Computed tomography images showed recurrent lower gingival squamous cell carcinoma spreading from the mandibular foramen to the middle cranial fossa. This extensive tumor spread followed exactly the same anatomic pathway as the inferior alveolar nerve. Histopathologic examination revealed squamous cell carcinoma. Tumor spread was thought to be caused by perineural invasion. This case demonstrates computed tomography features of perineural tumor spread along the inferior alveolar nerve.

Craniofacial malformations are common congenital defects caused by failed midline inductive signals. These midline defects are associated with exposure of the fetus to exogenous teratogens and with inborn genetic errors such as those found in Down, Patau, Edwards' and Smith-Lemli-Opitz syndromes. Yet, there are no studies that analyze contributions of synchronous neurocranial and neural development in these disorders. Here we present the first in-depth analysis of malformations of the basicranium of a holoprosencephalic (HPE) trisomy 18 (T18; Edwards' syndrome) fetus with synophthalmic cyclopi

Background The petrous part of the temporal bone plays a crucial role in various cranial surgical approaches, particularly those involving the middle cranial fossa. Understanding the morphometry of this region is essential for minimizing intraoperative risks and enhancing surgical outcomes. This study aims to provide a detailed morphometric analysis of the petrous bone and its anatomical landmarks in an Indian population, addressing a gap in the literature. Methods The study was conducted in the Department of Anatomy and Forensic Medicine and Toxicology and involved 100 heads of unclaimed adul

The extradural middle fossa approach is used to access lesions of the petroclival and cavernous sinus regions. It may be included in combined petrosal and anterolateral transcavernous approaches. Technically, it is a demanding exposure that provides a wide extradural corridor between the 5th, 7th, and 8th cranial nerves. Its major advantages are that it offers extradural dissection, limits temporal lobe retraction, and avoids the transposition of nerves or vessels. Its disadvantages are primarily related to the complicated anatomy of the petrous apex from the middle fossa trajectory, which can

The junction of the petrosal and squamosal portions of the temporal lobe has important relationships that are of interest to the otologist. The resultant suture extends from the glaserian fissure across the top of the middle ear cleft and into the mastoid portion of the temporal bone. It may permit quick passage of infection from the middle ear to the middle cranial fossa. The petrosa may override the squama, forcing it down into the tympanum, where it could cause malleus fixation and a conductive type hearing loss. In the mastoid the suture is identifiable on the surface, but in the interior

OBJECTIVE: To define the difference of meningiomas that originate in the area of Meckel's cave (MC) (primary MC meningiomas) in regard to the different surgical approaches and postoperative results.
METHODS: A retrospective analysis of all meningiomas involving the cranial base displayed 21 cases of meningiomas originating in MC (primary MC meningiomas). These cases were classified according to the tumor extension in four different types: Type I, tumors mainly confined to MC; Type II, MC meningiomas with extension into the middle fossa; Type III, MC meningiomas with extension into the posteri

The pneumatisation of the articular tubercle (PAT) of the temporal squama isa rare condition that modifies the barrier between the temporomandibular joint(TMJ) space and the middle cranial fossa. During a routine examination of thecone-beam computed tomography (CBCT) files of patients who were scannedfor dental medical purposes, we identified a case with multiple rare anatomicvariations. First, the petrous apex was bilaterally pneumatised. Moreover, bilateraland multilocular PAT were observed, while on one side it was further found thatthe pneumatic cells were equally dehiscent towards the ext

We report a 27-year-old male who presented with paroxysmal headache, stuffy nose, epistaxis and impairment of vision in each eye. A huge chondrosarcoma, arising from the skull base with extension into the middle cranial fossa and nasal cavity was suspected. The patient underwent craniotomy and endoscope-assisted transnasal resection and a satisfactory removal was achieved. Postoperative immunohistochemical examination confirmed the diagnosis of myxoma rather than chondrosarcoma. The patient remained neurologically intact except for poor vision in the left eye at six-month follow-up. The releva

OBJECTIVE: Recurrent cranial base meningiomas are among the most difficult tumors to treat surgically. Although they are histologically benign, these tumors often invade through the cranial base into the infratemporal and pterygopalatine fossae. We reviewed our experience with these tumors to describe the natural history of these lesions as well as provide a possible treatment paradigm.
METHODS: Between 2000 and 2004, seven patients with meningiomas recurring through the cranial base into facial structures were treated at the University of Utah. Five patients were treated with transcranial ap

Facial nerve schwannomas (FNS) are rare benign tumors that can develop in any segment of the facial nerve. Patients often experience facial palsy and hearing loss after FNS removal or even before surgery.1-3 Double-stage treatment is usually necessary for tumor resection and reconstruction of facial nerve function.4,5Video 1 describes the translabyrinthine approach for resection of the middle-posterior cranial fossa dumbbell FNS and end-to-side hypoglossal-facial nerve anastomosis in 1 stage. The case presented is of a 28-year-old female patient who suffered from right-side hearing loss for 1.

