[Neonatal cholestasis: the delay in referring patients for differential diagnosis].

OBJECTIVE: An efficient treatment of extrahepatic biliary atresia demands that the diagnostic differentiation between intra- and extrahepatic neonatal cholestasis be performed by the eighth week of life. This study aimed at evaluate the age of the patients admitted to a general hospital for differential diagnosis of cholestatic jaundice. METHODS: Forty nine children from the Pediatric Service at Hospital de Clínicas, in Porto Alegre, have been studied between 1984 and 1991, according to the protocol for diagnostic elucidation followed by this hospital, which includes biliary tract scintigraph…

Ultrasonographic findings in a cow with extra-hepatic cholestasis and cholangitis.

This case report describes a 6-year-old Holstein Friesian cow with extrahepatic cholestasis. The most prominent clinical signs were disturbed general condition and behaviour, icterus, yellowish brown urine, and bilirubinuria. An increased concentration of serum bilirubin and an increase in hepatic enzyme activity were the most important laboratory findings. Ultrasonographic examination of the liver and gall bladder revealed severe bile stasis. The intrahepatic bile ducts, the cystic duct, and the gall bladder were severely dilated. Histological examination of a liver biopsy revealed cholestasi…

A controlled trial of naloxone infusions for the pruritus of chronic cholestasis.

To test the hypothesis that opioid agonist activity contributes to the pruritus of cholestasis, a placebo-controlled single-blinded trial of naloxone, an opioid antagonist, was conducted in eight patients with primary biliary cirrhosis. After discontinuation of all conventional antipruritic medications, one or two continuous (24-hour) IV infusions of naloxone (0.2 micrograms.kg-1.min-1) and placebo solution were administered consecutively in an order that was not predetermined. Pruritus was assessed subjectively by means of four hourly recordings of a visual analogue score. In addition, object…

Intrahepatic cholestasis and phospholipidosis associated with the use of trimethoprim-sulfamethoxazole.

Although liver injury after administration of the trimethoprim-sulfamethoxazole combination is rare, hepatocellular necrosis and cholestasis have developed in a few cases. We describe a patient who developed a severe, prolonged cholestatic reaction after trimethoprim-sulfamethoxazole administration. The findings from serial liver biopsy samples showed characteristic abnormalities of phospholipidosis that have not been previously described for trimethoprim-sulfamethoxazole-related hepatic injury. The most prominent finding on electron microscopic evaluation of the liver was the presence of prom…

Hepatobiliary scintigraphy and the string test in the evaluation of neonatal cholestasis.

We evaluated [99mTc]diisopropylphenyl-carbamoylmethylimidodiacetic acid ([99mTc]DISIDA) cholescintigraphy with measurement of duodenal fluid radioactivity collected by the string test in patients with neonatal cholestasis. Twenty-six infants with prolonged jaundice and acholic stools were studied prospectively. Twelve patients had neonatal hepatitis, 12 biliary atresia, and one each Alagille syndrome and alpha 1-antitrypsin deficiency liver disease. All infants except the biliary atresia patients and four of the neonatal hepatitis patients revealed bowel activity on scan 6 h after tracer admin…

[Transfer of piperacillin into the body water and gallbladder tissue in cholestasis].

Acute inflammation becomes severe and the risk of perforation of the gallbladder increases every time gall stone colic occurs in patients with cholelithiasis. In the surgical treatment for these patients, interval operation is said to be preferable to emergency operation in the acute stage. Furthermore, the flow of bile into gallbladder is blocked by incarceration of stones at the neck of the gallbladder and other causes in the acute stage. In this case, therefore, how an antibacterial agent in blood moves to the gallbladder wall is important in suppressing infections of the gallbladder wall.

[Transhepatic percutaneous cholangiography with the Chiba needle in diagnosis of cholestasis].

A technique originally described by Okuda has been employed to study 78 patients suffering from cholestatic syndrome. Visualization of the biliary system was obtained in 70 (89.7%). Of these, 65 presented dilatation of the bile ways whereas in 5, the biliary tree was normal. The 8 cases in which no visualization was obtained were later examined with other techniques and 6 proved to be suffering from intrahepatic cholestasis. In patients with extrahepatic obstruction, the biliary system was visualized in 97%, while in intrahepatic cholestasis, the percentage was 45.5%. The main indication for P…

Acute cholestasis, hepatic failure, and fatty liver in the alcoholic.

Three alcoholic patients are described who presented with acute cholestasis and liver cell failure. In each patient the liver biopsy showed severe fatty change with cholestasis, but without typical alcoholic hepatitis. There was no evidence of extra-hepatic biliary obstruction, although one patient had chronic pancreatitis. Two of the three patients died of hepatic failure, but the third recovered and has remained well while abstaining from alcohol.

[Relative increase and metacritic aggravation in the diagnosis of anicteric cholestasis].

