Total femur replacement in non-oncologic indications: Functional and radiological outcomes from a French survey with a mean 6 years' follow-up.

INTRODUCTION: There are few published studies on total femur replacement (TFR) because its indications are rare. Other than malignant diseases, the indications extend to revisions and interprosthetic femur fractures; however, the outcomes of these indications have not been well defined. The aim of this retrospective survey was to analyze the complication rate and functional outcomes of these newer indications. HYPOTHESIS: The morbidity and outcomes after TFR are comparable to those reported in the literature for non-cancer indications. MATERIAL AND METHODS: Between 1997 and 2016, 29 TFR proc

Should dry spinal tuberculosis be managed differently than wet spinal tuberculosis?

BACKGROUND CONTEXT: On radiopathological examination of spinal tuberculosis (TB), 2 predominant forms are known: dry and wet types. Wet TB, as the name suggests, has abscess formation as its predominant presenting feature and is the exudative form; dry TB includes caseation and sequestration with minimal exudate. Dry TB often exhibits poorer recovery patterns than the wet counterparts, which can be possibly ascribed to vasculitis, ischemia, or tubercular myelitis, rather than isolated mechanical compression. These pathologic processes may lead to neurological deficit which is less responsive t

Total joint replacement of the hip and knee in patients with arthrogryposis multiplex congenita: a report of six joints.

INTRODUCTION: Arthrogryposis multiplex congenita (AMC) is a rare congenital condition that leads to severe joint contractures and deformities. As painful joint dysplasia and degeneration might develop over time, total joint replacement (TJR) can be a potential treatment option for these patients. The aim of this study is to investigate functional results, implant survivorship and potential complications in patients with AMC who undergo hip or knee arthroplasty. MATERIALS AND METHODS: We retrospectively identified six TJR in three patients at a single centre performed between 2006 and 2019. Th

Short- to long-term follow-up of total femoral replacement in non-oncologic patients.

BACKGROUND: Compromised bone stock and heavily impaired structural integrity after multiple endoprosthetic revision surgeries can lead to a comparable condition as encountered in musculoskeletal tumor surgery. Total femoral replacement (TFR) can restore femoral integrity and allow patients to resume ambulation. Even though several authors reported their results of TFR, so far many questions are still on debate: Which patients are at risk to experience low functional outcome? Do complications and clinical outcome after TFR depend on the indication for the surgery (e.g. periprosthetic fractures

Long-term locomotor training for gait and balance in a patient with mixed progressive supranuclear palsy and corticobasal degeneration.

BACKGROUND AND PURPOSE: Progressive supranuclear palsy (PSP) and corticobasal degeneration (CBD) are slowly progressive tauopathies characterized by impaired balance, disturbances in gait, and frequent falls, among other features. Wheelchair dependence is an inevitable outcome in people with these disorders. Insufficient evidence exists regarding the effectiveness of exercise in the management of people with these disorders. This case report describes a program of exercise and long-term locomotor training, using a treadmill (both with and without body-weight support), to reduce falls and impro

The effect of ambulatory status on outcomes of percutaneous vascular interventions and lower extremity bypass for critical limb ischemia in the Vascular Quality Initiative.

OBJECTIVE: Ambulatory status has been shown to be an important predictor of postoperative morbidity and mortality for a variety of surgical procedures. We sought to assess contemporary practice patterns in treating critical limb ischemia (CLI) and outcomes based on ambulatory status. METHODS: The Vascular Quality Initiative (2010-2015) was queried for patients undergoing percutaneous vascular interventions (PVIs) or lower extremity bypass (LEB) for CLI. Ambulatory status was classified as ambulatory, ambulatory with assistance, and nonambulatory (composite of wheelchair bound and bedridden).

Correlation between cerebellar lesion topography and differential diagnosis with clinical presentation in dogs.

BACKGROUND: Cerebellar diseases in dogs cause diverse neurologic signs depending on the affected region. Although functional topographic maps linking cerebellar areas to motor, behavioral, and vestibular functions have been established in humans, such mapping is poorly characterized in veterinary medicine. HYPOTHESIS/OBJECTIVES: Evaluate how specific clinical signs in dogs correspond to cerebellar lesion locations and contribute to differential diagnoses. ANIMALS: One hundred two client-owned dogs from 4 referral centers in Spain and the United Kingdom. METHODS: Multicenter retrospective st

Biomechanical analysis of step-up and step-down tasks in knee osteoarthritis: Insights from leading and trailing limbs.

