DTI Study on Brain Structure and Cognitive Function in Patients with Chronic Mountain Sickness.

In chronic mountain sickness (CMS) patients, the structure of the brain, memory and cognition are often irreversibly damaged by chronic hypoxia due to red blood cell overcompensation, elevated haemoglobin and blood stasis. In this study, we aimed to evaluate this damage using diffusion tensor imaging (DTI) and to study the correlations among the fractional anisotropy (FA),the apparent diffusion coefficient (ADC) value, the severity index of CMS and the simple Mental State Examination (MMSE) score in CMS patients. A total of 17 patients with CMS and 15 healthy controls were recruited for conven

Exploring the Link between Hypertension and Cerebral White Matter Changes in Chronic Kidney Disease.

BACKGROUND: Patients diagnosed with chronic kidney disease (CKD) are at a higher risk of encephalopathy, a condition exacerbated by the presence of various chronic diseases. Hypertension is a significant risk factor for brain damage in the general population but is limitedly discussed in patients with CKD. Brain magnetic resonance imaging (MRI) is an excellent tool for evaluating cerebral white matter lesions. Most previous studies showed the association between hypertension and cerebral white matter lesions in the general population but were less focused on CKD patients. Therefore, the presen

Pearls & Oy-sters: Mesial Temporal Seizures in the Absence of the Mesial Temporal Lobe. Seizure Onset vs Seizure Network.

Seizure semiology represents the clinical expression of the activation of the several brain regions comprising an epileptic network. In mesial temporal lobe epilepsy (MTLE), this network includes the insular-opercular-neocortical temporal-hippocampal (IONTH) regions. In this study, we present the case of a patient with pharmacoresistant seizures characterized by nausea, lip-smacking, semipurposeful hand movements, and speechlessness, suggesting dominant hemisphere MTLE, with scalp video-EEG findings and left hippocampal sclerosis on brain MRI confirming the diagnosis. She underwent anterior te

Repetitive transcranial magnetic stimulation (rTMS) combined with multi-modality aphasia therapy for chronic post-stroke non-fluent aphasia: A pilot randomized sham-controlled trial.

Repetitive transcranial magnetic stimulation (rTMS) shows promise in improving speech production in post-stroke aphasia. Limited evidence suggests pairing rTMS with speech therapy may result in greater improvements. Twenty stroke survivors (>6 months post-stroke) were randomized to receive either sham rTMS plus multi-modality aphasia therapy (M-MAT) or rTMS plus M-MAT. For the first time, we demonstrate that rTMS combined with M-MAT is feasible, with zero adverse events and minimal attrition. Both groups improved significantly over time on all speech and language outcomes. However, improvement

Significant improvements in SEIzure interVAL (time between seizure clusters) across time in patients treated with diazepam nasal spray as intermittent rescue therapy for seizure clusters.

OBJECTIVE: Intermittent rescue therapy may be used for seizure clusters, which are clinical emergencies that may persist ≥24 h and increase risk of status epilepticus, emergency room visits, and reduced quality of life for patients with epilepsy. Beyond effectiveness for aborting seizure clusters, no data exist on how intermittent rescue therapy may impact the long-term natural course of seizure clusters. This novel analysis explores SEIzure interVAL (SEIVAL; time between seizure clusters) in patients from a long-term safety study of diazepam nasal spray (Valtoco) to assess SEIVAL changes with

Effects of nonconscious perception on motor response.

The present study reviews the literature on the empirical evidence for the dissociation between perception and action. We first review several key studies on brain-damaged patients, such as those suffering from blindsight and visual/tactile agnosia, and on experimental findings examining pointing movements in normal people in response to a nonconsciously perceived stimulus. We then describe three experiments we conducted using simple reaction time (RT) tasks with backward masking, in which the first (weak) and second (strong) electric stimuli were consecutively presented with a 40-ms interstim

Joint deformity patterns in severely physically disabled patients.

Deformity patterns of the spine and upper and lower extremities were investigated in 64 patients with severe physical disability. Among the subjects, C-shaped and S-shaped scoliosis was found in 48 and nine, respectively. The hips were windblown in 20, adducted in 22, and abducted in seven. Knees were flexed in 39 and extended in four. Deformities of the ankle and upper extremities were variable. The most common combination, which was C-shaped scoliosis with convexity to the adducted side of hips, windblown hips, flexed knees, and dorsiflexed ankles, was noted among nine patients. Joint deform

Brain damage in preterm newborns: might enhancement of developmentally regulated endogenous protection open a door for prevention?

We present a two-component model of brain white matter damage in preterm neonates. The insult component comprises infection and hypoxia-ischemia, which are both associated with inflammation-related abnormalities in the white matter. The developmental component comprises at least three factors, ie, immaturity of the ependymal/endothelial, oligodendroglial, and endogenous protection systems. All three factors are likely contributors to an increased vulnerability of the preterm newborn's white matter. In this article, we focus on recent developments in oligodendrocyte biology that support the vie

Linguistics, human communication and psychiatry.