The authors report 3 cases and report the diagnostic usefulness of two signs of minor cholestasis described by one of them in 1966. A relative increase, in the absence of obvious virus hepatitis or cirrhosis, of the serum bilirubin, cholesterol, lipids and alkaline phosphatase, together with B.S.P. excretion. suggest minor cholestasis. The sign of "metacritical aggravation" when there is some suspicion of minor cholestasis, the supervision of the course of the disease, or a retrospective inquiry, permit, in the presence of minor symptoms, such as, pain, fever, jaundice, or pruritus, one to mak…

Study on the mechanism of DLK1 in placenta of intrahepatic cholestasis of pregnancy.

BACKGROUND: Intrahepatic cholestasis of pregnancy (ICP) is a pregnancy-specific disorder, prominently characterized by elevated serum bile acids and adverse fetal outcomes. This study focused on the expression of Delta-like 1(DLK1) in the placenta of patients with ICP,along with the alterations in placental blood vessels and function. The bile acid - induced ICP cell model was utilized to clarify the impact of DLK1 on the function of trophoblast cells under the pathological context of ICP. METHODS: Human placental tissues were subjected to immunohistochemical staining, immunofluorescence, Wes…

Fetomaternal Outcome in Obstetric Cholestasis in a Tertiary Care Hospital.

INTRODUCTION: Intrahepatic cholestasis is a potentially serious condition that can develop during the late half of pregnancy. It can increase the risk of preterm birth and stillbirth in fetus and postpartum hemorrhage in mothers. The main symptoms are severe pruritus. Since this condition is hormone-induced, it is a reversible condition and likely to recur in subsequent pregnancy. Early diagnosis, adequate treatment, and well-timed delivery are imperative for better maternal and fetal outcomes. MATERIALS AND METHODS: This is a prospective observational study done in a tertiary care center, RI…

Navigating the Intrahepatic Cholestasis of Pregnancy: An Autobiographical Case Report.

Intrahepatic cholestasis of pregnancy (ICP) is a liver condition commonly occurring during pregnancy, with an unclear etiology. This condition not only causes significant discomfort for the mother due to severe itching but also poses serious risks to the fetus. Effective and timely management of ICP, including diagnosis, consistent monitoring, and treatment, is crucial to mitigate maternal discomfort and prevent fetal complications. The challenges in managing ICP include the absence of clear initial diagnostic criteria, delays in lab results, evolving treatment guidelines, and the financial bu…

Recurrent Intrahepatic Cholestasis of Pregnancy with History of Fetal Demise.

Intrahepatic cholestasis of pregnancy (ICP) is a rare, potentially fatal liver disorder that occurs in late pregnancy. It is characterized by pruritus, jaundice, and elevated liver enzymes, with spontaneous relief of signs and symptoms after birth. Early detection and treatment are required due to the potential risk of fetal complications, including fetal death. A 30-year-old woman with deeply icteric sclera and skin scratch marks presented to a gastroenterology-hepatology referral clinic. Her liver enzymes, bilirubin levels, and serum bile acid levels were all abnormally high. She also disclo…

Hepatitis-E-induced cholestasis in a child: a case report.

UNLABELLED: Hepatitis E virus (HEV) is a major cause of epidemic water-borne hepatitis in tropical and subtropical countries in areas with poor sanitary conditions. The infection by HEV in children is frequent but typically asymptomatic or manifested only as a very mild disease without jaundice, and usually, it often goes undiagnosed. In this case report, the authors describe a case of hepatitis-E-induced cholestasis in a child. CASE PRESENTATION: An 8-year-old male child presented to a Pediatrics emergency with fever and features of acute cholestasis. On abdominal examination, there was non-…

Maternal and Perinatal Outcome in Pregnancy Complicated by Intrahepatic Cholestasis.

Introduction Intrahepatic cholestasis of pregnancy (IHCP) is characterized by pruritus of the hand and sole with abnormal liver function test and bile acid metabolism. IHCP occurs in the second and third trimesters of pregnancy and usually resolves after delivery. The overall prevalence is about 1.2 to 1.5%. This study was conducted to assess the fetomaternal outcome according to maternal serum bile acids levels and its correlation with liver function tests in patients with IHCP. Material and methods This ambispective observational study was conducted in the department of Obstetrics and Gyneco…

Navigating Uncertainty: A Case Study of Intrahepatic Cholestasis of Pregnancy.

Intrahepatic cholestasis of pregnancy (ICP), the most common liver disorder of pregnancy, is associated with complications for both a pregnant person and their fetus. The underlying cause is not well understood. The pruritus associated with ICP is uncomfortable for pregnant people; however, the primary concern is the fetal risk. Fetal risks include preterm labor and birth and intrauterine fetal demise. This is particularly significant for certain populations because of the disparities in incidence of ICP; in the United States, it disproportionately affects Latinx people, the largest and fastes…
