Care Seeking and Treatment of Febrile Children with and without Danger Signs of Severe Disease in Northern Uganda: Results from Three Household Surveys (2018-2020).

Identification, stabilization, and prompt referral of children with signs of severe febrile disease (danger signs) in rural communities are … care seeking in the public sector was 26% and similar to care seeking in the private sector (24%). Community health workers were used as the first source of care by 12% of the children. Approximately 38% of the children who were reported to have danger signs of severe disease requiring prompt referral and treatment did not seek care from a health provider. Understanding …

The role of community co-designed interventions for malaria prevention and control in conflict-affected communities in Cameroon's Southwest and littoral regions.

BACKGROUND: In 2022, Cameroon was ranked the 11th of the 11 high-burden countries responsible for 70% of the global malaria burden largely due to sustained conflict in the Southwest and Northwest regions since 2018. From May 2021 to April 2024, a mixed-methods study was conducted of three community-based, co-created interventions, a community dialogue approach called the Community Health Participatory Approach (CoHPA), community health workers (CHWs) supportive supervision, and health vouchers for treatment and transportation. The study involved formative, intervention and endline phases; the …

Integrated surveys as a tool for early case detection in leprosy control programme.

A project for total leprosy case detection particularly early leprosy cases was undertaken in Wardha District which has a rural population of about 6 lacs in 905 villages, through an integrated survey with the help of all health workers like Leprosy Technicians, Sanitary Inspectors, Co-ordinators, Malaria Workers, Smallpox Vaccinators and Auxillary Nurse Midwives. The training given to the Non Leprosy Health Worker was for a period of 3 days making them just fit to suspect all leprosy cases. The final diagnosis was to be made by the fully trained Leprosy Technicians. The surveys were conducted

Cross-sectional dataset of low- and semi-skilled gig workers in India: COVID-19 and human security.

The paper presents a cross-sectional dataset of low- and semi-skilled gig workers in India, collected during a single survey wave in the pandemic. Respondents retrospectively reported their experiences at two time points pre-pandemic (July-November 2019) and during the pandemic (December 2020-January 2021) across multiple human security dimensions, framed within the United Nations Human Security Framework (2016). The dataset represents original primary data and captures the economic, food, health, environmental, personal, community, and political experiences of gig workers during the crisis. T

Who could be One Health Activist at the community level?: A case for India.

BACKGROUND: Community health workers (CHWs) are the mainstay of the public health system, serving for decades in low-resource countries. Their multi-dimensional work in various health care services, including the prevention of communicable diseases and health promotion of non-communicable diseases, makes CHWs, the frontline workers in their respective communities in India. As India is heading towards the development of One Health (OH), this study attempted to provide an insight into potential OH activists (OHA) at the community level. Thus, this case study in one of India's western cities, Ahm…

Expert views of peer-based interventions for prisoner health.

PURPOSE: Formalised support services for prisoners that rely on peer methods of delivery show promising health and social outcomes but there is also conjecture that negative effects, both at an individual and organisational level, can occur. The paper aims to discuss these issues. DESIGN/METHODOLOGY/APPROACH: Individuals with recognised professional expertise from various sectors (including ex-prisoners) were invited to contribute to an expert symposium to share their perceptions of the positive and negative effects of peer interventions in prison. Discussions and debate were audio recorded w

The community-directed, ivermectin-treatment programme for onchocerciasis control in Uganda--an evaluative study (1993-1997).

The first 5 years of a community-directed, ivermectin-treatment programme, to control onchocerciasis in 1805 endemic communities in 10 districts in Uganda, are evaluated. Each year, the desired treatment coverage of the population eligible to take invermectin (90%) was achieved in 42.6%-51% of the 1713 communities for which complete data were available; 67%-74.8% achieved 80% coverage. The annual cost per person treated with ivermectin (ACPTI) was much higher in the districts with small populations to be treated (< 15,000) than in those with large populations (> 40,000) (U.S.$0.40 v. U.S.$0.10

Essential work, unequal places: Neighborhood characteristics of the essential workforce in the United States during the COVID-19 pandemic.

