Effect of allergic conjunctival inflammation on the allogeneic response to donor cornea.

PURPOSE: Immunologic rejection is the most common cause of corneal allograft rejection. Ipsilateral ocular inflammation has been identified as a predictor of future corneal graft failure. This study investigates the effect of perioperative allergic conjunctivitis on corneal allograft survival. METHODS: C57BL6 donor corneas were transplanted into naive A/J mice, A/J mice sensitized to short ragweed (SRW) pollen by intraperitoneal injection and then challenged with topical SRW to induce allergic conjunctivitis (Sens(+)Chall(+)), and A/J mice sensitized to SRW and challenged with topical PBS (Se

Causal relationships between allergic diseases and significant declines in lung function: a multivariable Mendelian randomization study.

BACKGROUND: Asthma and other allergic diseases are increasing globally each year, with some patients experiencing the co-occurrence of two or more conditions, significantly impacting their quality of life. While these diseases may share certain immune mechanisms, the independent causal relationships between them and lung function remain unclear. OBJECTIVE: This study aims to investigate the independent and interactive effects of allergic asthma, atopic dermatitis, allergic rhinitis, and allergic conjunctivitis on significant declines in lung function. By elucidating the potential causal relat

The pathophysiology of ocular allergy: current thinking.

Seasonal allergic conjunctivitis is the only ocular disease to involve solely Type-1 hypersensitivity, the other main forms of ocular allergy--perennial allergic conjunctivitis, vernal and atopic keratoconjunctivitis and giant papillary conjunctivitis--each having a more complex immunological basis and a chronic inflammatory component. Involvement of secondary inflammatory cells, particularly eosinophils, in addition to the mast cells resident in the conjunctival substantia propria, can lead to more serious corneal damage with vision-threatening potential. Thoughtful management of allergic con

Classification of ocular atopic/allergic disorders and conditions: an unsolved problem.

PURPOSE: To review the different classifications of ocular allergic diseases and to define the most suitable one according to the current understanding of the pathophysiology and clinical course of these disorders. METHODS: Review of the major published ocular allergy classifications. RESULTS: Based on past and present classifications, the author proposes the following classification: 1. allergic conjunctivitis (acute and chronic); 2. giant papillary conjunctivitis; 3. vernal keratoconjunctivitis; and 4. atopic keratoconjunctivitis. In addition, use of the term "allergic conjunctivitis" as a

Evaluation methods using tear volume in a conjunctivitis mice model.

Allergic conjunctival disease is an immune-mediated inflammatory disease of the conjunctiva. To develop clinically useful drugs, it is necessary to develop quantitative evaluation methods that reflect the clinical symptoms in experimental animal models. Allergic conjunctivitis model mice were systemically sensitised with ovalbumin (OVA) administered intraperitoneally and locally sensitised with OVA eye drops between day 14-28. Next, conjunctivitis induced by ocular administration of OVA solution to sensitised mice was evaluated based on tear volume. Additionally, we evaluated increase in tear

The usefulness of measuring tear periostin for the diagnosis and management of ocular allergic diseases.

BACKGROUND: Chronic ocular allergic diseases such as vernal keratoconjunctivitis (VKC) and atopic keratoconjunctivitis (AKC) are accompanied by serious comorbidities; however, the underlying pathogenesis remains obscure. Furthermore, diagnosing conjunctival lesions in patients with atopic dermatitis and estimating the severity in AKC are important for the treatment of ocular allergic diseases. OBJECTIVE: We addressed whether periostin, a novel mediator and biomarker in allergic inflammation, is involved in the pathogenesis of ocular allergic diseases and whether periostin can be a biomarker f

Comparison of Meibomian Gland Loss Between Pediatric Patients with Blepharo-Keratoconjunctivitis and Age-Matched Controls Using Non-Contact Meibography.

Objectives: To compare the extent of meibomian gland loss in pediatric patients with blepharo-keratoconjunctivitis (BKC), allergic conjunctivitis, and healthy controls using non-contact infrared meibography. Methods: This retrospective observational study included 81 patients aged 18 years or younger: 20 with BKC, 29 with allergic conjunctivitis, and 32 healthy controls. Meibomian gland loss in the upper and lower eyelids was quantified on non-contact infrared meibography images. Right and left eyes were analyzed separately, and only eyes with the same laterality were compared among groups. Re

Updates in the treatment of ocular allergies.

Allergic diseases have greatly increased in industrialized countries. About 30% of people suffer from allergic symptoms and 40%-80% of them have symptoms in the eyes. Atopic conjunctivitis can be divided into seasonal allergic conjunctivitis (SAC) and perennial allergic conjunctivitis (PAC). The treatment of SAC is simple; antihistamines, anti-inflammatory agents, or chromoglycate. In severe cases of SAC, subcutaneous or sublingual immunotherapy is helpful. PAC needs longer therapy, often year round, with mast cell stabilizers, antihistamines, and sometimes local steroids. Atopic keratoconjunc

Pediatric Conjunctivitis: A Review of Clinical Manifestations, Diagnosis, and Management.

