Identifying risk factors for renal failure and myocardial infarction following colorectal surgery.

BACKGROUND: The development of acute renal failure and myocardial infarction (MI) following colectomy prolongs recovery and is associated with worse outcomes. The purpose of this study is to identify perioperative factors that predispose patients to an adverse cardiac or renal complication. MATERIALS AND METHODS: We conducted a retrospective review of colectomies from 2001 to 2009. Patients were evaluated based upon the electronic inpatient record and followed to determine the incidence of acute renal failure (creatinine elevation over 50% of baseline) and myocardial injury. RESULTS: A total

Scintigraphic pattern of planar gallium scan in wound infection after colorectal surgery.

BACKGROUND: The incidence of postoperative wound infection is higher after colorectal surgery than after other surgical procedures. No studies have evaluated the usefulness of gallium 67 scintigraphy for detecting wound infection after colorectal surgery. We investigated the clinical potential of gallium imaging for detecting wound infection after colorectal surgery. METHODS: Fifty patients who had fever after colorectal surgery were enrolled in this study. All patients had open surgical incisions at the midline of the abdomen. Planar gallium scintigraphy was performed 24 h, and then as neede…

Intra-abdominal recurrence of colorectal cancer detected by radioimmunoguided surgery (RIGS system).

Since 1986, 32 patients with metastatic colorectal cancer have undergone second-look radioimmunoguided surgery (RIGS system). The primary tumor was located in the right and transverse colon in 11 patients, left and sigmoid colon in 16, and rectum in five. The carcinoembryonic antigen level was elevated in 30 patients (94%); all patients underwent a computed tomographic scan of the abdomen and pelvis. The overall sensitivity of the computed tomographic scan was 41% (abdomen other than liver, 27%; liver, 58%; and pelvis, 22%). The RIGS system identified recurrent tumor in 81% of the patients. Th

Low-dose aspirin can reduce colorectal cancer mortality after surgery: A 10-year follow-up of 13 528 colorectal cancer patients.

BACKGROUND AND AIM: The chemopreventive effect of aspirin in colorectal cancer (CRC) is well studied, but its benefit in patients after CRC diagnosis and surgery is unclear. This study aims to investigate the effects of low-dose aspirin use in mortality among CRC patients after surgery. METHODS: Patients were analyzed in two cohorts: (i) patients taking aspirin before CRC diagnosis and continued or discontinued aspirin after surgery and (ii) patients, who never used aspirin before CRC diagnosis, received or did not receive aspirin after surgery. SAGES colorectal and metabolic bariatric surgery committee joint task force call to action for synchronized severe obesity and colorectal cancer management.

BACKGROUND: Severe obesity (body mass index ≥ 40 kg/m2 or ≥ 35 kg/m2 with obesity-related comorbidities) is increasingly prevalent and independently associated with elevated perioperative morbidity and inferior oncologic outcomes in patients with colorectal cancer (CRC). Despite these risks, intentional preoperative weight optimization is not routinely incorporated into CRC management, owing to concerns regarding treatment delay, absence of guideline endorsement, and limited supporting evidence. METHODS: A literature review was conducted using PubMed and Embase to evaluate the impact of sever

Sequential compression devices as prophylaxis for venous thromboembolism in high-risk colorectal surgery patients: reconsidering American Society of Colorectal Surgeons parameters.

The American Society of Colorectal Surgeons (ASCRS) recently endorsed low-molecular-weight heparin and low-dose heparin as primary prophylaxis for venous thromboembolism (VTE) in highest-risk patients. Our study evaluates the feasibility of sequential compression device (SCD) use for VTE prophylaxis in these patients. Computerized databases of discharge diagnoses from three hospitals were reviewed. All patients with colorectal cancer or inflammatory bowel disease during a 7-year period were identified. Those who underwent major abdominal surgery and received VTE prophylaxis exclusively with SC

Indirect Evidence for the Volume-Outcome Relationship in Corrective Surgery for Hirschsprung Disease: Insights from Adult Colorectal Surgery.

