BACKGROUND: C1 fracture accounts for 2% of all spinal column injuries and 10% of cervical spine fractures, and is most frequently caused by motor vehicle accidents and falls. We present a rare case of C1 anterior arch fracture following standard foramen magnum decompression for Chiari malformation type 1.
CASE DESCRIPTION: A 63-year-old man underwent standard foramen magnum decompression (suboccipital craniectomy and C1 laminectomy) under a diagnosis of Chiari malformation type 1 with syringomyelia in June 2009. The postoperative course was uneventful until the patient noticed progressive pos

CASE: We describe the clinical presentation, management, and outcomes of 3 patients, aged 1.5 to 4 years, with C1 synchondrosis ("Jefferson") fractures treated nonoperatively. Minimally widened anterior arch fractures were identified on computed tomography that were not visible on conventional radiographs. Serial imaging showed fracture healing in all patients at minimum 1-year follow-up.
CONCLUSION: These cases highlight the susceptibility of preschool-aged children to "baby Jefferson" fractures through the anterior synchondroses, the importance of early computed tomography imaging when conv

OBJECTIVE: Sella turcica abnormalities such as sella turcica bridging (STB) have been observed frequently among patients with craniofacial anomalies. Ponticulus posticus (PP) is an important structure and its presence causes complications during surgeries. Due to the importance of these structures, this study aimed to evaluate the prevalence of STB, PP, and dimensions of the sella turcica in individuals with and without cleft.
DESIGN: A cross-sectional study.
SETTING: Orthodontic and Radiology Departments in a public dental school, Tehran, Iran.
PATIENTS: Eighty-three lateral cephalograms o

PURPOSE: To compare the Atlas (C1) lateral mass screw placement between screw trajectories of 0° and 15° medial angulation while using the intersection between lateral mass and inferomedial edge of the posterior arch.
METHODS: Forty-eight Atlas lateral masses were prepared and divided into 2 groups: Group 1; screws inserted at 3 mm lateral to the reference point with screw trajectory of 0° angulation(N = 24) and Group 2; those inserted with screw trajectory of 15° medial angulation(N = 24). We evaluated the atlas anatomy, screw purchase and the presence of any breaches using CT scan.

Fractures of the odontoid process seem to have an unclear and not well-understood behavior. This is well demonstrated in the literature as there is a disagreement in many fields, including the patterns of fractures, the percentage of pseudarthrosis or the appropriate treatment that is required. This situation makes the fracture itself a "challenging" one. Indeed, despite the existence of extensive literature, there are still many unresolved problems concerning these fractures. We recognize six main classification schemes of the odontoid process fractures which clearly demonstrate the presence

Fusion of the atlas with the occipital bone is a rare congenital dysplasia known as occipitalization of the atlas, occipitocervical synostosis, assimilation of the atlas, or atlanto-occipital fusion. It is a component of the paraxial mesodermal maldevelopment and commonly associated with other dysplasias of the craniovertebral junction. External auditory canal atresia or external aural atresia is a rare congenital absence of the external auditory canal. It occurs as the consequence of the maldevelopment of the first pharyngeal cleft due to defects of cranial neural crest cells migration and/or

Congenital spinal fusion of an odontoid process to an atlantal hemiarch is very rare. The unfamiliarity of the medical fraternity with this congenital malformation can easily be mistaken for an acute fracture, chronic infection or inflammatory disease. We present our experience of managing an adult who presented with neck pain after a motor vehicle accident. Radiological investigation revealed congenital fusion of the odontoid process to the atlantal hemiarch. The prevalence, embryology and clinical significance of this anomaly are discussed. As the natural progression of this anomaly is not w

The case is reported of an unusual atlas fracture with no reports of such an injury in the literature. The diagnosis of this injury emphasises the importance of simple clinical decision instruments, and systematic interpretation of investigations.

STUDY DESIGN: A case report of a patient with a congenital anomaly of the posterior arch of the atlas and review of the literature are reported.
OBJECTIVE: To describe a unique presentation of cervical myelopathy caused by a mobile isolated bone fragment in an otherwise healthy young male. The description is supplemented by a video showing the repeated trauma the bone fragment caused to the dorsal spinal cord with neck extension.
SUMMARY OF BACKGROUND DATA: Congenital anomalies of the posterior arch of C1 are well described and are almost always asymptomatic and found incidentally. Neurologi

This study tries to explain the reason why the Jefferson fracture is a burst fracture, using two different biomechanical models: a finite element model (FEM) and a cadaver model used to determine strain distribution in C1 during axial static compressive loading. For the FEM model, a three-dimensional model of C1 was obtained from a 29-year-old healthy human, using axial CT scans with intervals of 1.0 mm. The mesh model was composed of 8200 four-noded isoparametric tetrahedrons and 37,400 solid elements. The material properties of the cortical bone of the vertebra were assessed according to the

We report the data on 42 monaural BAEP recordings from 10 patients with craniovertebral malformations. Of 8 preoperative recordings from 3 patients 6 showed an increased I-III interpeak latency (IPL). Of 28 recordings taken shortly (1-12 months) after surgery 21 showed an abnormal I-III IPL. On the other hand, of 6 recordings from 3 patients 20 years after surgery 5 were in the normal range and 1 showed an abnormal I-III IPL. Although the most important site of craniovertebral malformations is the cervicomedullary junction, the results reported suggest unilateral or asymmetrical bilateral invo

PURPOSE: Hemivertebra within the craniovertebral junction is a rare but complex spinal deformity. Torticollis caused by hemi-atlas is extremely rare. There is no consensus on the treatment of these patients. We present our experience with one case of hemi-atlas excision and torticollis correction via a combined anterior-posterior surgical approach and short-segment fixation.
METHODS: An 11-year-old girl with progressive torticollis due to hemi-atlas underwent surgery consisting of combined anterior-posterior hemivertebra resection and instrumentation and had a follow up of 20 months. Pre- and
