Neurogenic thoracic outlet syndrome.

Current concepts for the diagnosis of neurogenic thoracic outlet syndrome are presented together with the surgical experience and results in series of 51 patients caused by a cervical rib. Surgical treatment is recommended in patients with persistent and disabling symptoms not responding to conservative therapy. In carefully selected patients good to excellent results can be achieved.

[The upper thoracic outlet vascular syndrome].

A 16 patients with 20 vascular TOS have been evaluated at the our Institute. Fourteen of them were female, and 2 male patients, with average age of 33.1 (18-44) years. 19 of them had congenital, and one acquired TOS after trauma at neck-shoulder region. 13 cases had arterial, and 7 venous TOS. In 10 cases a cause of TOS was cervical rib, in one scar tissue after clavicle fracture, while in 9 soft tissue anomalies. Eight cases with arterial TOS had a hand ischemia, one TIA and 5 periodical symptoms only during the arm hyperabduction. Two cases with venous TOS also had symptoms and signs during …

Magnetic Resonance Imaging-Negative Lupus Myelitis: A Narrative Review.

Magnetic resonance imaging (MRI)-negative lupus myelitis (MNLM) is a rare neurological manifestation of systemic lupus erythematosus (SLE). Diagnosing MNLM without prior systemic involvement remains challenging. This review aims to analyze all published cases and compare MNLM patients with (Group A) and without prior SLE diagnosis (Group B), emphasizing on demographic and clinical profiles, treatment, and outcomes. A PubMed literature review on MNLM was conducted. Demographic profile, clinical presentations, and management were analyzed. MNLM patients were categorized into Group A and B and co

The scalenus medius band and the seventh cervical transverse process.

A case is reported of compression of the lowest trunk of the brachial plexus by a fibrous scalenus medius band. The band, when present, connects the transverse process of the seventh cervical vertebra, to the first rib. Its presence may on occasions be inferred from the abnormal shape of the seventh cervical transverse process. Similar case reports are reviewed. A study of the length and shape of the transverse process in symptoms-free patients was contrasted with the appearance in a patient with a scalenus medius band and symptoms of brachial neuritis. It is concluded that the shape rather th

[Metastatic sternal tumor from thyroid papillary carcinoma; report of a case].

A 68-year-old male was referred to our department for treatment of a metastatic sternal tumor in the manubrium sterni. Primary lesion was papillary carcinoma of the left lobe of the thyroid gland. Total thyroidectomy, cervical lymph node dissection, resection of manubrium sterni with concomitant resection of bilateral clavicles, 1st ribs and 2nd ribs, and chest wall reconstruction using Marlex Mesh were performed on January 29, 2001. Considering relatively good prognosis and good response to multimodality therapy, surgical resection of sternal metastatic lesion from differentiated thyroid canc

Thoracic outlet syndrome. Results of 282 transaxillary first rib resections.

Thoracic outlet syndrome may follow trauma but also may be seen as a result of postural abnormalities of the shoulder girdle. Cervical ribs and other anatomic variations are not prerequisites for the diagnosis, although they may be more common in patients with thoracic outlet syndrome. The diagnosis is made by history and physical examination. There is no reliable laboratory diagnostic test to confirm or exclude the diagnosis. Proper selection of candidates for surgery can produce excellent and good results in a high percentage of cases. The transaxillary approach to first rib resection is tol…

Arterial thoracic outlet syndrome.

Arterial vascular complications resulting from thoracic outlet compression, although rare, can be substantial and potentially limb threatening. Bony abnormalities such as cervical ribs can lead to chronic trauma to the subclavian artery. Early on, the clinical symptoms can be subtle and confusing, causing potential delay in diagnosis.

Primary hyperparathyroidism due to ectopic parathyroid adenoma with bone lesions mimicking skeletal neoplasm: a case report.

BACKGROUND: Parathyroid adenoma accounts for the majority of primary hyperparathyroidism (PHPT) cases. However, ectopic presentations-particularly those arising outside the typical cervical anatomical landmarks-remain rare and diagnostically challenging. These lesions often mimic skeletal pathologies, thereby increasing the risk of misdiagnosis. CASE DESCRIPTION: We report the case of a 60-year-old male tile-layer who was admitted with a 10-month history of progressive left-sided rib pain. The medical history of the patient was notable for multiple fractures and chronic low back pain. On phys

Assessment of Neck Imbalance in Adolescent Idiopathic Scoliosis Patients: A Cross-Section Study Based on Body Image of 115 Patients with Main or Double Thoracic Curve.

