Thinking beyond the obvious: hepatotoxicity secondary to idiosyncratic depakote toxicity.

Depakote-induced hepatotoxicity has been well established as an adverse effect, and periodic monitoring of drug level is often required. Depakote-induced hepatotoxicity mostly occurs at supratherapeutic drug level. Rarely, an idiosyncratic response is triggered, and hepatotoxicity can occur at the therapeutic drug level mostly in chronic users. Here, we describe a rare case of idiosyncratic depakote-induced hepatotoxicity. A 25-year-old female with non-insulin-dependent diabetes mellitus, hypothyroidism, seizure disorder, and Dandy Walker Syndrome presented with an unwitnessed seizure and alte

Perirolandic cortex in infants: signal intensity on MR images as a landmark of the sensorimotor cortex.

PURPOSE: To correlate the perirolandic low signal intensity seen on T2-weighted magnetic resonance (MR) images in neonates and infants with … signal intensity. In contrast, an apparent mismatch was found in two hemispheres in a patient with Dandy-Walker syndrome. CONCLUSION: The perirolandic low signal intensity seen on T2-weighted MR images is located exactly in the anatomic sensorimotor cortex in normal brains, whereas a mismatch can occur in abnormal brains.

Asymptomatic Dandy Walker Malformation In An Elderly Male With Acute Haemorrhagic Stroke - A Case Report.

OBJECTIVES: Polyarteritis nodosa (PAN) is a rare vasculitis in childhood and poor information is known about its long-term outcome. Our aim was to describe the clinical features, at onset and during the disease course, of childhood-onset PAN and identify a potential correlation with persistent organ damage and worse outcome in a cohort of paediatric patients with a confirmed diagnosis of PAN. METHODS: A retrospective collection of demographic and clinical data of 52 Caucasian children diagnosed with PAN, fulfilling the EULAR/PRES diagnostic criteria, recruited from eight paediatric rheumatolo

Expansion of the CCDC22 associated Ritscher-Schinzel/3C syndrome and review of the literature: Should the minimal diagnostic criteria be revised?

The Ritscher-Schinzel syndrome (RTSCS) is a rare condition with craniofacial, cardiac and fossa posterior abnormalities. RTSCS is subdivided into Ritscher-Schinzel syndrome 1 (RTSCS1) caused by pathogenic variants in coiled-coil domain-containing protein 22 (CCDC22), and Ritscher-Schinzel syndrome 2 (RTSCS2) caused by pathogenic variants in WASH complex subunit 5 (WASHC5). CCDC22 is inherited in an X-linked recessive manner while WASHC5 is inherited in an autosomal recessive manner. Only 17 individuals with a molecular diagnosis are reported. In the past, the diagnosis of RTSCS was solely base

Direct demonstration of Staphylococcus biofilm in an external ventricular drain in a patient with a history of recurrent ventriculoperitoneal shunt failure.

External ventricular drains (EVD) are associated with a high infection rate. Early detection of infection is frequently problematic due to a lack of clinical signs and the time period required for culturing. Bacterial biofilms have been suggested to play an important role in the infection of EVD, but direct evidence is as yet lacking. We report the case of a 17- year-old male with Dandy-Walker malformation who presented with headache, nausea and drowsiness; a CT scan revealed enlarged ventricles. The patient had a history of ventriculoperitoneal shunt revision 3 weeks prior to admission. The s

Dandy-Walker's variant and tetralogy of Fallot with atrial septal defect and patent ductus arteriosus and primary hypothyroidy--a new association.

An 11-month-old girl who has Dandy-Walker's variant (DWV) associated with tetralogy of Fallot (TOF), atrial septal defect (ASD), patent ductus arteriosus (PDA), and primary hypothyroidy is presented. There has been no report describing a case of DWV associated with TOF, ASD and PDA and primary hypothyroidy. The first case of Dandy-Walker malformation associated with TOF was reported by Kohyama et al in 1988, since then, a few cases were reported in the literature. Our patient is the first reported case.

[Enlargement of the fetal posterior fossa--what is the significance?].

The posterior fossa may be enlarged due to: mega cisterna magna, arachnoid cyst or Dandy Walker malformation. These anomalies can be identified during routine ultrasound screening performed in the beginning of the second half of pregnancy (22nd week). The outcome of these findings is not clear. Therefore, it is difficult to advise the parents how to manage the pregnancy after the discovery of this anomaly. Most of the studies report a good prognosis in cases of mega cisterna magna or arachnoid cyst, as long as this finding is isolated. A Dandy-Walker malformation or non-isolated enlargement of

Sonographic diagnosis of fetal cerebral ventriculomegaly: an update.

