Cavitary pulmonary cryptococcosis complicating Churg-Strauss vasculitis.

Churg-Strauss vasculitis (CSV) is an interesting but uncommon pulmonary vasculitis. Cavitation in CSV is distinctly unusual and should prompt a work-up for other causes. We have described a case of CSV complicated by pulmonary cryptococcosis, which was treated with a novel antifungal regimen.

Prostatic cryptococcosis in acquired immune deficiency syndrome.

A rare care is presented of recurrent dysuria and urinary obstruction due to cryptococcal prostatitis in a thirty-six-year-old male with acquired immune deficiency syndrome. Although there are a handful of reports in the world literature of patients with cryptococcal prostatitis, this appears to be the first case reported related to acquired immune deficiency syndrome.

R 51211 (itraconazole) therapy of murine cryptococcosis.

A new antifungal triazole, itraconazole (R51211), was compared with ketoconazole in treatment of murine cryptococcosis. Itraconazole is sparingly soluble in water, and must be administered orally in solvents such as polyethylene glycol. Serum concentrations are lower than those achieved with ketoconazole, but sustained for longer periods. Itraconazole was more effective than ketoconazole by MIC, but was similar to ketoconazole by minimum fungicidal concentration. Both drugs prolonged equally survival after intraperitoneal or intracerebral challenge with Cryptococcus neoformans. Tissue counts o…

Thoracic cryptococcosis: immunologic competence and radiologic appearance.

Cryptococcosis, a potentially fatal pulmonary infection, is being recognized with increased frequency in both immunologically compromised patients and those without obvious predisposing factors. This review of 24 cases of proven pulmonary cryptococcosis includes 15 immunologically compromised patients and nine noncompromised hosts. The noncompromised hosts tended to have a radiographic pattern of a peripheral nodule or nodules. The compromised hosts demonstrated a wider variety of radiographic abnormalities including single nodules, multiple nodules that progressed to confluence and/or cavitat…

Cutaneous cryptococcosis without evidence of systemic involvement.

As many as 15% of cases of systemic infection with Cryptococcus neoformans have cutaneous involvement. The existence of primary cutaneous disease is controversial. We report a patient with cutaneous cryptococcosis without evidence of visceral involvement at the time of diagnosis. She has been followed up for 5 years and remains free of systemic involvement. Cutaneous cryptococcosis does not always signify systemic disease.

Enzyme-linked immunosorbent assays in murine cryptococcosis.

In this investigation, enzyme-linked immunosorbent assay (ElISA) procedures were used to study the time of appearance and the duration of demonstrable antigen and antibody in body fluids of mice with disseminated cryptococcosis. The ELISA antigen procedure detected cryptococcal capsular polysaccharide (CCP) in the serum and urine of infected mice 3 days after infection--4 days before it could be demonstrated by the latex agglutination procedure. ELISA-reactive antibody was present throughout the course of infection (mean death time, 32 days), whereas antibody was not detected by whole cell agg…

Laryngeal Cryptococcosis: An Evolving Rare Clinical Entity.

OBJECTIVES: Describe the demographics and clinical manifestations of laryngeal cryptococcosis. Develop a simple approach to the diagnostic workup and treatment of localized laryngeal cryptococcal infection. METHODS: A new case of laryngeal cryptococcosis encountered at our institution is presented and placed in context of the literature surrounding prior reported cases. PubMed, Google Scholar, SCOPUS, and Web of Science were queried from inception to August 2018 with the terms Larynx or Laryngeal and Cryptococcosis or Cryptococcus by two independent reviewers for English-language cases of cry…

Unilateral Adrenal Cryptococcosis on FDG PET/CT.

A 70-year-old man with T-cell deficiency experienced continuous right waist pain and a low degree of fever. Abdominal CT found a large right adrenal mass. FDG PET/CT imaging was acquired to identify the nature of this likely malignant mass and search for potential metastases. The images revealed increased FDG activity in the mass. The patient received right adrenalectomy. Postsurgical pathological and microbial examination demonstrated adrenal cryptococcosis.

Neonatal Cryptococcosis: Beware of False-positive Results.

Although cryptococcal meningitis is uncommon in children and rare in neonates, it does occur. We highlight circumstances in which the diagnosis should be considered and methods required to confirm the diagnosis in young patients.

Pretransplant cryptococcosis and outcome after liver transplantation.

The posttransplant outcomes and optimal management of patients with end-stage liver disease who develop cryptococcosis prior to transplantation have not been defined. We discuss these issues in the context of successful liver transplantation and pretransplant cryptococcal disease. Our report suggests that liver transplantation may be cautiously considered under the umbrella of fluconazole therapy in patients with end-stage liver disease and pretransplant cryptococcosis, provided that disease control is achieved with adequate treatment before transplantation.

Cryptococcosis in nonhuman immunodeficiency virus-infected children.

Between 1991-2006, nine patients below age 18 years, with a microbiologic documentation of Cryptococcus neoformans infection and no evidence … pectively. With prompt antifungal therapy, all survived, but one presented the sequel of blindness. Cryptococcosis is uncommon in the nonhuman immunodeficiency virus-infected pediatric population. Cutaneous cryptococcosis in solid organ transplant recipients.

Clinical manifestations, treatment, and outcomes of cutaneous cryptococcosis in solid organ transplant (SOT) recipients are not fully defined. In a prospective cohort comprising 146 SOT recipients with cryptococcosis, we describe the presentation, antifungal therapy, and outcome of cutaneous cryptococcal disease. Cutaneous cryptococcosis was documented in 26/146 (17.8%) of the patients and manifested as nodular/mass (34.8%), maculopapule (30.4%), ulcer/pustule/abscess (30.4%), and cellulitis (30.4%) with 65.2% of the skin lesions occurred in the lower extremities. Localized disease developed i…

[Laryngeal cryptococcosis revealing AIDS: a case report].

The cryptococcal infection of the larynx is uncommon. We present a case of a 65-year-old woman who was seen because of chronic hoarseness. Direct laryngoscopy revealed an infected lesion of the laryngeal vestibule. Histologic examination showed yeasts surrounding by capsules allowing a diagnosis of laryngeal infection by cryptococcus neoformans. The patient had also meningitis and pulmonary localisation. We have undergone a risk factors research and this results in finding out that HIV serology was positive. She received a treatment with amphotericin B followed by fluconazole during 6 months w