OBJECTIVE: Neck imbalance negatively affects body aesthetics of adolescent idiopathic scoliosis (AIS) patients. The evaluation of neck imbalance is currently limited to radiographic parameters, but lacks visual indicators. Therefore, the purpose of this study was to establish indexes of neck imbalance based on body image and to investigate whether these indexes can truly reflect neck imbalance in AIS patients. METHODS: We performed a cross-sectional study at a single institution between June 2017 and September 2020 and there were 115 subjects involved in this research. All patients were diagn

A neonate with an anomalous bone attached to the cervical spine.

BACKGROUND CONTEXT: A digit/rib-like ectopic bone is a rare congenital anomaly that is most commonly seen in the thorax or pelvis. There is a single report of an adult possessing a phalanx-like bone in the cervical region; however, whether the abnormal bone was congenital or acquired remains elusive. PURPOSE: To elucidate that a phalanx-like bone in the cervical region represents a congenital anomaly. STUDY DESIGN: Case report. METHODS: Report of the imaging findings in a neonate with a palpable bone projection in the posterior neck. RESULTS: Plain radiographs demonstrated an ectopic bone

Adding Rib Mobilization to Diaphragm Release Techniques in Patients With Non-Specific Neck Pain: Randomized Controlled Trial.

BACKGROUND: Non-specific neck pain (NSNP) is a frequent issue that can negatively affect both mobility and function. Recently, there has been growing interest in newer therapeutic approaches, including rib mobilization and diaphragm release techniques, as potential ways to address NSNP and support better outcomes for those affected. PURPOSE: To find out the immediate effects of how (DRT) combined with (RMT) affects the level of pain and the extent to which patients' functional abilities are improved in cases of NSNP. METHODS: For this prospective RCT, 96 participants aged 20 to 45 years were

Cervical lung herniation of the azygous lobe: a case report and literature review.

PURPOSE: To report an adult case of cervical lung herniation involving the azygous lobe and to review the literature. METHODS: A trauma patient was incidentally found to have a cervical lung herniation involving the azygous lobe. We reviewed the literature on cervical lung herniation, described techniques used to diagnose and evaluate cervical lung herniations and summarized management options. RESULTS: Cervical lung herniation results in a defect in Sibson's fascia allowing the apical lung to protrude above the thoracic inlet. In the adult population, these are commonly seen after trauma or

Biomechanical properties of the costovertebral joint.

Proper modeling of the human trunk requires a quantitative assessment of the stiffness of the costovertebral joints. Twelve samples (adjacent thoracic vertebrae and one rib segment) were harvested from three subjects. The ribs were loaded in the cranial-caudal direction, the ventral-dorsal direction and in torsion around the cervical rib axis. The force applied to and the displacement of the loading point on the rib were measured and used to determine the moment-angle responses. Characteristic average curves and boundary curves containing the dataset were developed. The torsion response presen…

Operative treatment of thoracic outlet syndrome in Finland.

The data from the nationwide hospital discharge register was used for collecting the diagnoses of the thoracic outlet syndrome (TOS) which w … sion in 53%, TOS NUD (not classified) in 21%, TOS with subclavian artery compression in 19%, with a cervical rib in 4% and TOS with venous compression in 3%. The large proportion of the diagnosis TOS NUD clearly shows the need for a better definition for the TOS diagnosis. As long as clear diagnostic criteria are lacking, the division of TOS into subgroups is …

[Pancoast-Tobias syndrome of hydatid etiology].

The authors report a case of right-sided complete Pancoast and Tobias syndrome related to a multivesicular extrapleural hydatid cyst developing at the cervico-thoracic area with involvement of the first rib. This occurred in a 27 year old country dweller who was a non-smoker who presented with a progressive history of right sided cervico-brachial neuralgia some four months before hospitalisation, there was a subclavicular tumour swelling (8-10 cm), solid, non-inflammatory and a right sided Claude-Bernard-Horner syndrome. Screening of the thorax showed an opacity at the right apex, dense and ho

High proximal pouch esophageal atresia with vertebral, rib, and sternal anomalies: an additional component to the VATER association.

The base of the proximal esophageal pouch in babies born with esophageal atresia varies in location from the low cervical region to the upper third of the mediastinum, but the relevance of this finding to the VATER association is not known. The initial radiographs and hospital records of 42 newborns with esophageal atresia and a distal tracheoesophageal fistula treated in the Newborn Special Care Unit of the Yale-New Haven Hospital between 1977 and 1986 were examined to (1) determine the location of the base of the upper esophageal pouch on its superimposed vertebral body; (2) identify coexist
