Beyond RSV: why the bronchiolitis burden in infants will not simply disappear.

Nirsevimab and maternal respiratory syncytial virus (RSV) vaccination are delivering real-world important reductions in RSV-bronchiolitis hospitalisations, a landmark achievement in paediatric preventive medicine. We argue, however, that these successes should not be mistaken for the resolution of severe bronchiolitis as a clinical and public health problem. First, RSV's epidemiological dominance may partly reflect its transmission efficiency rather than a unique pathogenic superiority: rhinovirus, human metapneumovirus, adenovirus, and bocavirus already cause substantial disease, and genetic

Mapping Bronchiolitis in Connecticut Before and After the COVID-19 Pandemic.

OBJECTIVE: New preventive therapeutics could substantially reduce bronchiolitis cases requiring acute care. Understanding the post-pandemic geographic distribution of bronchiolitis morbidity can guide targeted immunization campaigns in high-risk areas. This study aimed to identify geographic clusters of bronchiolitis acute care discharges across Connecticut. METHODS: This retrospective cross-sectional cohort study used a statewide database to identify home ZIP Code Tabulation Areas (ZCTAs) of children under 2 years discharged following bronchiolitis emergency visits or hospitalizations. Disch

Impact of regional heterogeneity of RSV infection prophylaxis on bronchiolitis in Italy.

Respiratory syncytial virus (RSV) remains a leading cause of bronchiolitis and hospitalization in infants across Italy, particularly during predictable seasonal outbreaks. With the 2023-2025 RSV seasons marking the first large-scale rollout of the monoclonal antibody nirsevimab, this study aimed to evaluate the implementation strategies, regional variability, and early clinical impact of RSV prophylaxis at the national level. We conducted a nationwide, multicenter, observational study across 19 Italian regions. Data were collected through the Italian Society of Pediatrics and the Italian Socie

[Respiratory syncytial virus bronchiolitis in newborns: a new era in prevention.].

Respiratory syncytial virus (RSV) prevention in early infancy has entered a new era with the availability of highly effective preventive strategies, particularly the long-acting monoclonal antibody nirsevimab. Real-world evidence from large population-based studies has confirmed a major reduction in RSV-related hospitalizations, pediatric intensive care admissions, and respiratory support requirements among infants receiving nirsevimab during their first RSV season. A recent French nationwide study involving more than 160,000 infants further supports the substantial clinical impact of this app

Pediatric bronchiolitis disease severity is associated with immune checkpoint dysregulation.

BACKGROUND: Immune dysregulation plays a pivotal role in the pathogenesis of bronchiolitis. This study aimed to investigate the role of immune checkpoint molecules and regulatory cytokines in relation to disease severity. METHODS: A prospective cohort of 151 pediatric patients aged 1 month to 2 years was enrolled and classified into control, mild, moderate, and severe bronchiolitis groups. Immune checkpoint molecules (CTLA-4, TIM-3, TIGIT, GARP) and cytokines (IL-2Rα, 4-1BB, TGF-β1, LAG-3, galectin-9) were evaluated in CD4 + FOXP3+ (Treg) and CD4 + FOXP3-T cells, and in plasma using flow cyto

Nasogastric Hydration Utilization in Bronchiolitis, an EMO Trial Observational Substudy.

BACKGROUND: Nasogastric (NG) fluid replacement is an effective and nutritionally beneficial alternative to intravenous (IV) hydration for pediatric inpatients with bronchiolitis. Existing evidence demonstrates equipoise in the use of NG vs IV hydration; however, interinstitutional data on NG hydration utilization are lacking. OBJECTIVES: To describe NG hydration in bronchiolitis across institutions and its association with clinical factors and outcomes. METHODS: This observational multicenter substudy of the Eliminating Monitor Overuse (EMO) Trial collected data on fluid replacement for pedi

Bronchiectasis in Bronchiolitis Obliterans Syndrome after Hematopoietic Cell Transplantation.

