Our experience with 80 cases of SARS-CoV-2-Clostridioides difficile co-infection: An observational study.

Beside the changes in the gut microbiota in context of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, the increased use of high-risk broad-spectrum antibiotics during the actual pandemic raises concerns about a possible increase of Clostridioides difficile infections (CDIs). We retrospectively analyzed 80 consecutive patients, with SARS-CoV-2 pneumonia and CDI. The mean length of hospitalization was 19.63 days. The mean time of the onset of the digestive symptoms related to CDI was 5.16 days. Patients with an onset of the digestive symptoms from hospital admission have…

Association between Socioeconomic Status and Incidence of Community-Associated Clostridioides difficile Infection - United States, 2014-2015.

We evaluated the association between socioeconomic status (SES) and community-associated Clostridioides difficile infection (CA-CDI) incidence across 2474 census tracts in 10 states. Highly correlated community-level SES variables were transformed into distinct factors using factor analysis. We found low SES communities were associated with higher CA-CDI incidence.

Pre-Trained Deep Convolutional Neural Network for Clostridioides Difficile Bacteria Cytotoxicity Classification Based on Fluorescence Images.

Clostridioides difficile infection (CDI) is an enteric bacterial disease that is increasing in incidence worldwide. Symptoms of CDI range from mild diarrhea to severe life-threatening inflammation of the colon. While antibiotics are standard-of-care treatments for CDI, they are also the biggest risk factor for development of CDI and recurrence. Therefore, novel therapies that successfully treat CDI and protect against recurrence are an unmet clinical need. Screening for novel drug leads is often tested by manual image analysis. The process is slow, tedious and is subject to human error and bia…

The Patients' Voice in Clostridioides difficile Infection: Large Language Model-Assisted Thematic Analysis of Patient Testimonials.

BACKGROUND: Clostridioides difficile infection (CDI) imposes a burden that extends well beyond the gastrointestinal tract, yet existing outcome measures only partially capture the patient experience. We used frontier large language models (LLMs) on patient and caregiver narratives at scale to describe how burden shifts with disease course. METHODS: We analyzed 189 testimonials from the Peggy Lillis Foundation corpus, sorted into 4 cohorts with recurrence (r) and fulminant (f) severity as axes (rfCDI, fCDI, rCDI, non-rfCDI). Two independent LLMs coded 8 thematic domains, 4 fulminant flags, 13 …

A novel RNA nucleic acid amplification test more accurately distinguishes active Clostridioides difficile infection from colonization.

BACKGROUND: Clostridioides difficile (C. diff) is a leading cause of hospital-acquired infections with severity ranging from mild diarrhea to fulminant colitis and death. Current antigen tests lack adequate sensitivity and DNA-based nucleic acid amplification tests (DNA-NAATs) exhibit limited specificity for active infection, leading to either underdiagnosis or inappropriate treatment of colonized individuals. Unlike DNA, mRNA is expressed only by metabolically active bacteria and is rapidly hydrolyzed, providing natural advantages to distinguish active infection from colonization. METHODS: I…

Insights on Recurrent and Sequential Clostridioides difficile Infections From Genomic Surveillance in Minnesota, USA, 2019-2021.

BACKGROUND: The frequent temporal recurrence of Clostridioides difficile infection (CDI) may be the result of relapse with the same strain or reinfection with a different strain. We used whole-genome sequencing (WGS) to assess the genetic diversity and molecular evolution of strains that caused recurrent or sequential CDI. METHODS: We analyzed data from active population- and laboratory-based surveillance of CDI in Minnesota, USA. We performed WGS on isolates collected from 306 patients with multiple CDI events during 2019-2021. We identified multi-locus sequence types (MLSTs), nucleotide var…

Carbadox, a veterinary growth promoter exhibits anticlostridial activity through induction of oxidative stress in Clostridioides difficile.

BACKGROUND: Clostridioides difficile is the leading cause of antibiotic-associated diarrhea in hospitalized patients and is classified as an urgent public health threat. Current therapies, while effective, are limited by antibiotic-induced dysbiosis, treatment failure, and high recurrence rates, underscoring the need for novel therapeutics. Carbadox (CRX), an antibacterial growth promoter, has recently been identified as a potential anti-C. difficile inhibitor. This study aimed to comprehensively evaluate the anti-C. difficile activity, mechanism of action, and in vivo efficacy of CRX. METHOD…

Vancomycin-resistant gut commensal Clostridium innocuum emerges as a pathobiont through synergy with toxigenic Clostridioides difficile.