Conjunctivitis is a common pediatric problem and is broadly divided into infectious and non-infectious etiologies. Bacterial conjunctivitis makes up the majority of cases in children and often presents with purulent discharge and mattering of the eyelids. Treatment is supportive with an individual approach to antibiotic use in uncomplicated cases since it may shorten symptom duration, but is not without risks. Viral conjunctivitis is the other infectious cause and is primarily caused by adenovirus, with a burning, gritty feeling and watery discharge. Treatment is supportive. Allergic conjuncti

Ocular allergic disease.

Ocular allergy is a common condition that usually affects the conjunctiva of the eye and is therefore often referred to as allergic conjunctivitis. The severity of the disease can range from mild itching and redness, as seen in seasonal allergic conjunctivitis, to the more serious vision threatening forms of ocular allergy which affect the cornea, such as atopic keratoconjunctivitis. The pathogenesis of allergic conjunctivitis involves a complex mechanism which centers around IgE-mediated mast cell degranulation and release of multiple preformed and newly formed inflammatory mediators. The dia

Atopic conjunctivitis. A cytologic examination.

Tarsal conjunctival scrapings of the lower and upper eyelids of 21 atopic, pollen allergic patients with conjunctivitis were examined three times; during a symptom-free period, during a challenge test and during the pollen season. Eosinophils were also counted from the conjunctival fluid of 17 patients, but only of 12 patients during the challenge. The controls were 10 atopic patients without conjunctivitis (K0), 31 normal persons (K1), who were examined routinely in the course of a spectacle prescription and 45 patients with acute conjunctivitis (K2) of other than allergic origin. A statistic

Hyposensitizing therapy with standard antigenic extracts: an important source of thimerosal sensitization.

The frequency of thimerosal sensitization was determined in 5 groups of subjects: military recruits; healthy subjects; patients with asthma or rhinitis undergoing hyposensitizing therapy with thimerosal-preserved antigenic extracts; patients with allergic contact dermatitis; patients with allergic contact conjunctivitis. Our patients with allergic contact dermatitis have a higher incidence of positive patch tests to thimerosal than healthy subjects. The source of thimerosal sensitization in this group remains obscure, their history of exposure to this or to other mercury derivatives being comp

Immunomodulators for conjunctivitis.

PURPOSE OF REVIEW: Allergic conjunctivitis is an IgE-mediated inflammatory of conjunctiva. Despite the fact that histamine is a major mediator in this condition, use of topical antihistamines often gives only temporarily relief. Therapeutic agents aiming at inflammatory cascades of the disease are desirable particularly in cases with prolonged and severe allergic conjunctivitis. The article reviews these new treatment armamentariums with regard to their mechanism of actions and clinical efficacy. RECENT FINDINGS: Some second generation of antihistamines demonstrated anti-inflammatory properti

[Allergic and immunopathological diseases of the ocular surface].

Allergic and immunopathological diseases of the ocular surface are inflammations that can occur with mild to severe symptoms that cause visual impairment. Allergic inflammations mainly affect the conjunctiva, causing acute and/or chronic conjunctivitis. Several forms are distinguished: seasonal allergic conjunctivitis, vernal conjunctivitis, atopic keratoconjunctivitis, contact allergy, giant papillary conjunctivitis. The most common is the seasonal form, which is linked to seasons. Allergic ocular surface processes require local treatment with artificial tears, anti-allergic eye drops. If com

A comprehensive review on vernal keratoconjunctivitis with emphasis on proteomics.

Allergic conjunctivitis presents as a spectrum of different clinical entities, such as perennial allergic conjunctivitis, seasonal allergic conjunctivitis, atopic keratoconjunctivitis and vernal keratoconjunctivitis. Vernal keratoconjunctivitis (VKC) is a disorder that is often associated with allergens and is seen during the spring season. Herein, we focused on vernal keratoconjunctivitis (VKC) and reviewed its epidemiology, clinical presentations, ocular associations, available treatment options, and the progressive understanding of its histopathological features; we have also systematically

Ocular allergy guidelines: a practical treatment algorithm.

The treatment of ocular allergy requires a better understanding of the spectrum of clinical disorders involving various components of the immune system, and of interactions at the conjunctival surface. The immune response focuses primarily on the different levels of activity of Th2 lymphocytes and various other immune cells associated with allergic disorders, including mast cells, eosinophils, fibroblasts, and epithelial and endothelial cells. Ocular allergic disorders include seasonal allergic conjunctivitis (SAC), perennial allergic conjunctivitis (PAC), vernal keratoconjunctivitis (VKC), gi
