Stercoraceous perforation of the sigmoid colon has rarely been reported in the literature. This lesion is assumed to be produced by the pressure from a hard scybalum resulting in a perforated ulcer with necrotic edges. Two cases of stercoraceous perforation of the sigmoid colon are presented in this paper. It is difficult to diagnose this lesion preoperatively, although ultrasonograms proved useful in showing the colon perforation. To improve the quality of life of the patient we used completely detubularized sigmoid colon for bladder reconstruction along with radical cystoprostatectomy in 6 men with invasive bladder cancer. Followup was 8 to 20 months. Postoperatively, all of the patients were continent during the day but only 4 (66.7%) were continent at night, although they had to awaken twice to remain dry. Acute surgical abdomen is a very serious situation that arises in various medical conditions. Common among them are appendicitis, foreign-body ingestion, and intestinal obstruction or perforation. These complications usually require emergency abdominal surgery. A case of rectal bleeding accompanied by acute surgical abdomen following the accidental ingestion of a toothpick is described in this report.

Hartmann resection has been the treatment of choice for perforated diverticulitis of the sigmoid colon at the Vancouver General Hospital for nearly 20 years. A retrospective analysis of 70 patients who had the colonic stomas closed following the Hartmann resection was done. A modified Hartmann resection for the initial procedure has been recommended on the basis of fewer complication and shorter hospital stay when the closure operation is performed. We have recorded the blood supply to a high or subsplenic sigmoid loop from branches of the left colic and first sigmoid arteries. An accessory middle colic present in this case crossed the region immediately superior to the apex of the sigmoid loop. In this instance it arose from the dorsal pancreatic which also gave origin to inferior pancreaticoduodenal branches. Since the high sigmoid may be retroperitoneal, as in the case described, it would not be readily apparent; it would require lateral views to separate it from the descending colon radiographically.

Sigmoid volvulus demonstrates geographical, racial, and gender variation. This autopsy study was undertaken to establish morphological differences of the sigmoid colon and its mesocolon in which the length and other characteristics were assessed. A total of 590 cadavers were examined (403 African, 91 Indian, and 96 White). The patient is a 52-year-old woman who visited the general practitioner because of positive fecal occult blood test by medical examination.
