Cochlear implantation without general anaesthesia: a resource-efficient surgical pathway.

OBJECTIVES: This study aimed to compare the environmental and financial impact of cochlear implantation performed with and without general anaesthesia. METHODS: Two cochlear implant procedures performed on the same day by the same surgical and anaesthetic teams were prospectively evaluated: one under total intravenous anaesthesia with bispectral index monitoring, and one under local anaesthesia with propofol sedation. Anaesthetic drugs, consumables, equipment, electricity use and procedural duration were recorded. Financial costs were estimated using procurement data and national formularies.…

Age of cochlear implantation and ASL skill impact literacy development.

After decades of research, many researchers (and the parents, educators, and clinicians who rely on the results of their scientific inquiry) maintain opposing views regarding appropriate strategies for ensuring adequate language and literacy development among young deaf children. These viewpoints range from the exclusive use of cochlear implants (with no visual language) to the exclusive use of a visual language (with no implantation). Recent authors have posited the potential benefits of early sign language for implanted children and have refuted claims that signing will negatively impact the

Cochlear implantation in children with Bartter syndrome: A case report.

Bartter syndrome (BS) is an autosomal recessive disorder characterized by polyhydramnios, premature birth, polyuria, renal salt-wasting, hypokalemic metabolic alkalosis, normal blood pressure with increased levels of renin and aldosterone, and the presence of hearing loss. Mutations in BSND, CLCNKA, and CLCNKB cause the disorder. We present a 3-year-old girl with BS type 4 and sensorineural hearing loss who received cochlear implantation (CI) after failing a hearing screening. The patient had a successful left-side implantation but later developed a flap infection, initially treated with antib…

International Consensus Statements on Intraoperative Testing for Cochlear Implantation Surgery.

OBJECTIVES: A wide variety of intraoperative tests are available in cochlear implantation. However, no consensus exists on which tests constitute the minimum necessary battery. We assembled an international panel of clinical experts to develop, refine, and vote upon a set of core consensus statements. DESIGN: A literature review was used to identify intraoperative tests currently used in the field and draft a set of provisional statements. For statement evaluation and refinement, we used a modified Delphi consensus panel structure. Multiple interactive rounds of voting, evaluation, and feedba…

Leflunomide-Associated Wound Complication After Cochlear Implantation: A Case Report.

INTRODUCTION: Cochlear implantation has become an increasingly common strategy for aural rehabilitation in patients with severe to profound hearing loss who no longer benefit from conventional amplification. In conjunction, immunosuppressive therapies (e.g. disease-modifying anti rheumatic drugs (DMARDs) have become the keystone of management in numerous autoimmune conditions. Given the increasing prevalence of both, a greater proportion of patients will undergo cochlear implantation while on immune-modulating medications. While these medications are usually well tolerated, immunosuppression m…

On the Difficulty Predicting Word Recognition Performance After Cochlear Implantation.

HYPOTHESIS: Preimplantation word scores cannot reliably predict postimplantation outcomes. BACKGROUND: To date, there is no model based on preoperative data that can reliably predict the postoperative outcomes of cochlear implantation in the postlingually deafened adult patient. METHODS: In a group of 228 patients who received a cochlear implant between 2002 and 2021, we tested the predictive power of nine variables (age, etiology, sex, laterality of implantation, preimplantation thresholds and word scores, as well as the design, insertion approach, and angular insertion depth of the electro…

Overcoming developing-world challenges in cochlear implantation: A Nigerian perspective.

The emergence of cochlear implantation (CI) in the mid-20th century was a transformation to the field of restorative otology. The advance in this field has not been felt in lower-income countries where a huge burden of profound hearing loss lies. The authors sought to review the literature on the practice of cochlea implantation in Nigeria. Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) extension for scoping reviews, we conducted a scoping review of the literature on CI in Nigeria. All observational studies with information on cochlear implants and/or…