BACKGROUND: Petroclival tumors and ventrolateral lesions of the pons present unique surgical challenges. This cadaveric study provides qualitative and quantitative anatomic comparison for an anterior petrous apicectomy through the transcranial middle fossa (TMF) and expanded endoscopic transsphenoidal-transclival approaches.
METHODS: In 10 silicone-injected heads, the petrous apex and clivus were drilled extradurally using middle fossa and endonasal approaches. With in situ and frameless stereotactic navigation, we defined consistent points to compare working areas, bone removal volumes, appr

The goals of treatment in Meniere's disease are hearing preservation and control of vertigo. Controversy abounds with regard to the most expeditious and efficacious means of attaining these goals. Low-salt diet, diuretic therapy, endolymphatic-mastoid shunt, and middle cranial fossa vestibular neurectomy are all modes of treatment employed for Meniere's disease at the University of Iowa. This review of our experience from 1973 to 1980 summarizes the efficacy of each therapeutic option.

We report a 36 year-old woman who presented with headaches and hypoesthesia of the face. MRI revealed a large dumbbell shaped trigeminal neurinoma extending into both the middle and the posterior cranial fossae. In addition, there was a Chiari I malformation and syringomyelia. Within two months of tumor resection, both the Chiari malformation and the syringomyelia resolved. The association of an intracranial space-occupying lesion with Chiari malformation and syringomyelia is reviewed.

There is no consensus on the cause of Bell's palsy, just as there is no proven medical therapy. Disregarding the numerous theoretic causes, preponderant anatomic, electrophysiologic, radiologic, clinical, and pathologic evidence supports entrapment at the medical foramen of the fallopian canal as a final common pathway resulting in facial nerve ischemia and degeneration. Middle cranial fossa decompression of the meatal foramen may benefit those patients most severely affected with Bell's palsy.

OBJECTIVE: Considerable studies have focused mainly on the facial deformity of Tessier No.0 cleft with a bifid nose, but the deformity of the skull is not well understood. Therefore, our study aimed to explore the evolution of cranial dysmorphology and the chronology of Tessier No.0 cleft with a bifid nose, by three-dimensional measurements.
METHODS: Ninety-six non-surgical patients and computed tomographic scans were included (Tessier No.0 cleft with a bifid nose, n = 48; controls, n = 48) and divided into five age subgroups. Craniofacial cephalometric measurements were analyzed by Mimics so

Soft-tissue glomus tumors (or glomangiomas) are unrelated to neuroendocrine paragangliomas (glomus tympanicum, jugulare, and vagale). The authors present the first reported case of an orbital soft-tissue glomus tumor in a child. An 8-year-old girl developed rapidly progressive right-eye blindness, proptosis, and a sixth cranial nerve palsy. Magnetic resonance imaging demonstrated a homogeneously enhancing lesion extending from the right orbit through the superior orbital fissure to the cavernous sinus and middle cranial fossa. A biopsy specimen demonstrated the lesion to be a soft-tissue glomu

Cranial meningoencephalocele is a rare condition consisting of the herniation of meninges, CSF, and brain tissue through a cranial or skull base defect. Sphenoid sinus lateral recess meningoencephalocele is of particular interest due to the complex anatomy surrounding the bone defect and their demanding surgical management. In this technical note, we reported a step-by-step description of a rare case of sphenoid sinus lateral recess meningoencephalocele causing headache due to recurrent cerebrospinal fluid leak treated with a subtemporal craniotomy with extradural middle cranial fossa drilling

Reduction malarplasty (RMP) is performed frequently in East Asia. Although various complications of RMP have been reported, intracranial hemorrhage has been described only rarely. Although intracranial hemorrhage is an extremely rare complication, it is a catastrophic event that should not occur in esthetic plastic surgery. In this review, I describe the occurrence and prevention of this serious complication through a detailed analysis of intracranial hemorrhage following RMP. Evaluation of RMP surgical techniques was performed. A search of PubMed and Google Scholar was conducted to identify s