INTRODUCTION: At the onset of the COVID-19 pandemic, more than 55 million U.S. workers continued providing "essential" services crucial to public health, safety, and economic stability, even as large segments of the workforce sheltered at home. Despite the central role these workers played in safeguarding the economy and the greater American public, little is known about the neighborhood environments in which essential workers lived and how well these communities were equipped to support the health and well-being of essential workers during the pandemic. Therefore, the primary objective of thi

Community-based intermittent mass testing and treatment for malaria in an area of high transmission intensity, western Kenya: development of study site infrastructure and lessons learned.

BACKGROUND: Malaria transmission is high in western Kenya and the asymptomatic infected population plays a significant role in driving the transmission. Mathematical modelling and simulation programs suggest that interventions targeting asymptomatic infections through mass testing and treatment (MTaT) or mass drug administration (MDA) have the potential to reduce malaria transmission when combined with existing interventions. OBJECTIVE: This paper describes the study site, capacity development efforts required, and lessons learned for implementing a multi-year community-based cluster-randomiz

Diagnosed Chronic Health Conditions Among Injured Workers With Permanent Impairments and the General Population.

OBJECTIVE: To profile chronic health conditions of an injured worker sample before and after workplace injury and compare injured workers to a matched community sample. METHODS: Logistic regression analyses compared risk of certain chronic health conditions for permanently disabled injured workers in the pre- and post-injury periods to comparator subsamples from the Canadian Community Health Surveys 2003 and 2009/2010. RESULTS: There were notable health differences between the injured worker and comparator samples for the post-injury period. Injured men and women were more likely to report a

It depends on how you tell: a qualitative diagnostic analysis of the implementation climate for community-wide mass drug administration for soil-transmitted helminth.

OBJECTIVES: Current soil-transmitted helminth (STH) morbidity control guidelines primarily target deworming of preschool and school-age children. Emerging evidence suggests that community-wide mass drug administration (cMDA) may interrupt STH transmission. However, the success of such programmes depends on achieving high treatment coverage and uptake. This formative analysis was conducted to evaluate the implementation climate for cMDA and to determine barriers and facilitators to launch. SETTINGS: Prior to the launch of a cMDA trial in Benin, India and Malawi. PARTICIPANTS: Community member

Cost of adapting the International Guide for Monitoring Child Development, Guatemala and India.

OBJECTIVE: To assess the cost of co-creating an adaptation of the International Guide for Monitoring Child Development (intervention for use by community health workers (CHWs)) in rural Guatemala and India. METHODS: We developed survey instruments to capture the costs, from health-system and societal perspectives, of adapting the guide with local partners and international experts in the two sites. We included the costs of human resources, information technology, and infrastructure and logistic support. To improve the transparency and comparability of cost estimates, we reported the required …

The community drug distributor passport: a case study of training and recognition in Chad.

INTRODUCTION: Community health workers (CHWs), particularly volunteer community drug distributors (CDDs), play essential roles in advancing universal health coverage (UHC) and neglected tropical disease (NTD) control, yet their responsibilities and formal recognition remain poorly defined across health systems. This creates policy-implementation disconnects, especially during mass drug administration (MDA) campaigns for schistosomiasis and soil-transmitted helminthiases, where CDDs deliver critical services amid declining motivation and weak community awareness. METHODS: From 15 September 202…

Effectiveness of a Multicomponent Strategy for Implementing Guidelines for Treating Tobacco Use in Vietnam Commune Health Centers.

INTRODUCTION: Strategies are needed to increase implementation of evidence-based tobacco dependence treatment (TDT) in health care systems in low-and middle-income countries (LMICs). AIMS AND METHODS: We conducted a two-arm cluster randomized controlled trial to compare the effectiveness of two strategies for implementing TDT guidelines in community health centers (n = 26) in Vietnam. Arm 1 included training and a tool kit (eg, reminder system) to promote and support delivery of the 4As (Ask about tobacco use, Advise to quit, Assess readiness, Assist with brief counseling) (Arm 1).