BACKGROUND: Psycholinguistics and sociolinguistics have extended our understanding of the abnormal communication seen in psychosis, as well as that of people with autism and Asperger's syndrome. Psycholinguistics has the potential to increase the explanatory power of cognitive and neuropsychological approaches to psychosis and new methods of assessment and therapy are now being developed, based on linguistic theory. METHOD: A MEDLINE literature search was used. RESULTS: Of 205 relevant articles identified, 65 were selected for review. CONCLUSIONS: Greater familiarity with linguistic theory

Neurologic complications of inherited mitochondrial abnormality, and neurologic consequences of inborn errors of metabolism.

The possibility of gene therapy for patients with Menkes or Gaucher's disease has been improved by the isolation of a promising candidate gene and production of a mouse model, respectively. Many mutations of mitochondrial DNA are being associated with mitochondrial encephalomyopathies, and protection of the resultant biochemical deficiency can be achieved with a remarkably low percentage of normal mitochondrial DNA. The correlation between mutation, biochemical deficiency, and neurologic consequence, however, remains frustratingly obscure. Gas chromatography-mass spectrometric urinalysis is be

[Asthenic-autonomic disorders in the decompensation phase of residual organic brain conditions].

The paper provides characteristics of asthenic and vegetative-vascular defects in decompensation of residual organic cerebral insufficiency. The defects were examined vegetologically with the help of photoplethysmography, thermovisiography, the test of the vegetative, psychological, exercise performance. Cutaneous-galvanic reflex, attention, memory, asthenia severity were also evaluated. The data obtained by the authors are important for making differential diagnosis of cerebrogenic with psychogenic asthenia in neurosis which allows differential therapy of asthenia symptoms depending on the et

Neurological assessment: adaptations for special populations with mental retardation.

The incidence of mental retardation in the United States is approximately 125,000 births per year. Recent changes in government regulations necessitate that nurses caring for mentally retarded persons perform physical appraisals. These in-depth appraisals are a new skill for many nurses caring for the retarded and the routine neurological assessment processes must be modified in order to meet the special needs and behaviors of the mentally retarded population. This article provides examples of how a functional abilities approach to the neurological appraisal process can be modified. Further, t

Hand preference in homosexual men.

The present study examined the distribution of hand preference and its relationship to immune system functioning and performance on neuropsychological tests in a sample of 993 homosexual men from the Multicenter AIDS Cohort Study comprising 502 HIV-1 seronegatives, 436 asymptomatic HIV-1 seropositives, and 55 men with diagnoses of AIDS or AIDS Related Complex. The prevalence of left-handedness in all of the groups (13.1-14.5%) was consistent with prior published reports of prevalent left-handedness in the general population. The distribution of hand preference scores (on a 5-item self-report q

Old syndromes and new cytogenetics.

This annotation has been confined to well-established clinical syndromes with recently discovered chromosomal etiologies. It deliberately omits retinoblastoma, the oft-cited paradigm of a contiguous gene syndrome, since it is usually inherited as a Mendelian single gene disorder. However, it was recognition of both the deletion of band q14 of chromosome 13 in mentally retarded children with retinoblastoma, and the linkage of retinoblastoma with the genetic marker esterase D, which resulted in the eventual cloning of the gene. Also omitted are microdeletions of the X chromosome. These disorders

Disrupted eyelid conditioning in a patient with damage to cerebellar afferents.

A 54-year-old woman with damage to cerebellar circuitry resulting from a cerebrovascular accident underwent classical conditioning of the eye-blink response to a tone conditioned stimulus and an air-puff unconditioned stimulus. In contrast to 5 age-matched controls who readily acquired the conditioned response (CR), emitting a mean of 56.7 CRs over 70 trials, the patient emitted only 6 CRs in 100 trials and never emitted 2 consecutive CRs. There were no differences in spontaneous blink rate, sensitivity to the air puff, or sensitivity to the tone between the experimental subject and the contro

[Markers of schizophrenic language. A comparative patholinguistic study of dialogues with schizophrenic, manic, depressive and brain-damaged patients].

Transcripts of dialogues with patients showing schizophrenic, manic, depressive or organic syndromes, 15 persons in each group, were analyzed in order to determine their degree of cohesion and coherence. The cohesion of a text is defined by the grammatical and lexical relation between its elements. Coherence, however, means that the sequence of sentences appears reasonable in the context of communicative action. There is a significantly higher incidence of incoherences in dialogues with schizophrenic patients than in dialogues with patients who show manic, depressive or organic syndromes. Schi
