Ipsilateral fractures of the scapular neck and clavicle disrupt the suspensory mechanism of the shoulder, resulting in functional loss and deformity. Fifteen patients who had such fractures were managed with open reduction and internal fixation and were followed for an average of twenty-five months (range, fourteen to forty-seven months). All but one patient had a good or excellent functional result according to the scoring system of Rowe. All fractures healed at an average of eight weeks postoperatively. After a short presentation of the treatment methods of clavicle shaft fracture and the atypical use of Zespol stabilization in other than union of long bones shaft operation, the treated patient with ++multi-fragment clavicle shaft fracture with indications for operative reposition and fragments osteosynthesis has been presented. The Zespol plate and 4 screws were used above the skin as an external stabilizer for immobilization of the fragments. A good result was achieved: bone union and full mobility of the joints.

The complications due to migration of a Kirschner wire from the clavicle are described. The major complication of a vascular and pulmonary kind were found in five cases. Migration of the wire into major vessels (aorta, subclavian artery) were seen twice and in three cases, the wire entered the lung. Removal of the wire from the aorta had a fatal result. INTRODUCTION: Chronic Recurrent Multifocal Osteomyelitis (CRMO) represents a chronic inflammatory bone condition which does not result from infectious pathogens. The non-bacterial osteomyelitis spectrum includes this condition, which affects children and teenagers through recurrent multifocal bone lesions. Medial end clavicle fractures are rare injuries and typically treated conservatively. In contrast, displaced fractures have a higher incidence of delayed healing or non-union and might benefit from operative treatment. Thus, assessments of the stability and fracture morphology are essential for selection of the optimal treatment procedure. This article describes the diagnostics and surgical treatment of a displaced extra-articular fracture of the clavicle with a small medial fragment using an anatomically preformed radius plate.
