Trends in Glycemia Risk Index Over Time in Patients With Diabetes and Cancer Undergoing Remote Continuous Glucose Monitoring.

BACKGROUND: Continuous glucose monitoring (CGM) is increasingly used in patients with cancer, a population in whom HbA1c may be unreliable. The Glycemia Risk Index (GRI) is a CGM-derived measure integrating hypoglycemia and hyperglycemia risk. We evaluated trends in GRI and other CGM metrics among adults followed in a remote CGM (RCGM) clinic. METHODS: Retrospective chart review of adults (≥18 years) with diabetes and cancer seen at a comprehensive cancer center in 2024. Eligible patients had ≥2 CGM assessments (≥1 RCGM) and a baseline glucose management indicator (GMI) or HbA1c ≥8.0%. Genera…

Utility of continuous glucose monitoring for identifying silent hypoglycemia in fructose-1,6-bisphosphatase deficiency: a pilot prospective evaluation.

BACKGROUND AND OBJECTIVE: Fructose-1,6-bisphosphatase (FBPase) deficiency is a rare gluconeogenic disorder characterized by hypoglycemia, lactic acidosis, hyperuricemia, and ketosis, triggered by fasting or infection. Although dietary management aims to prevent hypoglycemia, accurate tools to monitor asymptomatic episodes are lacking. Continuous glucose monitoring (CGM) has not been systematically evaluated in FBPase deficiency. This study aimed to assess the utility of CGM in detecting silent hypoglycemia and its relationship with dietary management. METHODS: Ten genetically confirmed patien…

Association Between Plant-Based Diet and Continuous Glucose Monitoring-Derived Glycemic Dynamics Among Pregnant Women With Gestational Diabetes.

We aim to investigate the association of plant-based diets with the continuous glucose monitoring (CGM)-derived glycemic metrics among gestational diabetes mellitus (GDM) patients. We included 1756 GDM patients in the present analyses and assessed plant-based dietary patterns through constructing a plant-based diet index (PDI), healthy PDI (hPDI), and unhealthy PDI (uPDI). CGM-glycemic metrics, such as time in range (TIR), mean blood glucose (MBG), time below range (TBR), low blood glucose index (LBGI), mean of daily differences (MODD), and glycemic risk assessment in diabetes equation (GRADE)…

A randomized controlled trial of real-time continuous glucose monitoring (RT-CGM) for self-management of gestational diabetes.

BACKGROUND: Gestational diabetes mellitus (GDM) - a risk factor for adverse pregnancy outcomes - is on the rise. Real-time continuous glucose monitoring (RT-CGM) may enhance glucose control during GDM and thereby improve outcomes; however, more data are needed. OBJECTIVE: This study aimed to determine if RT-CGM improved glycemic indices in GDM. STUDY DESIGN: An open-label, randomized controlled trial was conducted with GDM patients diagnosed between 14 and 30 weeks estimated gestational age (EGA), assigning them either to RT-CGM or blinded CGM in addition to self-monitoring of blood glucose …

A Pilot Study Using Continuous Glucose Monitoring among Patients with a Low 1-Hour Glucose Challenge Test Result versus Controls to Detect Maternal Hypoglycemia.

A low 1-hour glucose challenge test (GCT) result (<10th percentile for population) has been associated with neonatal morbidity, including small-for-gestational-age birth weight, and it is hypothesized that underlying maternal hypoglycemia may contribute to this neonatal morbidity. We sought to assess whether eligible patients would undergo continuous glucose monitoring to allow comparison of maternal hypoglycemia between those with a low GCT result versus controls. This exploratory study enrolled patients who completed a GCT between 24 and 30 weeks' gestation from June to September 2022. Englis…
