Cardiology perspective on seizure-related bradyarrhythmias and SUDEP: A survey study.

OBJECTIVE: To determine cardiologist knowledge of and experience with seizure-related bradyarrhythmias and sudden unexpected death in epilepsy (SUDEP). BACKGROUND: Autonomic changes related to acute seizures are common and can occur during the ictal or postictal period. Two concerning changes in these periods are significant bradycardia and asystole. Postictal asystole has been investigated as a potential mechanism for SUDEP. METHODS: A 27-question survey delivered to cardiologists and cardiology fellows assessed demographics, personal experience, and training involving SUDEP and seizure-rel…

Assessment of blood pressure skills and belief in clinical readings.

BACKGROUND: Accurate blood pressure (BP) measurement is essential for the diagnosis and management of hypertension. In clinical practice, BP is estimated using noninvasive methods with significant variability of application of guidelines in clinical practice, impacting the accuracy and certainty of BP measurements. OBJECTIVE: We sought to assess how BP is measured in clinical practice. METHODS: A survey was administered through professional societies that included predominantly cardiologists. Assessment of adherence to guideline recommendations for BP assessment was measured and compared to …

Outpatient Hypertension Control and Prescribing Habits for Hypertension in Taiwan.

BACKGROUND: Hypertension control is of the utmost importance for reducing cardiovascular risk. In Taiwan, the hypertension control rate of the general population is low (25%). We investigated the factors affecting outpatient hypertension control to determine whether the low control rate stems from clinician unawareness or inadequate public education. METHODS: Hypertensive patients were recruited between 2003 and 2004 by 13 cardiologists and 9 non-cardiologists from 19 hospitals distributed across four geographical areas of Taiwan. Each clinician recruited 100 consecutive patients from outpati…

A deep neural network using audio files for detection of aortic stenosis.

BACKGROUND: Although aortic stenosis (AS) is the most common valvular heart disease in the western world, many affected patients remain undiagnosed. Auscultation is a readily available screening tool for AS. However, it requires a high level of professional expertise. HYPOTHESIS: An AI algorithm can detect AS using audio files with the same accuracy as experienced cardiologists. METHODS: A deep neural network (DNN) was trained by preprocessed audio files of 100 patients with AS and 100 controls. The DNN's performance was evaluated with a test data set of 40 patients. The primary outcome meas…

Comparison of fluoroscopic coronary angiography and multi-slice coronary angiography in the characterization of anomalous coronary artery.

Anomalous coronary arteries with an inter-arterial course are associated with sudden cardiac death. We reported a study comparing the accuracy of fluoroscopic coronary angiography (FCA) with that of multi-slice computed tomography (MSCT) coronary angiography in determining the proximal course of anomalous coronary arteries. Twelve patients with thirteen anomalous coronary arteries had both FCA and MSCT coronary angiography were included in this study. Twelve cardiologists individually reviewed FCAs of anomalous coronary arteries and determined the proximal course of anomalous coronary arteries…

Physician variation in anticoagulating patients with atrial fibrillation. Dartmouth Primary Care COOP Project.

We investigated variations in the oral anticoagulant treatment of atrial fibrillation by physicians in three specialties: family physicians (or general practitioners), general internists, and cardiologists. Results showed general agreement in the anticoagulation decision regarding patients with either mitral valve disease or a history of chronic alcohol abuse, but substantial disagreement in other categories of patients. Estimations of the risk of embolization and risk of hemorrhage differed widely among all physicians, cardiologists generally rating the embolization risks lower than the other…

Audibility of an artificial third heart sound in relation to its frequency, amplitude, delay from the second heart sound and the experience of the observer.

The possibility of detecting synthetic third heart sounds was studied. A special unit was used that added a sound to a previously recorded phonocardiogram. The sound could be changed in frequency, amplitude and delay from the second heart sound. Four groups of observers--cardiologists, residents, nurses and students--listened to 32 random examples. Detection rate increased with experience of the observer (p less than 0.0001) as well as with amplitude (p less than 0.0001), frequency (p less than 0.0001) and delay of the sound (p less than 0.05). The sensitivity was highest among the cardiologis…

Accuracy of the initial evaluation of heart murmurs in neonates: do we need an echocardiogram?

