Methadone to buprenorphine/naloxone induction without withdrawal utilizing transdermal fentanyl bridge in an inpatient setting-Azar method.

BACKGROUND AND OBJECTIVES: Although buprenorphine/naloxone is widely recognized as first-line therapy for opioid use disorder, the requirement for moderate withdrawal prior to initiation in efforts to avoid precipitated withdrawal can be a barrier to its initiation. METHODS: We present a case utilizing transdermal fentanyl as a bridging treatment to eliminate withdrawal during the transition from methadone to buprenorphine/naloxone in a patient who had ongoing significant intravenous heroin use while on methadone. RESULTS: Patient was successfully transitioned from methadone to buprenorphine

Isolated myocardial bridging and exercise-related cardiac events.

Myocardial bridging, which is defined as cardiac muscle overlying a part of a coronary artery, is the most common congenital coronary artery anomaly. Myocardial bridging is usually benign, but has been associated with exercise-related cardiac events. Guidelines for athletic participation in these patients are primarily based on reports from the general population with myocardial bridging. We performed a systematic literature search of PubMed, Ovid and Google Scholar for articles addressing exercise-related cardiac events associated with myocardial bridging. We identified 69 cases of which only

[Perioperative handling of anticoagulation].

A growing number of patients in Germany receive a long-term prophylactic anticoagulation with phenprocoumone or one of the novel direct oral anticoagulants (NOAC), such as dabigatran, rivaroxaban or apixaban. The most common indication for an oral anticoagulant therapy is atrial fibrillation (approximately 75%) where the anticoagulant therapy can reduce the risk for an embolic event, particularly stroke by 60%. Operations carried out during such a therapy can result in major bleeding complications. On the other hand, suspending anticoagulant therapy can lead to an increased risk of thromboembo

Biventricular assist device as a bridge to cardiac transplantation in the treatment of peripartum cardiomyopathy.

Peripartum cardiomyopathy is an unexpected complication of the puerperium with a high mortality rate. Appropriate therapy requires accurate identification of this disease, which is frequently difficult in a patient who has been previously healthy. Medical therapy using alteration of intravascular volume (to optimize ventricular preload), the addition of inotropic agents (to correct ventricular function), and intra-aortic balloon counterpulsation (to improve afterload reduction) is the first line of therapy. Surgical therapy, involving cardiac transplantation, is the ultimate treatment. This th

Peri-procedural antithrombotic management: time to burn the bridge?

Emerging evidence suggests the use of peri-procedural bridging during interruptions in warfarin therapy increases bleed risk without reducing thromboembolic events. We implemented a peri-procedural anticoagulant management risk assessment tool in a single, outpatient anticoagulation clinic within an academic teaching institution. In this retrospective, pre-post observational study, we evaluated adults who required an interruption in warfarin therapy for an invasive procedure. The primary outcome was the proportion of patients who received bridging prior to and following implementation of the t

Upper-body extracorporeal membrane oxygenation as a strategy in decompensated pulmonary arterial hypertension.

Pulmonary arterial hypertension (PAH) is a disease with significant morbidity and mortality, particularly during an acute decompensation. We describe a single-center experience of three patients with severe Group 1 PAH, refractory to targeted medical therapy, in which an extubated, nonsedated, extracorporeal membrane oxygenation (ECMO) strategy with an upper-body configuration was used as a bridge to recovery or lung transplantation. All three patients were extubated within 24 hours of ECMO initiation. Two patients were successfully bridged to lung transplantation, and the other patient was op

Short-term mechanical circulatory support as a bridge to durable left ventricular assist device implantation in refractory cardiogenic shock: a systematic review and meta-analysis.

Short-term mechanical circulatory support (MCS) is increasingly used as a bridge to decision in patients with refractory cardiogenic shock. Subsequently, these patients might be bridged to durable MCS either as a bridge to candidacy/transplantation, or as destination therapy. The aim of this study was to review support duration and clinical outcome of short-term MCS in cardiogenic shock, and to analyse application of this technology as a bridge to long-term cardiac support (left ventricular assist device, LVAD) from 2006 till June 2016. Using Cochrane Register of Trials, Embase and Medline, a

An Updated Review on Myocardial Bridging.

Myocardial bridging is a congenital coronary anomaly with normal epicardial coronary artery taking an intra-myocardial course also described as tunneled artery. The majority of patients with this coronary anomaly are asymptomatic and generally it is a benign condition. However, it is an important cause of myocardial ischemia, which may lead to anginal symptoms, acute coronary syndrome, cardiac arrhythmias and rarely sudden cardiac death. There are numerous studies published in the recent past on understanding the pathophysiology, diagnostic and management strategies of myocardial bridging. Thi

Outcome of Veno-Pulmonary Extracorporeal Life Support in Lung Transplantation Using ProtekDuo Cannula: A Systematic Review and Description of Configurations.

