Psychological distress was observer-rated with the Brief Psychiatric Rating Scale (BPRS) and self-rated with the Symptom Questionnaire (SQ) every other week for a 6-month period in 9 apparently remitted bipolar patients attending a lithium clinic. Substantial clinical fluctuations in psychopathology were observed with both rating methods. Compared to normal control subjects, patients suffering from bipolar disorder displayed significantly higher BPRS and SQ anxiety, somatization and total distress scores.

Flexor and extensor postural reflexes are most commonly seen in coma after structural brain lesions, but may also occur in metabolic coma. We report 10 patients who exhibited flexor or extensor posturing as an early and transient feature of coma resulting from sedative drug ingestion. In comatose patients with normal pupillary reactivity but no ocular movements, these postural responses suggest sedative drug overdose.

A combined cholestatic-hepatocellular injury and noncaseating granulomas occurred in two patients 1 and 4 weeks after phenylbutazone therapy. Both patients were jaundiced, one had a macular rash, and both had peripheral blood eosinophilia. Symptoms and signs subsided, and abnormal findings from tests of hepatic function rapidly returned to normal following withdrawal of the drug. Sections of liver biopsy specimens 6 months later showed no granulomas or other pathologic changes. Previously reported cases are reviewed.

While benzodiazepines possess many positive attributes for the treatment of alcohol withdrawal, they are not ideal drugs. The occurrence of toxic effects is not infrequent, and unless recognized promptly, toxicity may significantly impact treatment and length of stay. Use of shorter acting benzodiazepines may result in seizure activity under certain conditions of withdrawal.
