Manipulation and brace fixing for the treatment of congenital clubfoot in newborns and infants.

BACKGROUND: As one of the most common congenital deformities in children, clubfoot has long been a challenge for orthopedic surgeons. This paper describes the experience of our team with manipulation and above-the-knee brace fixation without percutaneous Achilles tenotomy for the treatment of clubfoot in newborns and infants. METHODS: In the orthopedic department of our hospital, 32 infants and newborns (56 feet) with congenital clubfoot underwent manipulation and above-the-knee brace fixation between 2008 and 2012. External rotation brace was used for 1-4 years during the night after deformi…

The Ponseti method of treatment for clubfoot in Brazil: barriers to bracing compliance.

BACKGROUND: Clubfoot is the most common extremity birth defect. It causes the feet of affected individuals to point inward and downward, preventing them from walking normally. Neglected clubfoot causes disabilities that result in a lack of social integration, creating a psychological and financial burden for the family and community. Clubfoot has been effectively treated through the Ponseti method, a treatment utilizing serial casts to correct the deformity followed by use of an abduction brace for approximately 2-4 years. sustained use of the brace is necessary to prevent relapse and ensure a…

The French functional physical therapy method for the treatment of congenital clubfoot.

The French method, also called the functional physical therapy method, is a combination of physiotherapy, splinting and surgery à la carte. The French functional physical therapy method consists of daily manipulations of the newborn's clubfoot by a specialized physical therapist, stimulation of the muscles around the foot and temporary immobilization of the foot with elastic and nonelastic adhesive taping. Physiotherapy is optimized by early triceps surae lengthening. Sequences of plaster can also be used. If conservative treatment is no longer effective, surgery should be considered. Mini-inv…

[Limited operations and Ilizarov technique for correction of congenital clubfoot in adolescents].

OBJECTIVE: To observe the effectiveness of limited operations and Ilizarov techniques for correction of congenital clubfoot (CCF) in adolescents, and to explore the new methods and new ideas for functional reconstruction of CCF. METHODS: Between September 2003 and July 2010, 25 adolescent patients (40 feet) with CCF were treated. Of the 25 patients, 14 were male (20 feet) and 11 were female (20 feet) with an age range from 12 to 25 years (mean, 15.7 years). The left feet were involved in 4 cases, the right feet in 6 cases, and both feet in 15 cases. According to Qin's clubfoot deformity scale…

Long-term results of the posteromedial release in the treatment of idiopathic clubfoot.

BACKGROUND: Short-term follow-up studies show good results in foot function, after surgical treatment of idiopathic clubfeet. Long-term follow-up studies are rare and probably represent a mixture of experience of different treating orthopedic surgeons. The purpose of this study is to present the long-term results of the posteromedial release (PMR) in congenital clubfoot treatment of 1 treating surgeon. MATERIALS AND METHODS: Thirty-eight patients with 58 clubfeet had an a la carte PMRe, performed by 1 experienced pediatric orthopedic surgeon before the age of 2 years. This group had a mean fo…

A review of the Ponseti method and development of an infant clubfoot program in Vietnam.

BACKGROUND: The Feet for Walking clubfoot project from Australia formally introduced the Ponseti technique in Vietnam in 2004 and is based at the Da Nang Orthopedic and Rehabilitation Centre in central Vietnam. METHODS: We provide an initial overview of the management of infant clubfoot deformity using the nonsurgical Ponseti method. RESULTS: Early indicators of the outcome of implementing this clubfoot project are largely positive but also require ongoing review. Further analyses of the use of the Ponseti method (or obstacles preventing the same) following training of personnel is underway.…

Leg muscle atrophy in idiopathic congenital clubfoot: is it primitive or acquired?

PURPOSE: To investigate whether atrophy of the leg muscles present in congenital clubfoot (CCF) is primitive or secondary to treatment of the deformity. METHODS: Magnetic resonance imaging (MRI) of both legs was taken in three cohorts of patients with unilateral congenital clubfoot (UCCF): eight untreated newborns (age range 10 days to 2 weeks); eight children who had been treated with the Ponseti method (age range 2-4 years); eight adults whose deformity had been corrected by manipulation and casting according to Ponseti, followed by a limited posterior release performed at age 2-3 months (a…

Use of the Ponseti method for recurrent clubfoot following posteromedial release.

