[Cutaneous EBV-associated lymphoproliferative disorder in a dermatomyositis treated with immunosuppressive agents].

We report a case of Epstein Barr virus-associated large B cell lymphoproliferative disorder, with an abdominal cutaneous localization, in an adult treated for 10 years with immunosuppressive agents for a dermatomyositis. This is the third case of immunosuppressive induced lymphoproliferative disorder localized to skin in a patient with dermatomyositis. Diagnosis was unexpectedly obtained by the histologic examination of surgical samples of skin necrosis possibly induced by edetate calcium disodium subcutaneous injections in calcinosis cutis.

Transsphenoidal hypophysectomy for four dogs with pituitary ACTH-producing adenoma.

Four dogs diagnosed as pituitary-dependent hyperadrenocorticism were treated with transsphenoidal hypophysectomy and postoperative hormone supplementation therapy. On histological examination of the resected tissues, the tumors were ACTH-producing adenoma of the anterior lobe. Clinical signs such as alopecia and calcinosis cutis, as well as endocrinological abnormalities, were markedly alleviated after surgery. The clinical courses of these 4 dogs suggest that transsphenoidal hypophysectomy may be a useful treatment for pituitary-dependent hyperadrenocorticism.

Nail involvement in systemic sclerosis.

BACKGROUND: Nail involvement has rarely been recognized in systemic sclerosis (SSc). Indeed, only a few small series have assessed nail chan … ontrol subjects. The presence of fingernail changes was associated with digital ulcers (P < .0001), calcinosis cutis (P = .004), and higher values of mean nailfold videocapillaroscopy score (P = .0009). LIMITATIONS: The cohort originated from a single center. CONCLUSION: This study underlines that fingernail changes are correlated with more severe forms of SSc c …

Juvenile Dermatomyositis: A Case Report and Review of Literature.

Juvenile dermatomyositis (JDM) is a systemic inflammatory disease involving children, which primarily affects the skin and the musculoskeletal system. The characteristic findings include Gottron papules, heliotrope rash, calcinosis cutis, and symmetric proximal muscle weakness. Histologically, it is characterized by the presence of lymphocytic vascular inflammation and endothelial swelling. Herein, we report a case of a 10-year-old girl of Indian origin, who presented to us with classical clinical and histological features of JDM.

A case of persistent subepidermal calcified nodule in an adult treated with CO2 laser.

A subepidermal calcified nodule (SCN) is an uncommon form of calcinosis cutis that is not associated with other dermatologic or systemic illnesses. Clinically, SCN presents as a solitary, firm tumor that can be white, white-yellow, or erythematous. Most SCNs occur in children; they are extremely rare in adults. The treatment of choice is believed to be surgical excision. We describe a case of SCN on the right ear lobe of a 33-year-old male, which was treated using a CO2 laser and electrodessication.

Change in calcinosis over 1 year using the scleroderma clinical trials consortium radiologic scoring system for calcinosis of the hands in patients with systemic sclerosis.

INTRODUCTION: Calcinosis cutis is a debilitating complication of systemic sclerosis (SSc). We previously developed a radiographic scoring system to assess severity of calcinosis affecting the hands in patients with SSc. We sought to further validate our radiographic scoring system to assess for change over 1 year and to identify factors associated with improvement or progression. MATERIALS AND METHODS: Baseline and 1-year antero-posterior hand radiographs were obtained in 39 SSc patients with calcinosis prospectively enrolled at 6 centers within the US, Canada, Mexico and Australia. Two reade…

Eruptive syringomas with calcium deposits in a young woman with Down's syndrome.

Eruptive syringomas are uncommon in the general population. We describe here an 18-year-old female, affected by Down's syndrome, who presented with an abrupt eruption of small skin-colored or reddish papules on the face, neck and limbs. Light microscopy allowed us to diagnose syringomas, whereas the study of the ultrastructural features revealed calcium deposits in many lumina and also in the mitochondria. This observation confirms the hypothesis that the syringeal structure plays a role in the pathogenesis of calcinosis cutis.

[Anesthesiologic management in scleroderma patients. Presentation of a clinical case].

The scleroderma (literally, hard skin), or progressive systemic sclerosis (PSS), is a multisystem disease and can present various anaesthetic problems. The anaesthesist should be aware of the difficulty in opening mouth wide enough for laryngoscopy and intubation, the possibility that cardiopulmonary changes may be present and the probability of lesions in oesophagus, bowel, kidneys, skin and joints. In this article the authors describe the anesthetic management of a patient with Thibierge-Weissenbach syndrome (scleroderma with calcinosis cutis) and explore potential problems that should be an…

Further observations on spontaneous neoplasms in the Mongolian gerbil, Meriones unguiculatus.