Clostridium innocuum is increasingly detected in patients with Clostridioides difficile infection (CDI), particularly following vancomycin treatment, and is associated with delayed clinical recovery and poor outcomes. Although C. innocuum lacks classical toxins, its lipopolysaccharide-like cytotoxic activity and emerging pathogenic role remain poorly understood. In this study, we conducted co-infection models to explore the interactions between toxigenic C. difficile and C. innocuum in human intestinal epithelial cells. Our results showed that C. innocuum alone exhibited minimal cytotoxicity; …

Fulminant Clostridioides difficile pseudomembranous colitis requiring subtotal colectomy after pancreaticoduodenectomy: A case report and mini-review.

Pseudomembranous colitis due to Clostridioides difficile infection (CDI) is a serious complication of antibiotic therapy and hospitalization that can progress to toxic megacolon, necessitating emergency colectomy. Patients undergoing pancreaticoduodenectomy (PD) are particularly at high risk due to biliary obstruction, recurrent cholangitis, broad-spectrum antibiotic exposure, and prolonged postoperative stays. We report the case of an elderly woman with multiple cardiovascular comorbidities and recent CDI who underwent PD for pancreatic ductal adenocarcinoma (PDAC). Prior to surgery, she had …

Experimental glycopeptide antibiotic EVG7 prevents recurrent Clostridioides difficile infection by sparing members of the Lachnospiraceae family.

Oral vancomycin has a long history as the first-line treatment for Clostridioides difficile infection (CDI), but its use is associated with high relapse rates. Antibiotics that more selectively target C. difficile while sparing protective commensal gut bacteria, have the potential to prevent recurrent CDI (rCDI). Here, we investigate the experimental glycopeptide antibiotic, EVG7, in the context of rCDI. In vitro susceptibility assays reveal that clinical C. difficile isolates are up to 16-times more sensitive to EVG7 (MIC = 0.063-0.25 mg/L) compared to vancomycin (MIC = 0.5-2 mg/L). In a vali…

Clostridioides difficile recovered from hospital patients, livestock and dogs in Nigeria share near-identical genome sequences.

Genomic data on Clostridioides difficile from the African continent are currently lacking, resulting in the region being under-represented in global analyses of C. difficile infection (CDI) epidemiology. For the first time in Nigeria, we utilized whole-genome sequencing and phylogenetic tools to compare C. difficile isolates from diarrhoeic human patients (n=142), livestock (n=38), poultry manure (n=5) and dogs (n=9) in the same geographic area (Makurdi, north-central Nigeria) and relate them to the global C. difficile population. In addition, selected isolates were tested for antimicrobial su…

Combination of mitomycin C and low-dose metronidazole synergistically against Clostridioides difficile infection and recurrence prevention.

Clostridioides difficile is an anaerobic, spore-forming pathogen responsible for illnesses ranging from mild diarrhea to life-threatening colitis. Current treatments rely on antibiotics such as vancomycin and metronidazole (MTZ), but high doses can disrupt gut microbiota, contributing to recurrent infections. Mitomycin C (MMC), a Food and Drug Administration-approved anticancer agent, is known to induce prophage activation in lysogenic bacteria. Given that over 70% of C. difficile strains harbor prophages, we evaluated MMC's potential to enhance antibiotic efficacy against C. difficile infecti…

Inhibition of Clostridioides difficile-Specific DNA Adenine Methyltransferase CamA by Analogs of S‑Adenosyl‑l‑methionine.

Epigenetically targeted therapies, especially those inhibiting S-adenosyl-l-methionine (SAM)-dependent methylations of DNA, mRNA, and histones, have advanced rapidly in cancer treatment. However, these therapies remain underexplored for antibiotic development, despite the growing threat of antimicrobial resistance. Here, we screened a focused library of SAM analogs against the DNA adenine methyltransferase CamA specific to the enteric pathogen Clostridioides difficile. At the same time, we examined six other adenine methyltransferases, including two bacterial DNA methyltransferases and four hu…

Possible link between clostridioides difficile infection and oral mucositis: insights from a large real-world dataset.

BACKGROUND: Clostridioides difficile infection (CDI), the primary cause of antibiotic-associated diarrhea in the U.S., is typically gastrointestinal but may also present with extra-intestinal features such as oral mucositis. While this association has been noted in oncology populations, its occurrence in non-oncology patients remains unexplored. This study evaluates whether adults with CDI face an elevated risk of developing oral mucositis compared to matched controls without CDI. STUDY DESIGN: A retrospective cohort study utilizing the TriNetX real-world dataset examined patients with confir…