The objective of this study was to determine the differences between neonatologists and pediatric cardiologists with regards to the initial assessment of neonatal heart murmur and to evaluate the role of echocardiography in this group of patients. During a period of 1 year, all neonates with heart murmur seen in pediatric cardiology consultation from neonatal intensive care units at the Children Hospital of Eastern Ontario and Ottawa General Hospital were included in this study. Neonates with heart murmur were initially evaluated clinically by a neonatologist and the most likely clinical diagn…

Patent ductus arteriosus: an analysis of management.

To compare differences in the management of the neonatal patent ductus arteriosus between neonatologists and paediatric cardiologists, physicians throughout the South-West were contacted. In treatment-refractory cases, neonatologists considered ligation less frequently than paediatric cardiologists (0% versus 40%; p<0.05) and held haemodynamic effects more important for ligation decisions [median: 5 (range 2-5) versus median: 4 (range 2-5); p<0.05]. Furthermore, 81% felt the current guidelines were insufficient.

A case of acute thrombotic occlusion of an anomalous origin of the right coronary artery from the left coronary sinus: focus on the importance of dual-source computed tomography for failed emergency coronary angiography.

Coronary artery anomalies in patients undergoing coronary angiography are often technically challenging for invasive cardiologists and may delay revascularization time. We report a patient who underwent successful bailout revascularization using dual-source computed tomography after failed emergency angiography. This case emphasizes the utility of dual-source computed tomography, especially in an urgent clinical setting, for allowing interventional cardiologists to rapidly identify and effectively treat the aberrant coronary artery.

Augmented detection of septal defects using advanced optical coherence tomography network-processed phonocardiogram.

BACKGROUND: Cardiac auscultation is a traditional method that is most frequently used for identifying congenital heart disease (CHD). Failure to diagnose CHD may occur in patients with faint murmurs or obesity. We aimed to develop an intelligent diagnostic method of detecting heart murmurs in patients with ventricular septal defects (VSDs) and atrial septal defects (ASDs). MATERIALS AND METHODS: Digital recordings of heart sounds and phonocardiograms of 184 participants were obtained. All participants underwent echocardiography by pediatric cardiologists to determine the type of CHD. The phon…

The Impact of Virtual Fractional Flow Reserve and Virtual Coronary Intervention on Treatment Decisions in the Cardiac Catheter Laboratory.

BACKGROUND: Using fractional flow reserve (FFR) to guide percutaneous coronary intervention for patients with coronary artery disease (CAD) improves clinical decision making but remains underused. Virtual FFR (vFFR), computed from angiographic images, permits physiologic assessment without a pressure wire and can be extended to virtual coronary intervention (VCI) to facilitate treatment planning. This study investigated the effect of adding vFFR and VCI to angiography in patient assessment and management. METHODS: Two cardiologists independently reviewed clinical data and angiograms of 50 pat…

Health care services provided during physician office visits for hypertension: differences by specialty.

The changing health care system has reduced patients' access to specialty care. Often, patients with hypertension visit noncardiologists. The objective of this study is to compare differences by physician specialty in the provision of health care services during office visits for hypertension. The authors examined office visits for US physicians by using data from the National Ambulatory Medical Care Survey for 2003 to 2005. Of more than 274 million hypertension visits, 35.5%, 43.9%, 8.5%, and 12.1% visits were made to general practitioners/family physicians, internists, cardiologists, and oth…

Atrial fibrillation in the elderly: physicians' attitudes to anticoagulation.

The use of warfarin and aspirin for the primary prevention of stroke in elderly patients with atrial fibrillation (AF) is controversial. To establish current practice we circulated a questionnaire to 300 geriatricians (G) and 300 cardiologists (C). The response rates were 47% G and 51% C. Most physicians prescribed warfarin in AF associated with mitral stenosis (G vs C, 86% vs 89%, NS). Cardiologists were more likely to prescribe warfarin in AF associated with dilated cardiomyopathy (G vs C, 52% vs 86%, P < 0.01). A minority would prescribe warfarin in aortic valve disease and AF (G vs C, 37% …
