Fetal hydrops is associated with two distinct pathophysiologic situations. The isoimmune hydrops fetalis is a well understood disorder, and as the result of medical advances and prophylactic therapy its frequency is diminishing. The nonimmune hydrops fetalis is a poorly understood disease with a bad prognosis. The two disorders can be differentiated with the indirect Coombs test. In both cases the ultrasound examination plays an important role in the diagnosis, prognosis and management. Examination of the fetal blood sample gives recently a possibility to approach the disease. In NIHF the exam…

Examination of peripheral blood smears from a patient with sickle cell disease during crisis associated with an upper respiratory tract infection, pneumonia, a positive Coombs test, and an elevated cold agglutinin titer revealed unique abnormalities associated with her erythrocytes, which have not been reported previously. In addition to the usual changes in peripheral blood occurring during crisis in sickle cell anemia, large numbers of free, filamentous forms that appeared to be derived from erythrocytes were present on repeated Wright's-stained smears. Erythrocytes were markedly distorted a…

The antigenicity tests of Tazobactam/piperacillin (TAZ/PIPC), tazobactam (TAZ:beta-lactamase inhibitor) and piperacillin (PIPC:penicillin antibiotic) were performed in mice and guinea pigs. The following results were obtained. 1. TAZ/PIPC, TAZ or PIPC had no immunogenicity and allergenicity in either passive cutaneous anaphylaxis (PCA) test using BALB/c and C3H/He mice or in PCA test using guinea pigs. 2. Guinea pigs sensitized with TAZ/PIPC, TAZ or PIPC showed no anaphylactic symptoms in active systemic anaphylaxis (ASA) test. 3. Guinea pig PCA tests using protein conjugates as sensitizing an

A middle-aged man presented with myalgia, fever, hematemesis, and melena. He was found to have acute kidney injury, acute coronary syndrome, thrombocytopenia, and Coombs-negative hemolytic anemia, raising concern for thrombotic microangiopathy (TMA). While awaiting confirmatory testing, empirical plasma exchange was initiated. Initially, minimal red blood cell fragmentation was observed, which later progressed to florid fragmentation. Despite supportive care, his clinical status deteriorated requiring hemodialysis. The presence of TMA, along with ADAMTS13 metalloprotease activity above the dia

This report describes the diagnosis and successful management of a yearling filly with Coombs-positive anemia, thrombocytopenia, and fungal pneumonia. Diagnostic procedures, including thoracic ultrasonography and radiography, respiratory pathogen PCR testing, and evaluation of tracheal wash and bronchoalveolar lavage samples established multi-pathogen lower respiratory tract infection including a fungal agent. Orally administered voriconazole was a key component of treatment in this case to successfully eliminate fungal infection, alongside therapies for managing hematologic disease. This case

In order to evaluate the usefulness of a peripheral blood PCR assay in the posttreatment follow-up of brucellosis, a cohort of 30 patients was studied by means of blood cultures, rose Bengal, seroagglutination, Coombs' antibrucella tests, and PCR assay at the time of diagnosis, at the end of treatment, and 2, 4, and 6 months later. Of the 29 patients whose PCR assays were initially positive, 28 (96.5%) were negative at the conclusion of the treatment. PCR was positive for the two patients who had relapses and negative for another four who had suspected but unconfirmed relapses. PCR was negativ

To sum up, some hematological side effects are seen after antihypertensive drugs. They are, however, few, usually not severe and furthermore reversible. And so, from the viewpoint of the hematologist, there are no contraindications against antihypertensive treatment. The prevalence of side effects on blood and blood-forming organs from antihypertensive drugs is low. Of certain interest are 1) the tendency of modern diuretics (thiazides) to cause a moderate and reversible thrombocytopenia and 2) the positive antiglobulin reaction (Coomb's test) sometimes seen after alpha-methyldopa therapy and