Background: Refractory end-stage pulmonary failure may benefit from extracorporeal life support (ECLS) as a bridge to lung transplantation. Veno-venous (VV) extracorporeal membrane oxygenation (ECMO) has been recommended for patients who have failed conventional medical therapy and mechanical ventilation. Veno-arterial (VA) ECMO may be used in patients with acute right ventricular (RV) failure, haemodynamic instability, or refractory respiratory failure. Peripheral percutaneous approaches, either dual-site single-lumen cannulation for veno-pulmonary (VP) ECMO or single-site dual-lumen (dl)VP E

Myocardial bridges: effect of propranolol on systolic compression.

Myocardial bridges may exert an ischemic effect on the myocardium and often warrant therapy including surgical correction in selected cases. Decrease in systolic compression after the administration of propranolol has not been reported. We report a patient who demonstrated angiographic systolic compression of the coronary artery with subsequent improvement on treatment with propranolol. The reduction in severity of systolic compression caused by propranolol may be due to the negative inotropic effect and perhaps from an increase in coronary artery vascular tone secondary to a beta-blocker with

A Therapeutic Dilemma: Catastrophic ST-Elevation Myocardial Infarction Following Antiplatelet Therapy Interruption in a Patient With JAK2-Positive Polycythemia Vera.

Polycythemia vera is associated with a thrombotic-hemorrhagic paradox. We report a 75-year-old woman with recurrent ST-elevation myocardial infarction and stent thrombosis after premature dual antiplatelet therapy interruption. Revascularization was successful, and eptifibatide bridging enabled safe surgery. This case highlights the risks of interruption and the role of bridging therapy.

Clinically significant myocardial bridging.

Myocardial bridging is a common anatomical variant in which a major epicardial coronary artery takes an intramyocardial course, leading to dynamic systolic compression. Because coronary perfusion occurs primarily during diastole, most patients with this anatomical variant have no associated perfusion abnormalities or symptoms. Despite this, there is a subset of patients with myocardial bridging who experience ischaemic symptoms. Determining which anatomical variants are benign and which are clinically relevant remains a challenge. Further complicating the picture, functional factors such as di

Bridging surgery and radiotherapy: the role of stereotactic radiosurgery in pediatric brain tumor care.

BACKGROUND: Pediatric brain tumors (PBTs) are the most common solid malignancy in children. While surgery and fractionated radiotherapy remain cornerstones, stereotactic radiosurgery (SRS) is increasingly used for selected intracranial targets to maximize local control while limiting dose to surrounding normal tissue. However, evidence regarding efficacy and long-term toxicity in children remains uncertain. PURPOSE: This systematic review examines the role of SRS in pediatric brain tumor care, focusing on tumor local control (LC), overall survival (OS), and the toxicity associated with SRS an

Extracorporeal membrane oxygenation as a novel bridging strategy for acute right heart failure in group 1 pulmonary arterial hypertension.

Patients with group 1 pulmonary arterial hypertension (PAH) and decompensated right heart failure (RHF) were not previously considered for extracorporeal membrane oxygenation (ECMO) as bridge to transplantation (BTT) or bridge to recovery (BTR) because options were limited by long transplantation wait times and perceived inability to wean ECMO. In a retrospective review, we describe our center's multidisciplinary mechanical-medical approach to ECMO as a bridging therapy for PAH (2009-2012). Suitability for ECMO was determined using a defined algorithm. Six patients (age, 32 ± 11 years) underwe

Current issues in the antibiotic primary management of the febrile neutropenic cancer patient: a perspective from the National Cancer Institute.

In summary, the overall approach to the treatment of neutropenic cancer patients is to bridge the neutropenic gap. Empirical therapy serves as an important pillar of support, and a rational programme for modifications of the initial therapy, according to the evolving clinical course of the patient, forms another pillar in bridging this gap (Pizzo & Meyers, 1989). The incorporation of biologicals that might shorten the duration of neutropenia could potentially lead to greater improvements in outcome and greater success for therapy for the febrile neutropenic cancer patient.

Thrombectomy Alone or Alongside Intravenous Thrombolysis in Managing Acute Ischemic Stroke Caused by Basilar Artery Occlusion: A Multicenter Observational Study.

BACKGROUND AND PURPOSE: It remains unclear whether the combination of endovascular treatment (EVT) with intravenous thrombolysis (IVT) results in a more favorable functional outcome than EVT alone in managing cases of acute ischemic stroke (AIS) caused by basilar artery occlusion (BAO). Thus, this study aimed to compare the outcomes of 2 approaches-direct endovascular treatment (DEVT) and bridging therapy (IVT plus EVT)-in patients with acute BAO presenting within 4.5 hours of stroke onset. MATERIALS AND METHODS: This multicenter retrospective cohort study included 153 patients with acute BAO

Veno-Arterial Extracorporeal Membrane Oxygenation as a Bridge to Heart Transplant-Change of Paradigm.

Despite advances in medical therapy and mechanical circulatory support (MCS), heart transplant (HT) remains the gold standard therapy for end-stage heart failure. Patients in cardiogenic shock require prompt intervention to reverse hypoperfusion and end-organ damage. When medical therapy becomes insufficient, MCS should be considered. Historically, it has been reported that critically ill patients bridged with veno-arterial extracorporeal membrane oxygenation (VA-ECMO) directly to HT have worse outcomes. However, when the heart allocation system gives the highest priority to patients on VA-ECM
