Early diagnosis of bilateral sub-deltoid bursitis using clinic-based ultrasonography in a patient receiving infliximab therapy for ulcerative pouchitis.

BACKGROUND: Infliximab, a monoclonal chimeric antibody to tumour necrosis factor (TNF)α, is a novel therapy used in the management of chronic refractory pouchitis that is unresponsive to conventional medical therapy. METHODS: This report describes a case of non-infective bursitis following infliximab therapy and documents the role of musculoskeletal ultrasound in detecting soft tissue fluid collections and in guiding aspiration. CONCLUSION: A high index of suspicion is required when assessing new or worsening musculoskeletal pain in patients receiving infliximab and involvement of a rheumato…

The Journey From Knee to Heart: A Case of Methicillin-Sensitive Staphylococcus aureus Infective Endocarditis Secondary to Septic Prepatellar Bursitis.

Staphylococcus aureus is a cause of life-threatening gram-positive bacteremia and the most common causative organism of septic bursitis. Although there are several case reports of infective endocarditis complicated with septic bursitis and other rheumatic complications, there are very few cases reports of septic bursitis leading to infective endocarditis (IE). We present a patient with a history of recurrent prepatellar bursitis requiring catheter drainage, who presented with sepsis and IE.

Giant cell tumor of the pes anserine bursa (extra-articular pigmented villonodular bursitis): a case report and review of the literature.

Pigmented villonodular synovitis (PVNS) is a rare, benign, proliferating disease affecting the synovium of joints, bursae, and tendon sheaths. Involvement of bursa (PVNB, pigmented villonodular bursitis) is the least common, and only few cases of exclusively extra-articular PVNB of the pes anserinus bursa have been reported so far. We report a case of extra-articular pes anserine PVNB along with a review of the literature. The lesion presented as a painful soft tissue mass in the medial part of the proximal leg. A magnetic resonance imaging showed areas of low to intermediate signals in all se…

Iliopsoas bursitis-associated femoral neuropathy exacerbated after internal fixation of an intertrochanteric hip fracture in rheumatoid arthritis: a case report.

We present the case of a 63-year-old woman with a six-year history of rheumatoid arthritis (RA) and a left iliopsoas bursitis. Radiography had detected destructive changes in her hip joint associated with her bursitis, and she had reported some paresthesia along the left anterior distal thigh. Her pain and numbness remained tolerable, and her disease activity was well controlled until she accidentally fell on the floor, which resulted in an unstable intertrochanteric fracture of left femur with displacement of the proximal portion. The fracture was successfully treated with open reduction and …

Association of Borrelia burgdorferi with nuchal bursitis and elevated outer surface protein A-specific serum antibodies in horses of the northeastern United States.

OBJECTIVE: Evaluate the incidence of Borrelia burgdorferi in cases of equine nuchal bursitis (NB) and investigate the relationship between elevated serum outer surface protein A (OspA) antibodies and the molecular identification of B burgdorferi in bursal tissue or synovial fluid. Additionally, describe clinical cases and compare the histologic changes in NB with and without detection of B burgdorferi. METHODS: This was a retrospective multicenter cohort study (2013 to 2022). Medical records from horses with a diagnosis of NB and B burgdorferi PCR testing on NB tissue or synovial fluid were r…

Comparative efficacy and safety of the non-steroidal anti-inflammatory drugs nimesulide and diclofenac in patients with acute subdeltoid bursitis and bicipital tendinitis.

The efficacy and tolerability of nimesulide, a non-steroidal anti-inflammatory drug (NSAID) 100 mg twice daily were compared with diclofenac 75 mg b.i.d. in short term treatment of acute shoulder (acute subdeltoid bursitis and bicipital tendinitis) in adult patients. In this double-blind (double-dummy), randomised, parallel group study over two weeks, 122 patients were included. The Mann-Whitney statistics revealed therapeutic equivalence of both treatments with a slight superiority for nimesulide. The tolerability of nimesulide, judged by investigators and patients and analysed statistically,…

Differentiating Infective Bursitis and Prosthetic Joint Infection in a Patient With Medial Knee Pain After Total Knee Arthroplasty: A Case Report.

CASE: Pes anserinus bursitis after total knee arthroplasty (TKA) is rare. A 64-year-old woman with well-controlled type 2 diabetes mellitus presented with right-sided medial knee pain and swelling 18 months after bilateral TKA. Initial evaluation raised suspicion of prosthetic joint infection. Imaging and aspiration confirmed isolated pes anserine bursitis. Surgical excision was performed; the culture report showed Candida glabrata. The patient was successfully managed with voriconazole, repeated debridement, and negative pressure wound therapy. CONCLUSION: This is the first reported case of …

Dynamic ultrasound evaluation and treatment of subcoracoid bursitis after HPV vaccination: a case of shoulder injury related to vaccine administration (SIRVA).

Shoulder Injury Related to Vaccine Administration (SIRVA) is a complication caused by improperly administered vaccinations. It triggers an inflammatory cascade that damages shoulder structures, resulting in prolonged shoulder pain and restricted range of motion. Common diagnoses in SIRVA patients include shoulder bursitis (all classified as subacromial bursitis), adhesive capsulitis, and rotator cuff injuries such as tears or tendinopathy. Less frequent diagnoses include biceps tenosynovitis, shoulder synovitis, intramuscular hematoma, and polymyalgia rheumatica or polymyalgia rheumatica-like …

Ultrasound-detected bilateral subacromial-subdeltoid bursitis exceeding 3 mm differentiates polymyalgia rheumatica from rotator cuff tendinopathy: a cross-sectional observational study.