BACKGROUND: Stair climbing tests are pivotal when assessing physical performance in knee osteoarthritis patients, yet the biomechanical strategies that underpin poor stair climbing ability are heterogeneously reported. Single step tasks emulate a step-by-step gait pattern, an approach associated with knee pain when stair climbing. The objective of this study is to analyse the biomechanics and electromyography activity of both the leading and trailing limbs during single Step-up and Down tasks in knee osteoarthritis patients. METHODS: Three-dimensional motion analysis captured biomechanical da

Classifying Patient Characteristics and Determining a Predictor in Acute Stroke Patients: Application of Latent Class Analysis in Rehabilitation Practice.

Background/Objectives: Predicting comprehensive patient characteristics is essential for optimal individualized rehabilitation plans for acu … nine distinct classes (class size: 4-29%). Class 1, representing the mildest cases, was noted for independent ambulation and good upper limb function. Class 2 comprised those with the most severe clinical outcome. Other classes exhibited a gradient of severity, commonly encountered in clinical practice. For instance, Class 7 included right-sided paralysis with preserv …

Six-minute walk test and pulmonary function in ambulatory children with early-onset scoliosis.

PURPOSE: The six-minute walk test (6MWT) measures aerobic function, but normative values for early-onset scoliosis (EOS) do not exist. This study assesses 1) 6MWT distances for children with EOS and standard deviations [SD] from age- and sex-matched normal controls, and 2) correlations between 6MWT and pulmonary function tests (PFTs). METHODS: We performed a retrospective review of 6MWTs administered at a single institution from 2012 to 2023. PFTs were obtained: Forced vital capacity (FVC) percent (%) and forced expiratory volume in one second (FEV1) %. RESULTS: 163 ambulatory children with

Case Report: Unique management strategy for rare case of esophageal foreign body.

BACKGROUND: Foreign bodies that enter the esophagus can cause serious complications that may require extensive surgical intervention, including open surgery. The treatment method depends on the location, size, configuration, and number of foreign bodies in the esophagus, but to date, the best method to remove foreign bodies from the esophagus remains uncertain. Foreign bodies which can damage the walls of esophagus varies from bones and bone fragments, to metallic objects and batteries. In this article, we present a rare case of a "fish bone" penetrating through the esophagus walls and directl

A Missed Case of Synovial Osteochondromatosis.

Synovial osteochondromatosis or synovial chondromatosis is a benign joint pathology characterized by the development of multiple cartilaginous nodules or loose bodies in the synovial membrane that typically arise in the larger joints of the body. It usually presents as joint pain and, as seen in the present case, can occasionally be missed. Diagnosis involves a combination of clinical evaluation, imaging studies, and histopathological evaluation (which is confirmatory). Treatment depends on the severity of the disease, symptoms, and the patient's social situation, and may include monitoring fo

Definitions matter: Heterogeneity of COVID-19 disease severity criteria and incomplete reporting compromise meta-analysis.

Therapeutic efficacy in COVID-19 is dependent upon disease severity (treatment effect heterogeneity). Unfortunately, definitions of severity vary widely. This compromises the meta-analysis of randomised controlled trials (RCTs) and the therapeutic guidelines derived from them. The World Health Organisation 'living' guidelines for the treatment of COVID-19 are based on a network meta-analysis (NMA) of published RCTs. We reviewed the 81 studies included in the WHO COVID-19 living NMA and compared their severity classifications with the severity classifications employed by the international COVID

The short-term associations of weather and air pollution with emergency ambulance calls for paroxysmal atrial fibrillation.

A circadian variation in the cardiovascular parameters has been detected. It is plausible that the influence of the environment varies during different periods of the day. We investigated the association between daily emergency ambulance calls (EC) for paroxysmal atrial fibrillation (AF) that occurred during the time intervals of 8:00-13:59, 14:00-21:59, and 22:00-7:59, and weather conditions and exposure to CO and PM10. We used Poisson regression to explore the association between the risk of EC for AF and environmental variables, adjusting for seasonal variation. Before noon, the risk was as