BACKGROUND: A child with recurrent or incompletely corrected clubfoot after previous extensive soft tissue release is treated frequently with revision surgery. This leads to further scarring, pain and limitations in range of motion. We have utilized the Ponseti method of manipulation and casting and when indicated, tibialis anterior tendon transfer, instead of revision surgery for these cases. MATERIALS AND METHODS: A retrospective review of all children treated since 2002 (n = 11) at our institution for recurrent or incompletely corrected clubfoot after previous extensive soft tissue release…

Early experience with the Ponseti method for the treatment of congenital idiopathic clubfoot.

BACKGROUND: The preferred conservative treatment for congenital idiopathic clubfoot deformity remains a controversial issue. OBJECTIVES: To compare the outcomes of two groups of CICF patients who underwent two different treatment protocols. METHODS: The study cohort included 72 infants treated in our hospital from August 1998 to December 2002. Group 1 comprised 40 infants (61 clubfeet) who were treated by a traditional method (a modification of the Kite and Lovell technique), and group 2 consisted of 32 infants (48 clubfeet) treated with the Ponseti technique. Both groups were similar in age…

Comparison of posteromedial and subtalar release in surgical treatment of resistant clubfoot.

This preliminary study evalluated two surgical techniques for the treatment of resistant clubfoot: the posteromedial and the subtalar release. Two groups of matched patients with idiopathic clubfoot who underwent surgery between 1983 and 1995 were compared. In group 1 (12 patients, 18 feet), posteromedial release was performed. In this group, the results were satisfactory with a tendency to hypocorrection: 4 (22%) feet needed a secondary procedure (average follow-up: 97.8 months). Group 2 (12 patients, 17 feet) underwent complete subtalar release. The results were clinically better for group 2…

Long-term results of the operative treatment of clubfoot: a representative study.

This study reports the long-term results of operative treatment of clubfoot. Of the patients who underwent posteromedial release for congenital clubfoot between 1972 and 1975 at the Medical University in Pécs, Hungary, 41 feet of 30 patients have been under control examination. Control consisted of the distribution of questionnaires, physical examination, and in some cases, radiologic analysis. At an average of 20 years postsurgery, good clinical results were observed in 31 (75%) feet. In these patients, satisfying foot form and good function were achieved, yet the characteristic atrophy of th…

[Strength of leg and foot muscles after surgical treatment for clubfoot].

In 50 congenital clubfeet (in 28 children aged 62-126 months) operated on with subtalar release from dorsolateral and posteromedial approach isometric strength of flexors, extensors, pronators and supinators was assessed with tensometry. All the muscular groups showed decreased momentum if compared to normal foot. The smallest deficit was observed in plantar flexors. Better results were accompanied by greater muscular strength, but statistically significant relation existed between strength of dorsal flexors and the quality of result. Postoperative calcaneus position did not influence the stre

Gait analysis and dynamic foot pressure in the assessment of treated clubfoot.

Gait analysis and dynamic foot pressure were used in a follow-up study of 42 children with idiopathic clubfeet. Mean age at follow-up was 9 years (range 4-15 years). Twenty-four of the 62 clubfeet were treated conservatively and 38 had at least one operative intervention. Gait analysis showed that the vertical and posterior ground forces were significantly lower from the clubfoot compared with the normal foot. Dynamic foot pressure demonstrated a significant shift of the center of pressure to the lateral side in the affected compared with the normal side. In bilateral clubfeet, a relationship …

Cause of toe-in gait after posteromedial release for congenital clubfoot.

Eighteen children with 24 congenital clubfeet were examined for their foot-progression angle and radiographic angles before and after Turco's posteromedial release. The foot-progression angle was -12.9 degrees before surgery and 1.6 degrees after surgery. The ratio of correction of the foot-progression angle was 58%. Using multiple regression analysis, this ratio was mostly related to the ratio of correction of the anteroposterior talo-first metatarsal and lateral talocalcaneal angles. These results suggest that it is important in correction of toe-in gait of congenital clubfoot to correct add…

Histochemistry of abductor hallucis muscle in children with idiopathic clubfoot and in controls.

The histochemical composition of the abductor hallucis (AH) muscle was investigated in 39 children with idiopathic clubfoot (CF), aged 0-11 years, and in 42 controls. In the youngest group of patients (0-2 years) the percentage of type 1 (slow twitch, tonic) fibers was significantly higher than in controls. In older groups, there was no difference between patients and controls. The relative predominance of type 1 fibers could be due to immobilization, passive shortening or stretching, or primary overactivity of this muscle in CF promoted by an unknown neural factor. None of these interpretatio…