Dilatation of the fetal cerebral ventricles (ventriculomegaly) is a generic sonographic sign that is common to several pathological entities carrying different prognoses. The main causes of fetal ventriculomegaly are aqueductal stenosis, Chiari II malformation, Dandy-Walker complex, and agenesis of the corpus callosum. Ventriculomegaly is easily recognized by ultrasound by measuring the atrial width. This simple measure allows the recognition of mild forms of ventricular dilatation and is used in screening for ventriculomegaly. However, although the diagnosis of ventriculomegaly is easy, the p

Appearance of an interhemispheric cyst associated with agenesis of the corpus callosum.

We describe a fetus with agenesis of the corpus callosum (ACC) and Dandy-Walker malformation that developed a frontal paramidline cyst late in gestation. The interval appearance of the cyst occurred in concert with increasing size of the lateral ventricles, which supports the hypothesis that cysts associated with ACC can develop with increasing intraventricular pressure. Recognition of the potential for a changing appearance of neurologic abnormalities is important to providing appropriate patient counseling.

Magnetic resonance imaging and ultrasound in the assessment of the fetal central nervous system.

Ultrasound is the screening modality of choice for evaluation of the fetal central nervous system (CNS). However, in cases of difficult diagnosis further fetal investigation is desirable. Due to ultrafast magnetic resonance imaging (MRI) techniques artifacts from fetal motions are minimized. MRI involves no exposure to radiation and hence appears to be safe. Due to the better soft tissue contrast, additional investigation by MRI may extend the sonographic diagnosis of fetal CNS-anomalies. Ultrasound and MRI are complementary imaging methods in the evaluation of the fetal CNS. The most importan

Neuroendoscopic third ventriculostomy in dysmorphic brains.

Neuroendoscopic third ventriculostomy (NTV) has superseded shunting as the treatment of choice in non-communicating hydrocephalus. Intracranial developmental anomalies have been considered to be relative contraindications for this procedure. We present one patient with a Dandy-Walker malformation and another with septo-optic dysplasia who presented with hydrocephalus. An NTV was performed successfully in both patients using a flexible neuroendoscope and utilising transendoscopic Doppler ultrasound to ensure a safe target area for ventriculostomy.

External obstructive hydrocephalus: a study of clinical and developmental aspects in ten children.

At this time there are few published cases on this problem, but we have found that the syndrome does seem to correct itself with time. As the patients become stronger and their age increases, they catch up with their peer group. A 1978 study by Robertson documented five of six children attaining normal levels of skills appropriately. In another recent study, Ment researched eighteen infants with seventeen eventually developing within normal limits. Shunting of CSF through surgical intervention does not seem to be needed. We hope to continue our study and follow these children on a long-term ba

Genetics and genetic counselling in neonatal hydrocephalus.

In the majority of cases, neonatal hydrocephalus is a genetic disease. The obstetrician's role in antenatal diagnosis of this disease is important. It is essential to take an accurate family history and to perform autopsies on affected infants if successful genetic counselling is to be pursued. Every woman, at her first antenatal visit, should be asked if she had a positive family history of hydrocephalus. Advanced experience in sonography will make the detection of hydrocephalus possible before the 20th week of gestation.

Brain abnormalities on MR imaging in patients with retinoblastoma.

BACKGROUND AND PURPOSE: Although pineoblastoma is the main brain abnormality associated with hereditary retinoblastoma, recent studies suggest an association with pineal cysts. This association is important because some pineoblastomas mimic pineal cysts. If there is a relationship, then radiologists should be aware of it because diagnostic confusion is possible. Mental retardation and congenital brain anomalies are also reported in patients with retinoblastoma, mostly in combination with 13q deletion syndrome. In this retrospective study, the presence of brain abnormalities on MR images in a l

Cerebellar disorders in childhood: cognitive problems.

Over the last decade, increasing evidence of cognitive functions of the cerebellum during development and learning processes could be ascertained. Posterior fossa malformations such as cerebellar hypoplasia or Joubert syndrome are known to be related to developmental problems in a marked to moderate extent. More detailed analyses reveal special deficits in attention, processing speed, visuospatial functions, and language. A study about Dandy Walker syndrome states a relationship of abnormalities in vermis lobulation with developmental problems. Further lobulation or volume abnormalities of the

Infantile osteopetrosis and juvenile xanthogranuloma presenting together in a newborn: a case report and literature review.

Osteopetrosis (OP) is a clinically and genetically heterogeneous disorder characterized by increased bone density. Associations between OP and other clinical entities are rare but include muscular degeneration, Dandy-Walker syndrome, craniosynostosis, and poikiloderma. Infantile OP has also been diagnosed in a group of infants with neuronal storage disease. An association between OP and juvenile xanthogranuloma (JXG) has never been previously reported. Herein we present a case of an intermediate form of OP in a newborn who presented with hepatosplenomegaly and pancytopenia. Histologic evaluati…
