We report a consecutive case series of patients who underwent transvenous embolization of the paraspinal vein, which was draining the CSF-venous fistula, for treatment of spontaneous intracranial hypotension. These are the first-ever reported cases of this treatment for CSF-venous fistulas. All patients underwent spinal venography following catheterization of the azygous vein and then selective catheterization of the paraspinal vein followed by embolization of the vein with Onyx. All patients had improvement of clinical and radiologic findings with 4 patients having complete resolution of head

Tuberculosis is caused by Mycobacterium tuberculosis. Tuberculosis of the central nervous system is common and manifestations include meningeal and intraparenchymal diseases. However, intraventricular tuberculous abscess is a rare manifestation of intracranial tuberculous infection. We present a case of an immunocompromised female patient with high-grade fever and signs of meningism. The computed tomography and magnetic resonance imaging (MRI) of the brain showed hydrocephalus with rim-enhancing lesion in the right lateral ventricle. The MRI demonstrated a hypointense signal on T1-weighted ima

Spontaneous arteriovenous communications below the diaphragm is a very rare condition. Its association with polysplenia has perhaps not yet been reported in children. We reported a case in a 9-year-old boy presenting with acute onset of fever, vomiting, headache, seizures and altered sensorium. A CT scan revealed a large occipital abscess which was drained surgically. On examination he had marked central cyanosis and grade III clubbing of fingers and toes. Further investigations led to the discovery of a very rare finding of abdominal arteriovenous malformation associated with multiple-enhanci

Intracranial pial arteriovenous fistulas (AVFs) are rare vascular lesions of the brain. These lesions consist of one or more arterial connection to a single venous channel without true intervening nidus. A 24-year-old woman visited to our hospital because of headache, vomiting, dizziness and memory disturbance that persisted for three days. She complained several times of drop attack because of sudden weakness on both leg. Cerebral angiograms demonstrated a giant venous aneurysm on right frontal lobe beyond the genu of corpus callosum, multiple varices on both frontal lobes fed by azygos anter

A 55-year-old woman developed an intractable right orbitofrontal headache. The symptoms subsided spontaneously 2 months after onset, but diplopia due to right abducens nerve palsy had occurred, and gradually worsened. Orbito-ocular signs were never observed throughout the clinical course. Brain MRI and MR angiography demonstrated abnormal signal changes corresponding to the right cavernous sinus. Angiography confirmed a dural carotid-cavernous sinus fistula (CCF) with three directional drainage routes in the arterial phase. Although the most prominent draining vein was the superior ophthalmic

The clinician must maintain a high level of suspicion for central nervous system infections even if not all of the classic signs are present, because prompt treatment may make a difference in patient outcome. If bacterial meningitis is suspected, a CT scan of the head should be obtained prior to lumbar puncture if there is papilledema, a focal neurologic exam, or if the patient is comatose. In bacterial meningitis, empiric antibiotics should be chosen based on a patient's risk factors and should be started immediately. Depending on the resistance patterns of the institution, Streptococcus pneu

The authors report the case of a 14-month-old baby boy with an epidermoid cyst located entirely within the pons and medulla, without an exophytic component. The lesion was examined by computed tomography and magnetic resonance imaging. The child was operated upon three times after two recurrences of the lesion. A suboccipital, subtonsillar approach was used for the first and second procedures and a transtemporal approach for the last one. Excision was thought to be complete the first time, since a solid tumor was found and removed in a large cyst. The cyst wall was not identified. No tumor was

Alzheimer’s disease is characterized by a loss of temporal and spatial orientation and a progressive loss of cognitive and locomotor function. Therapeutic drug options are currently limited. There is evidence that degradation products of Alzheimer’s diseases such as the tau protein or amyloid beta in Alzheimer’s disease could be removed by the glymphatic system (lymphatic system of the brain), as these drain into the deep cervical lymph nodes. The microsurgical creation of lympho-venous anastomoses could improve this drainage and improve cognition in Alzheimer’s patients (probably also for oth

Hypothermia in a trauma patient has been associated with increased morbidity and mortality and is more frequently seen in those sustaining traumatic brain injuries (TBIs). Acidosis is an important consequence of hypothermia that leads to derangements across the spectrum of the coagulation cascade. Here, we present a case of a 31-year-old male presented after suffering a right parietal penetrating ballistic injury with an associated subdural hematoma and 7 mm midline shift requiring decompressive craniectomy and external ventricular drain (EVD) placement in the setting of severe hypothermia (28

Paediatric patients receiving cranial irradiation therapy for brain tumours are at increased risk of cerebrovascular complications. Radiation-induced moyamoya syndrome (MMS) is a well-recognised complication of this. We present a case of an 8-year-old boy with a history of medulloblastoma, who underwent surgical excision followed by post-operative adjuvant oncological treatment. Six years later, he developed cerebellar/intraventricular haemorrhage. He underwent an emergency external ventricular drain (EVD) insertion followed by posterior fossa suboccipital craniotomy. On dural opening, an abno