Hirschsprung disease (HD) is a rare malformation that requires complex corrective surgery. Low caseload, heterogeneous patient cohorts, and … study provides strong support for the transferability of the volume-outcome relationship from adult colorectal surgery to corrective surgery for HD. Despite population differences, the underlying mechanisms linking surgical complexity and institutional experience are comparable, reinforcing the relevance of volume-based quality control in pediatric surgery.

Current roles of colonoscopy in minimally invasive colorectal surgery: Preoperative guidance, intraoperative colonoscopy, and combined endoscopic-laparoscopic surgery.

Colonoscopy is a cornerstone in the detection and diagnosis of colorectal tumors, playing a critical role in both screening and clinical evaluation. More recently, its utility has expanded to therapeutic guidance, particularly with the advent of minimally invasive surgical techniques. Preoperative tattoo marking is commonly used for tumor localization; however, it poses challenges such as intraperitoneal ink scattering and difficulty in defining dissection planes in the lower rectum. To address these limitations, a new technology utilizing a near-infrared fluorescence clip placed preoperativel

Impact of structured multicentre enhanced recovery after surgery (ERAS) protocol implementation on length of stay after colorectal surgery.

BACKGROUND: Increased length of stay after surgery is associated with increased healthcare utilization and adverse patient outcomes. While enhanced recovery after surgery (ERAS) protocols have been shown to reduce length of stay after colorectal surgery in trial settings, their effectiveness in real-world settings is more uncertain. The aim of this study was to assess the impact of ERAS protocol implementation on length of stay after colorectal surgery, using real-world data. METHODS: In 2012, ERAS protocols were introduced at 15 Ontario hospitals as part of the iERAS study. A cohort of patie…

Enhanced recovery after surgery reduces length of stay after colorectal surgery in a small rural hospital in Ontario.

INTRODUCTION: Enhanced recovery after surgery (ERAS) programmes include pre-operative, intraoperative and post-operative clinical pathways to improve quality of patient care while reducing length of stay (LOS) and readmission. This study assessed the feasibility and outcomes of an ERAS protocol for colorectal surgery implemented over 2 years in a small, resource-challenged rural hospital. METHODS: A prospective cohort study used retrospectively matched controls to assess the effect of ERAS on LOS in patients undergoing colorectal surgery in a small rural hospital in northern Ontario, Canada. …

Studying Enhanced Recovery After Surgery (ERAS®) Core Items in Colorectal Surgery: A Causal Model with Latent Variables.

BACKGROUND: Previous Enhanced Recovery After Surgery (ERAS®) studies have not always taken into account that ERAS interventions depend on baseline covariates and that several confounding variables affect the composite outcomes. METHOD: A causal latent variable model is proposed to analyze data obtained prospectively concerning 1261 patients undergoing elective colorectal surgery within the ERAS protocol. Primary outcomes (composite of any complication, surgical site infection, medical complications, early ready for discharge (TRD), early actual discharge) and secondary outcomes (composite of …

Patient experiences of perioperative nutrition within an Enhanced Recovery After Surgery programme for colorectal surgery: a qualitative study.

AIM: Nutrition is an important element of the Enhanced Recovery After Surgery (ERAS) programme. Patients have previously indicated that nutrition is a key component of ERAS that requires improvement. Our aim was to explore the perioperative nutrition experiences of colorectal surgical patients to identify barriers and facilitators to the integration of nutrition within ERAS. METHOD: Sixteen individuals undergoing colorectal surgery participated in a semi-structured interview between postoperative day three and hospital discharge. The topic guide was developed iteratively throughout the study;…

Optimizing Postoperative Recovery in Colorectal Surgery: A Systematic Review on the Efficacy of Enhanced Recovery After Surgery (ERAS) Protocols.

Colorectal surgery carries substantial risks of postoperative morbidity, extended hospital stays, and increased healthcare costs. While the overall benefits of enhanced recovery after surgery (ERAS) protocols are well established, recent studies have expanded their scope to include immunological outcomes, telemedicine integration, and patient-centered recovery metrics. This systematic review synthesizes the latest evidence from randomized controlled trials (RCTs) evaluating ERAS protocols in colorectal surgery, with particular attention to these emerging dimensions. A comprehensive search of P…
