Carcinomas and malignant papillomas of choroid plexus occur more frequently in children and are mostly localized in lateral ventricles. Their clinical picture corresponds to that of plexus papillomas. The histological diagnosis is based upon unmistakeable criteria of malignancy in a tumour deriving from choroid plexus. Five such cases have been studied; in three of them the increase of malignancy was proved by the investigation of recurrences. One case was investigated electronmicroscopically.

A large tumor of the left lateral ventricle in a 3 1/2 year old male was diagnostic of malignant choroid plexus papilloma (choroid plexus carcinoma) as observed histologically. Focal neoplastic epithelial cells contained yellow-brown pigment which was not entirely compatible with melanin by histochemical techniques. Four cases of choroid plexus papilloma (CPP) obtained at the time of surgical excision were examined by light and electron microscopy and compared with normal choroid plexus (CP) of mouse and chick. The diagnosis of hyperornithinemia and gyrate atrophy (HOGA) depends upon the presence of five characteristic features: (1) typical chorioretinal lesions, (2) high myopia, (3) cataracts, (4) hyperornithinemia, and (5) autosomal recessive inheritance. We have seen three patients and described four new findings: (1) decreased whole blood glutamic acid, (2) low normal intelligence, (3) hepatic mitochondrial changes, and (4) urinary excretion of ornithine methyl ester. Investigations of amino acid metabolism in vivo are consistent with the presence of a defect in ornithine keto-acid transaminase.

Utilizing a ventricular perfusion technique, the rate of CSF formation was determined in a 2-year-old child before and after removal of a 74 g choroid plexus papilloma from the left lateral ventricle. Preoperatively, the CSF formation rate was 1.05 +/- SD 0.01 ml/min (1,656 ml/day). Postoperatively, the CSF formation rate was reduced fivefold to 0.20 +/- SD 0.01 ml/min (288 ml/day). Following intravenous injection, cytochrome c traverses the capillary endothelium of the rat choroid plexus and permeates the perivascular space and the extracellular space between epithelial cells.
