A 73-year-old woman with a history of chronic lymphocytic leukemia had bilateral dacryocystorhinostomies for bilateral chronic dacryocystitis. The procedures were performed 7 months apart. Twenty-four months after the first dacryocystorhinostomy on the left side, the patient developed a mass in the area of the left lacrimal sac causing epiphora. AIM OF THE STUDY: To assess the validity of transcanalicular diode laser assisted dacryocystorhinostomy (TLADCR) in the treatment of distal obstruction of the lachrymal pathways.
MATERIAL AND METHOD: Between June 1999 and February 2000, 29 TLADCR were performed in the ENT department on 26 patients. Surgical indications included chronic dacryocystitis in 19 cases and dacryostenosis in 10 cases. In all cases but one, the surgery was carried out under a standard general anesthetic. The microbial flora of 738 patients with acute bacterial conjunctivitis, corneal ulceration, blepharitis, dacryocystitis, and discharging sockets has been investigated. We report a 38-year-old man who presented with a 7-year history of a mass in the right medial canthal region. He had several episodes of dacryocystitis, epiphora and bloody discharge. A cystic lesion was defined on MRI. Lacrimal sac excision was performed and a dark red mass was extirpated. External dacryocystorhinostomy and bicanalicular silicone tube intubation was performed. In this report, we describe three adult patients diagnosed with lacrimal sac pyogenic granuloma. The presenting symptoms were acute dacryocystitis, lacrimal mass, and bloody tears. The nasolacrimal drainage pathway was obstructed in all cases. Radiologic evaluation performed in one patient revealed the presence of a well-defined mass in the sac with homogenous contrast uptake.
