess secondary to GBS is extremely rare. While temporal brain abscesses have been described as a sequelae of otogenic infections in children and adults, such a presentation has not been described in neonates. Case Description  An 8-day-old female infant presented with a fever and irritability along with bilateral purulent otorrhea. Maternal GBS screening was negative, but the delivery was complicated by chorioamnionitis. Workup revealed neutrophilic pleocytosis …

A Rare Case of COVID-19-Associated Solitary Aspergillus Brain Abscess.

Surgically operated case of solitary Aspergillus brain abscess caused by Aspergillus fumigatus in coronavirus disease 2019 (COVID-19) patient is not reported. The authors report a case of 33-year-old diabetic female patient presented with generalized seizure followed by left hemiparesis. Patient was treated with steroids for COVID-19 pneumonia. Initial imaging revealed a right frontal lobe infarct that later confirmed as a case of frontal lobe abscess. Patient underwent craniotomy and thick yellow pus was drained. Abscess wall was excised. Postoperatively patient improved with Glasgow coma sca…

Brain Abscess Caused by Nocardia farcinica in a Young Immunocompetent Patient.

Cerebral nocardiosis is a rare opportunistic infectious disease that occurs mainly in immunocompromised hosts; however, immunocompetent patients may be affected too. It often results in the formation of intraparenchymal brain abscess, which represents only 2% of all cerebral abscesses. The overall mortality rate exceeds 20% in immunocompetent patients and 55% in immunocompromised patients. Bacteriological diagnosis is often confirmed only after the surgical excision of the abscess. Thus, the initiation of effective therapy is frequently delayed. Our goal is to highlight a diagnostic approach t…

Spontaneous Escherichia coli Meningitis and Brain Abscess in an Immunocompetent Adult.

Escherichia coli is widely known to be a common cause of gram-negative bacterial meningitis in neonates and infants but is a rare cause of central nervous system infection in adults. Risk factors for E. coli meningitis (e.g., penetrating head trauma or neurosurgery) have been broadly discussed in the literature. Here, we describe a case of spontaneous E. coli meningitis with multiple enhancing brain lesions and liver abscess in an immunocompetent adult that presented as generalized weakness.

Recurrent Brain Abscess in a Child With Cyanotic Congenital Heart Disease.

Brain abscesses are rare but life-threatening conditions associated with morbidity and mortality. They are encountered in unoperated and partially treated cyanotic congenital heart diseases. Our patient is a diagnosed case of transposition of great arteries with a ventricular septal defect. She had recurrent abscesses for which a combination of antimicrobial therapy and surgical excision was performed. Surgical excision carries a great risk of rupture of abscess into the ventricular system and is associated with poorer outcomes. The outcome of brain abscess primarily depends on Glasgow Coma Sc…

18F-FET PET/CT as a Diagnostic Tool for Brain Abscess.

A 49-year-old man presented with sudden right-sided weakness and seizure. Brain MRI identified a lobulated mass with diffusion restriction and irregular wall enhancement in the left parietal lobe. 18F-FET (O-(2-[18F]fluoroethyl)-l-tyrosine) PET/CT was performed, which identified a cystic mass in the left parietal lobe accompanied by FET uptake. Compartmentalized uptake was also confirmed throughout the left parietal lobe. Considering the relatively low target-to-background ratio and uptake observed in the entire left parietal lobe, the lesion was more likely to be a brain abscess than a tumor.…

Ciliated respiratory epithelium encapsulating Pseudomonas brain abscess due to prior trauma.

Bacterial brain abscesses are typically spread through a haematogenous route. Open head wounds and neurosurgical interventions are uncommon aetiologies. Ectopic tissue found in the cerebral cortex is usually ascribed almost entirely from carcinomas. Here, we describe a 57-year-old gentleman who, 22 years after a fireworks related traumatic injury to the left orbit, presented with headaches and altered behaviour. Imaging revealed an abscess immediately superior to the orbit, whose bacterial aetiology was identified to be Pseudomonas aeruginosa, encapsulated by ciliated respiratory epithelium. T…

Unusual presentation of brain abscess in a 23-month-old infant.

BACKGROUD: Pediatric brain abscesses usually occur as a consequence of predisposing conditions, such as ENT (ear, nose, and throat) infection and physical damage. But there are still a number of cryptogenic infection cases. METHODS: We present an unusual cryptogenic infection case of multiple and multiloculated brain abscess in an infant, which was in the absence of ENT infection, meningitis, trauma, prior surgery, cyanotic heart disease, or immune defect. The child has no specific symptoms but keeping apathic and days of diarrhea. Deduced from clinical presentation and this case's DWI-MRI fe…

Brain abscess in children, a two-centre audit: outcomes and controversies.

OBJECTIVE: The aim of this study was to better characterise clinical presentation, management and outcome in infants and children with brain abscess. METHODS: The authors conducted a retrospective, multicentre study in two national reference centres over a 25-year period (1992-2017). During this period, 116 children and 28 infants (age <1 year) with brain abscess were treated. RESULTS: The median age at diagnosis was 101.5 (range: 13-213) months in children and 1 (0-11) month in infants. Significant differences were observed between children and infants. The most common predisposing factor w…

Risk Factors Associated With Mortality in Patients With Otogenic Brain Abscess.

OBJECTIVE: Otogenic brain abscess is a well-recognized clinical condition that describes brain abscess secondary to an ear infection or mastoiditis. Current evidence remains limited on risk factors associated with mortality as most data are from case series. We aimed to 1) report the mortality rate among patients who did and did not receive mastoidectomy 2) identify factors associated with inpatient mortality. STUDY DESIGN: Retrospective cohort study. SETTING: Multi-institutional. PATIENTS: We identified a cohort of patients for years 2008 to 2014 who in their inpatient hospitalization carr…

Predictors of clinical outcomes among patients with brain abscess in Thailand.

Although brain abscess is a fatal neurological infection, the studies in Thailand are quite limited and outdate. This study aims to identify predictors of mortality among patients with brain abscess in Thailand. Patients with a diagnosis of brain abscess admitted to Songklanagarind Hospital, a referral tertiary care hospital in southern Thailand, between 2002 and 2017 were enrolled into this retrospective case control study. Demographic data, neurological status, clinical presentations, predisposing factors, microbiological profiles, neuroimaging findings, treatments, and outcomes were collect…

Aspergillus flavus-Induced Brain Abscess in an Immunocompetent Child: Case report.

Intracranial aspergillosis is an extremely rare manifestation of invasive aspergillosis in immunocompetent children and is associated with high morbidity and mortality. We report a 12-year-old immunocompetent male child who was referred to the King Faisal Specialist Hospital & Research Centre, Riyadh, Saudi Arabia, in May 2010 after a sudden-onset headache and loss of consciousness. Brain imaging revealed a large right space-occupying occipital lesion and the patient underwent a craniotomy and resection. Histopathology of the lesion revealed necrotising granulomatous fungal encephalitis with m