BACKGROUND: Despite the fact that anemia is one of the most striking clinical features of visceral leishmaniasis (kala-azar), the factors involved in its pathogenesis are not fully understood. Although the cause of the anemia seen in these patients is often multifactorial, sequestration and destruction of the RBCs in the enlarged spleen, immune mechanisms, and alterations in RBC membrane permeability have been implicated.
STUDY DESIGN AND METHODS: To investigate whether RBCs of patients with kala-azar were coated with IgG, blood samples of 67 patients were tested, prospectively, before (Day 1

Hemolytic uremic syndrome associated with infection by neuraminidase-producing Streptococcus pneumoniae usually presents with fulminant pneumonia and has a high mortality rate. Post-pneumococcal hemolytic uremic syndrome may occur earlier in life than classical hemolytic uremic syndrome. We describe an 18-month-old male with hemolytic uremic syndrome, S. pneumoniae pneumonia, and T-antigen activation characteristic of neuraminidase activity. We have summarized the features of this case and 11 previously reported children. As the use of blood products containing IgM may aggravate this disorder,

Drug-induced haemolytic anaemia (DIHA) is a rare and potentially life-threatening condition associated with various medications, including cephalosporins. DIHA can be either immune-mediated or non-immune-mediated, distinguishable through direct antiglobulin testing, also known as Coombs testing. Cephalexin has been implicated in both forms. Non-immune DIHA likely results from direct drug toxicity to erythrocytes, whereas immune-mediated DIHA, a type II hypersensitivity reaction, involves drug-dependent antibodies and/or autoantibodies and is often triggered by penicillins, cephalosporins and n…

Paroxysmal Nocturnal Haemoglobinuria (PNH) is an acquired, rare life-threatening disorder characterised by compliment mediated hemolytic anemia, thrombosis and impaired bone marrow function. It occasionally presents in childhood or adolescence. This is a case of a 14-year old female presented with complaints of shortness of breath, palpitation and abdominal pain whose laboratory test results were consistent with Coomb's test negative haemolytic anaemia. Contrast enhanced Computed Tomography Scan (CT scan) of abdomen revealed splanchnic circulation thrombosis as well as partially occluding thro

A case is reported of hemolytic anemia following rifampicin administration and complicated by acute renal failure. Furthermore clotting analyses suggested a slight disseminated intravascular coagulation, very likely activated by hemolysis products. Both hemolysis and renal function impairment subsided spontaneously, after the sole withdrawal of rifampin. Direct antiglobulin test became negative within a few days, while an indirect Coomb's test was demonstrated persistently with the patient's serum using red blood cells sensitized in vitro with the drug. Otherwise from all reports in the litera

OBJECTIVES: To describe the serological and molecular characteristics of a rare Gerbich-negative phenotype detected in a Peruvian patient.
BACKGROUND: The Gerbich blood group (Ge) system includes high-prevalence antigens (Ge2, Ge3, Ge4) encoded by the GYPC gene. Absence of these antigens is extremely rare and has been reported mainly in Melanesian and Middle Eastern populations. To our knowledge, no molecularly confirmed cases have been reported in Peru.
METHODS/MATERIALS: A 65-year-old Peruvian man scheduled for orthopaedic surgery underwent pretransfusion testing, including an indirect ant

The production, function, and use of a new antiglobulin control cell is described. The cells are sensitized with monovalent Fab (fragment, antigen-binding) fragments and can be agglutinated by active antiglobulin serum only. In contrast to commercial antiglobulin control cells, which are frequently associated with false-positive results, the reactivity of the Fab-sensitized cells is entirely specific. It is concluded that Fab-sensitized cells provide greater quality assurance than currently available reagents.

INTRODUCTION: This study retrospectively characterized the laboratory features of direct antiglobulin test (DAT)-positive anemia in newly diagnosed multiple myeloma (MM) and explored associated immunologic alterations to better understand MM-related immune dysregulation.
METHODS: We enrolled 77 patients with newly diagnosed MM admitted to our hospital between 2021 and 2025. Based on DAT results, patients were assigned to a DAT-positive group (n = 52) or a DAT-negative group (n = 25). Clinical characteristics, laboratory parameters, and peripheral blood T-cell subsets were compared between gro