Polymyalgia rheumatica (PMR) is a common inflammatory disorder in the elderly, characterized by pain and stiffness in the shoulder and pelvic girdles. The diagnosis of PMR may be challenging due to its nonspecific clinical features and overlap with other musculoskeletal conditions, such as degenerative rotator cuff tendinopathy (dRCT). Although subacromial-subdeltoid bursitis (bSAD) is included in the 2012 ACR/EULAR classification criteria for PMR, its diagnostic specificity is limited unless further ultrasound (US) parameters are considered. In this cross-sectional study, 20 patients with new…

Changes of synovial fluid protein concentrations in supra-patellar bursitis patients after the injection of different molecular weights of hyaluronic acid.

Knee pain is commonly seen in orthopedic and rehabilitation outpatient clinical settings, and in the aging population. Bursitis of the knee joint, especially when the volume of the synovial fluid is large enough, can compress and distend the nearby soft tissues, causing pain in the knee joint. Out of all the bursae surrounding the knee joint, supra-patellar bursitis is most often associated with knee pain. Treatment strategies in managing supra-patellar bursitis include the aspiration of joint synovial fluid and then followed by steroid injection into the bursa. When supra-patellar bursitis is…

Septic Olecranon Bursitis With Osteomyelitis Attributed to Cutibacterium acnes: Case Report and Literature Overview of the Dilemma of Potential Contaminants and False-Positives.

We report an unusual case of acute septic olecranon bursitis, with probable olecranon osteomyelitis, where the only organism isolated in culture was initially considered a contaminant, Cutibacterium acnes. However, we ultimately considered it the likely causal organism when treatment for most of the other more likely organisms failed. This typically indolent organism is prevalent in pilosebaceous glands, which are scarce in the posterior elbow region. This case illustrates the often challenging empirical management of a musculoskeletal infection when the only organism isolated might be a conta…

Clicking shoulder due to snapping Subacromial subdeltoid (SASD) bursitis: a case report of an unusual presentation in clinical practice with imaging features.

A spectrum of pathologies can cause clicking, popping or snapping of the shoulder joint. These can lead to a patient seeking attention in clinical practice due to pain, discomfort and impact on activities of daily living. Traditionally these can originate from pathologies which involve biceps tendon, tears of the rotator cuff or the glenoid labrum, articular cartilage or due to loose bodies and scapulothoracic joint dysfunction [Guillin et al. in Br J Radiol 85:1343-1353, 2012; Vidoni et al. in Indian J Musculoskelet Radiol 4:61-69, 2022]. Subacromial subdeltoid (SASD) bursitis is a well-descr…

Free anterolateral thigh fasciocutaneous flap with a fat/fascia extension for reconstruction of tendon gliding surface in severe bursitis of the dorsal hand.

A 72-year-old man had severe bursitis in his left dorsal hand after resection of a ganglion twisted around the extensor tendons. After resection of the bursa, a free anterolateral thigh fascial flap with a skin island was used to fill the dead space and to reconstruct a two-layer gliding surface of the extensor tendons. The extensor tendons were wrapped in the fascial flap with the fat layer inside. The flap took completely and the patient was free of bursitis without loss of range of finger motion.

Three-dimensional treatment planning and target volume definition of tendinitis and bursitis - Consensus statement of the International Benign Target Volume Group (IBTVG).

Musculoskeletal disorders, particularly tendinitis and bursitis, are major contributors to global disability and healthcare burden. Low-dose radiotherapy (LDRT) has emerged as a promising treatment for refractory cases, demonstrating anti-inflammatory and anti-proliferative effects, especially in inflammatory conditions. However, the lack of standardized treatment volumes hampers reproducibility, radiation safety, and broader clinical adoption. To address this gap, an international panel of experts applied the Delphi method to develop consensus-based target volumes for LDRT in common indicatio…

During development of rheumatoid arthritis, intermetatarsal bursitis may occur before clinical joint swelling: a large imaging study in patients with clinically suspect arthralgia.

OBJECTIVES: Intermetatarsal bursitis (IMB) represents juxta-articular synovial inflammation of the intermetatarsal bursae. Recent MRI studies identified IMB as feature of early RA, but whether IMB already occurs in the pre-arthritic phase is unknown. We performed a large MRI study in clinically suspect arthralgia (CSA) to assess the occurrence and prognostic value of IMB. METHODS: A total of 577 consecutive CSA patients underwent contrast-enhanced MRI of the forefoot, metacarpophalangeal joints and wrist. MRIs were evaluated for subclinical synovitis/tenosynovitis/osteitis in line with the RA…

Extravascular compression of the femoral vein due to wear debris-induced iliopsoas bursitis: a rare cause of leg swelling after total hip arthroplasty.

We present a patient with unilateral, spontaneous, late leg swelling that developed 4 years after total hip arthroplasty. The etiology was the compression of the internal iliac vein by a voluminous iliopsoas bursitis caused by polyethylene debris. The expansive lesion was detected by ultrasound, arthrography, and magnetic resonance imaging. An ultrasound-guided aspiration provided transient relief of the patient's symptoms. The patient later required surgical excision through an abdominal approach. A second recurrence was detected and treated with revision surgery. We present the diagnosis and…