Bronchiectasis-bronchial dilatation accompanied by impaired mucociliary clearance, chronic infection, and chronic inflammation-may contribute to poor outcomes in bronchiolitis obliterans syndrome (BOS) complicating hematopoietic cell transplantation, though its epidemiology and impact are poorly understood. We assessed factors associated with bronchiectasis in BOS. We also assessed relationships between bronchiectasis and survival, respiratory infections, and percent predicted forced expiratory volume in one second (FEV1%). This single-center retrospective cohort study included adults who unde

Trends in Lower Respiratory Tract Diagnoses and Nonrecommended Bronchiolitis Therapies.

Primary biliary cholangitis (PBC) is a chronic, cholestatic condition associated with symptoms that directly impact the quality of life in those afflicted with the disease. In addition to pruritus and fatigue, patients with PBC may develop metabolic bone disease from reduced bone density, such as osteopenia and osteoporosis. Osteoporosis increases the risk of fractures, as well as morbidity and mortality. The prevalence of osteoporosis in PBC is expected to increase in conjunction with the rising prevalence of PBC as a whole. Timely diagnosis, prevention and management of osteoporosis are cruc

Influenza C Virus in Children With Acute Bronchiolitis and Febrile Seizures.

BACKGROUND: Influenza C virus (ICV), a lesser-known member of the Orthomyxoviridae family, usually causes mild respiratory illness in children. Due to its low prevalence and clinical similarity to other respiratory infections, it is not routinely screened. This study investigates the detection of ICV in children with acute bronchiolitis (AB) and febrile seizures (FS), comparing the findings with a healthy control group. METHODS: Between October 2009 and September 2011, 499 nasopharyngeal swabs were collected from children up to 6 years old at the University Medical Center Ljubljana. The study

Inpatient Management of Bronchiolitis Before and After the COVID-19 Pandemic.

Introduction Bronchiolitis is the most common cause of respiratory-related hospital admissions in children under two years. It is typically caused by viral infections, most commonly respiratory syncytial virus (RSV). Despite the availability of various international guidelines, Malta lacks local protocols, leading clinicians to rely on international recommendations for management. Objectives The primary aim of this audit was to compare epidemiological and management trends in bronchiolitis among children under one year admitted to Mater Dei Hospital between 2019 (pre-COVID) and 2024 (post-COVI

DNAH9 variants in children with post-infectious bronchiolitis/bronchitis obliterans.

Post-infectious bronchiolitis/bronchitis obliterans (PIBO) is a chronic irreversible obstructive lung disease that results in obstruction and/or obliteration of small airways. Previous reports have indicated that PCD-related gene mutations contribute to PIBO incidence. However, the relationship between DNAH9 variants and PIBO remains unclear. This study aimed to evaluate the association between DNAH9 mutations and the incidence of PIBO. In our cohort, 126 PIBO patients conducted Whole Exome Sequence (WES) test and twelve variants of DNAH9 gene were identified. Detailed clinical information, hi

Incidental diagnosis of cardiac fibroma in an infant with acute viral bronchiolitis.

OBJECTIVE: Pediatric cardiac tumors are rare and, among them, 90% are benign. Cardiac fibroma is the second most frequent tumor, after rhabdomyoma. The objective of this study is to report a case of cardiac fibroma diagnosed incidentally in a patient admitted with acute viral bronchiolitis. CASE DESCRIPTION: A 5-month-old male infant was admitted to the pediatric emergency department with acute viral bronchiolitis requiring hospitalization. He presented a detectable respiratory syncytial virus in oropharyngeal swab, blood test with lymphocytosis and a chest radiography revealed cardiomegaly.

Does weight influence the course of RSV bronchiolitis in hospitalized infants?

Acute bronchiolitis is among the most common causes of hospitalizations in infants worldwide. Associations between weight and severity of respiratory syncytial virus (RSV) bronchiolitis remain unclear. The aim of this study was to evaluate this association. A single-center, retrospective cohort study of infants aged under 24 months, who were hospitalized between 2018 and 2022 for RSV bronchiolitis. Data from computerized medical records were extracted using the MDclone platform. Participants were divided into three groups according to weight percentiles: underweight (below 5th percentile), nor