One hundred sixty Mongolian gerbils, Meriones unguiculatus, were examined for the presence of naturally occurring lesions. The first recognized cases of cecal adenocarcinoma, testicular teratoma, and sebaceous gland pad carcinoma were found. Neoplasms previously reported from the gerbil and also seen in the present study included ovarian theca lutein and granulosa lutein cell tumors, sebaceous gland pad adenoma, cutaneous squamous cell carcinoma, malignant melanoma, and renal and splenic hemangiomas. Calcinosis cutis was also observed in two male gerbils.

A case of CREST syndrome with rapidly progressive liver damage.

A patient with CREST syndrome (calcinosis cutis, Raynaud's phenomenon, esophageal dysmotility, sclerodactyly, and telangiectasia) who had severe jaundice (total bilirubin 29.1 mg/dl) and rapidly progressive liver damage is reported. The liver damage findings matched the criteria of autoimmune chronic active hepatitis (CAH). There have been no prior reports of a case of CREST syndrome with autoimmune CAH in Japan. Anticentromere antibody (ACA) was detected in the serum; ACA seemed to be related to the pathogenesis of these two diseases.

Subcutaneous granuloma annulare mimicking dermatomyositis.

We present three children who presented with papules and plaques over the knuckles, mimicking Gottron's papules of juvenile dermatomyositis, as well as subcutaneous nodules over the joints of the extremities that were initially thought to represent calcinosis cutis. However, thorough clinical and laboratory evaluation, as well as imaging, failed to support this diagnosis. Skin biopsies were consistent with a diagnosis of subcutaneous granuloma annulare. This unique phenotype of granuloma annulare should be recognized in order to prevent erroneous diagnosis and treatment.

Surgical management of autoimmune-associated calcinosis: a dermatomyositis case and case-based review.

Calcinosis cutis is a challenging complication of autoimmune connective tissue diseases, particularly dermatomyositis, and is associated with significant morbidity. Current medical treatments often show limited and inconsistent effectiveness, highlighting the need to better define the role of alternative approaches. To present a case of extensive dermatomyositis-associated calcinosis successfully managed with surgical excision and flap reconstruction, and to systematically review the role of surgical management in autoimmune-related calcinosis. A case report is presented together with a litera…

Multicentric Study on High-Frequency Ultrasound Characterization of Calcium Deposits in Dermal and Subcutaneous Calciphylaxis and Calcinosis.

OBJECTIVES: Calcium depositions are frequent in multiple inflammatory dermatosis, they can be explored by ultrasound (US) but the patterns of these depositions have not yet been described. The aim of this study is to describe different patterns of calcium deposition in inflammatory dermatoses. METHODS: The clinical and US data of 58 patients from 7 different centers with inflammatory dermatosis showing ultrasonography-detected calcium depositions was retrospectively reviewed. RESULTS: Dystrophic calcinosis represented 86.2%, calciphylaxis 8.6%, and metastatic calcinosis 5.2%. Three different

Painful tumors of the skin: "CALM HOG FLED PEN AND GETS BACK".

Painful tumors of the skin present as dermal or subcutaneous nodules. They can originate from several sources: adipose tissue, cartilage deg … ermatofibroma, granular cell tumor, endometriosis, thrombus, scar, blue rubber bleb nevus, angioma, calcinosis cutis, and keloid. An acronym - inspired by Charlotte's Web, a book that many children have read - that can be used as a memory aid for recalling the list of painful skin tumors is introduced: "CALM HOG FLED PEN AND GETS BACK".

Fulminant metastatic calcinosis with cutaneous necrosis in a child with end-stage renal disease and tertiary hyperparathyroidism.

Metastatic calcinosis is a common feature of chronic renal failure. Its first manifestations are bone demineralization and non-visceral and/or visceral calcification with mostly mural deposits in arteries and arterioles. It is initially characterized by hyperphosphataemia followed by secondary or tertiary hyperparathyroidism. Cutaneous involvement is a rare complication. Histologically, the lesions show vascular calcification with ischaemic skin necrosis. Extreme cases may produce calcinosis cutis (calciphylaxis), i.e. disseminated calcification of the subcutaneous tissue and dermis in the for…