BACKGROUND: Third ventricular colloid cysts are benign but may cause acute hydrocephalus, raised intracranial pressure, decreased consciousness level, and sudden death. These ventricular colloid cysts associated with stunned myocardium are rarely reported in the literature. This study reported a case of a third ventricular colloid cyst presented as acute hydrocephalus complicated with severe neurogenic pulmonary edema, stunned myocardium, and heart failure, which survived at the end.
CASE PRESENTATION: A 29-year-old woman presented to the emergency department with one day history of headache,

OBJECTIVE: Primary central nervous system non-Hodgkin lymphoma (PCNSL) is a malignant lymphoma limited to the cranial-spinal axis in the absence of systemic lymphoma. Historically, PCNSL accounts for fewer than 5% of all cases of primary intracranial neoplasms. PCNSL is rare in immunocompetent young adults. Although the prognosis for PCNSL is poor, approximately 20%-30% percent of cases achieve a cure.
METHODS: We report two cases of PCNSL originating in the ventricle in otherwise healthy immunocompetent young adults.
RESULTS: A 27-year-old man presented with 10 days of nausea, vomiting, and

The accumulation of small strokes has been linked to cognitive dysfunction. Although most animal models have focused on the impact of arteriole occlusions, clinical evidence indicates that venule occlusions may also be important. We used two-photon excited fluorescence microscopy to quantify changes in blood flow and vessel diameter in capillaries after occlusion of single ascending or surface cortical venules as a function of the connectivity between the measured capillary and the occluded venule. Clotting was induced by injuring the target vessel wall with femtosecond laser pulses. After an

INTRODUCTION: Surgical removal of giant cystic vestibular schwannomas is difficult because of adherences between the cyst membrane, brainstem, and the VII-VIII nerve complex. The recurrence of the cyst is frequent and requires reoperation. The aim of this study was to analyze the role of the palliative cystoperitoneal shunt in giant cystic vestibular schwannomas.
MATERIALS AND METHODS: Eighty-eight patients with a diagnosis of stage III or IV vestibular schwannoma were managed surgically from January 2000 to December 2005 in our department. Six schwannomas were deemed to be cystic according t

Mental health services in low income countries are inadequate. Health care delivery services in these countries are mainly focused on vector-borne diseases and HIV/AIDS. To make the situation worse, trained health workers are leaving developing countries for higher paying jobs elsewhere; this "brain drain" is robbing countries of their energetic and highly trained manpower. In response to this dire situation, Ethiopia started a psychiatric nursing training program where by nurses are trained and assigned to different parts of the country as mental health workers. Their activities range from pr…

The authors report a series of 16 patients with venous anomalies or abnormalities of the posterior fossa studied by angiography, CT and/or MRI. We believe that so-called venous angiomas are extreme anatomic variants that drain normal territories, and we prefer to call them developmental venous anomalies (DVAs). Posterior fossa DVAs, like the supratentorial ones are classified according to their drainage into deep and superficial types. They are exclusively located in the cerebellum or tectum. In 4 cases DVA was an incidental finding; in 3 an associated cerebral venous malformation (CVM) was fo

OBJECTIVES: To determine the long-term risk factors for rehemorrhage (primary outcome) in patients with ruptured brain arteriovenous malformations (BAVMs) after different treatment strategies and to evaluate the impact of different treatment strategies on lesion obliteration (secondary outcome) over a 10-year follow-up.
METHODS: This single-center retrospective cohort included 645 patients with ruptured brain arteriovenous malformations (BAVMs) treated between 2010 and 2019, with a median follow-up of 96 months. Patients were randomly divided into training (70%) and validation (30%) cohorts t

The potential existence and roles of the meningeal lymphatic system in normal and pathological brain function have been a long-standing enigma. Recent evidence suggests that meningeal lymphatic vessels are present in both the mouse and human brain; in mice, they seem to play a role in clearing toxic amyloid-beta peptides, which have been connected with Alzheimer disease (AD). Here, we review the evidence linking the meningeal lymphatic system with human AD. Novel findings suggest that the recently described meningeal lymphatic vessels could be linked to, and possibly drain, the efferent parava

OBJECTIVES: Cerebral edema is a key poor prognosticator in traumatic brain injury. There are no biomarkers identifying patients at-risk, or guiding mechanistically-precise therapies. Sulfonylurea receptor-1-transient receptor potential cation channel M4 is upregulated only after brain injury, causing edema in animal studies. We hypothesized that sulfonylurea receptor-1 is measurable in human cerebrospinal fluid after severe traumatic brain injury and is an informative biomarker of edema and outcome.
DESIGN: A total of 119 cerebrospinal fluid samples were collected from 28 severe traumatic bra

BACKGROUND: Phase-contrast MRI (PC-MRI) offers a rapid, noninvasive technique to directly quantify flow through ventricular shunts, and it serves as an adjunct MRI sequence to inform decision-making in complex postoperative settings.
OBSERVATIONS: A 19-year-old man with Dandy-Walker-associated hydrocephalus underwent proximal catheter exchange of a right-sided parietal ventriculoperitoneal shunt. Postoperatively, he developed intraventricular hemorrhage and symptomatic shunt occlusion and required emergency left frontal external ventricular drain (EVD) placement. After 5 days of drainage, the
