Invasive fungal infections in patients with chronic mucocutaneous candidiasis.

Patients with chronic mucocutaneous candidiasis have persistent Candida infection of nails, skin, and mucous membranes, but rarely, if ever, does an invasive fungal infection develop. We describe two patients with chronic mucocutaneous candidiasis who died of fungal infection; one had C albicans meningitis and the second had Cryptococcus neoformans meningitis. The cases of four other patients with chronic mucocutaneous candidiasis and invasive fungal infection, three with crytococcosis and one with histoplasmosis, have been reported previously. Immunological profiles on these six patients reve…

Experimental murine candidiasis: cell-mediated immunity after cutaneous challenge.

Male CBA/J mice, sensitized by cutaneous inoculation with viable Candida albicans blastospores, were used to study in vivo and in vitro cellular immune responses. Three antigens of C. albicans, viz., a cell wall glycoprotein (GP), a membrane extract (ppt-HEX), and soluble cytoplasmic substances (SCS), were used in vitro in a lymphocyte stimulation assay, whereas the GP and ppt-HEX were used in vivo to detect delayed hypersensitivity by the footpad assay. Delayed hypersensitivity to GP and ppt-HEX was transferred from sensitized donors to naive recipients with peritoneal exudate cells and not w

Invasive Candidiasis in Liver Transplant Recipients Undergoing Antifungal Prophylaxis.

INTRODUCTION: Invasive candidiasis (IC) is the most frequent invasive fungal infection (IFI) affecting liver transplant (LT) recipients. Antifungal prophylaxis (AP) effectively reduces the incidence IFI after LT. However, a considerable proportion of LT recipients undergoing AP are still affected by IC. OBJECTIVES: To assess the incidence and factors associated with IC in high-risk LT recipients undergoing AP. PATIENTS AND METHODS: Retrospective single-center study including high-risk LT recipients undergoing AP between 2015 and 2020. The primary outcome was the occurrence of IC within the f…

Biofilm-Associated Candidiasis: Pathogenesis, Prevalence, Challenges and Therapeutic Options.

The rising prevalence of fungal infections, especially those caused by Candida species, presents a major risk to global health. With approximately 1.5 million deaths annually, the urgency for effective treatment options has never been greater. Candida spp. are the leading cause of invasive infections, significantly impacting immunocompromised patients and those in healthcare settings. C. albicans, C. parapsilosis and the emerging species C. auris are categorized as highly dangerous species because of their pathogenic potential and increasing drug resistance. This review comparatively describes

Diagnostic Approaches to Invasive Candidiasis: Challenges and New Perspectives.

Invasive candidiasis is a serious infectious disease that affects 2-10% of patients in the intensive care unit. It is caused by fungi of the genus Candida, and its diagnosis relies on multiple complex laboratory methods. Blood culture remains the gold standard for yeast detection in the bloodstream but has limited clinical utility because of its low sensitivity and prolonged turnaround time. Newer diagnostic approaches, such as molecular methods for Candida spp. DNA detection, which enable faster identificationbut still entail limitations, including variability in sensitivity. Another valuable…

Drug design strategies for the treatment azole-resistant candidiasis.

INTRODUCTION: Despite the availability of novel antifungals and therapeutic strategies, the rate of global mortality linked to invasive fungal diseases from fungal infection remains high. Candida albicans account for the most invasive mycosis produced by yeast. Thus, the current arsenal of medicinal chemists is focused on finding new effective agents with lower toxicity and broad-spectrum activity. In this review article, recent efforts to find effective agents against azole-resistant candidiasis, a common fungal infection, are covered. AREAS COVERED: Herein, the authors outlined all azole-ba…

Salivary Histatin 5 Level in Women with Vaginal Candidiasis.

Histatins (Hsts) are considered a prominent member of antimicrobial peptides rich in histidine, bearing antifungal activity against Candida species. Hst5 is the most effective among them. Although Hst5 is not found in the cervicovaginal fluid, it has been detected in the human serum. Saliva acts as a mirror, reflecting the cause and effect relationship between several diseases. We aimed to show the salivary Hst5 levels with vaginal candidiasis. Women in the reproductive age group (18-50 years) were enrolled in the study. Patients and controls were classified based on the presence or absence of…

Smoking-aggravated oral candidiasis: Nrf2 pathway dampens NLRP3 inflammasome.

While cigarette smoke compounds are known to have immunosuppressive effects on the oral mucosa, the relationship between in vivo immune dysfunction caused by smoking and the development of oral Candida infections remains largely unexplored. In a recent issue of The Journal of Cellular and Molecular Medicine, Ye and colleagues provide evidence that smoking increases oral mucosa susceptibility to Candida albicans infection via the activation of the Nrf2 pathway, which in turn negatively regulates the NLRP3 inflammasome. This opens new perspective in considering Nrf2 as a relevant target for smok

Does probiotics work for bacterial vaginosis and vulvovaginal candidiasis.

The different Lactobacillus strains of probiotics have been applied to the treatment and prevention of bacterial vaginosis and vulvovaginal candidiasis. The experimental data demonstrated that it works well via reducing the number of harmful bacteria, maintaining the acidic microenvironment, inhibiting the immune response, and so on, to restore the vaginal microecology. However, the clinical data indicated that it is not sufficient to support the use of probiotics in the intervention of vulvovaginal candidiasis rather than bacterial vaginosis. Hunting for novel probiotic strains and uncovering…

A Complex Microbial Interplay Underlies Recurrent Vulvovaginal Candidiasis Pathobiology.

While extremely prevalent, painful, and difficult to treat, vulvovaginal candidiasis remains largely understudied in the field of women's health. In a recent issue of mSystems, McKloud et al. (E. McKloud, C. Delaney, L. Sherry, R. Kean, et al., mSystems 6:e00622-21, 2021, https://doi.org/10.1128/mSystems.00622-21) shed light on a pivotal role of a complex Candida-Lactobacillus interplay that may regulate the pathophysiology of recurrent vulvovaginal candidiasis (RVVC). This advancement not only gives new insight into the molecular mechanisms governing interkingdom interactions modulating RVVC …

Co-Occurrence of Chronic Mucocutaneous Candidiasis with Woolly Hair.

Chronic mucocutaneous candidiasis is an immunodeficiency state, inherited or acquired, characterized by recurrent and/or persistent candidiasis of skin, nails, and mucous membranes. Woolly hair is a congenital structural anomaly of scalp hair, characterized by light-colored, short, extremely kinky, and thin hair due to premature termination of anagen phase of hair cycle. Both conditions are known to present in syndromic and non-syndromic forms. We report the co-occurrence of both these conditions in a 10-year-old female child. The diagnosis was confirmed with clinical, trichoscopic, microbiolo…

Hepato-splenic candidiasis confirmed following splenectomy: a case report.

We report a case of severe refractory hepato-splenic candidiasis confirmed following splenectomy in an immunocompromised 54-year-old woman treated for acute lymphoblastic leukemia. Liver biopsies did not contribute to identifying the etiology of the multiple hepato-splenic lesions. In view of the lack of improvement after several lines of antifungal treatments, a splenectomy was performed and confirmed splenic candidiasis. PET/CT scan was used to monitor the evolution of hepato-splenic hypermetabolism. Splenectomy can be envisaged to establish fungal infection in immuno-compromised patients, t…

Invasive candidiasis in critical care: challenges and future directions.

Invasive candidiasis is the most common critical care-associated fungal infection with a crude mortality of ~ 40-55%. Important factors contributing to risk of invasive candidiasis in ICU include use of broad-spectrum antimicrobials, immunosuppressive drugs, and total parenteral nutrition alongside iatrogenic interventions which breach natural barriers to infection [vascular catheters, renal replacement therapy, extracorporeal membrane oxygenation (ECMO), surgery]. This review discusses three key challenges in this field. The first is the shift in Candida epidemiology across the globe to more …
