Contact sensitivity to flavourings and perfumes in atopic dermatitis.

16 children with atopic dermatitis and 4 nonatopics were skin tested with flavourings and perfumes. Immediate reactions to balsam of Peru and fragrance-mix were found in 9 atopics, and none among nonatopics. An irritant is more probable than an immunologic mechanism. Allergen solutions should probably be assayed at a lower concentration in atopic patients. This study points to a possible aggravating factor from perfumes and flavourings ingested, inhaled, or used as cosmetics.

Flare factors and atopic dermatitis: the role of allergy.

Atopic dermatitis is a genetically determined eczematous skin disease strongly influenced by environmental conditions called flare factors. Allergic reactions are one such flare factor. These reactions include contact urticaria, allergic contact dermatitis, and late phase reactions. Contact urticaria could induce eczema by eliciting scratching. A late phase reaction may be involved in eczema produced by prolonged epicutaneous applications of antigens in individuals with immediate sensitivity to these antigens. Mechanisms of allergic contact dermatitis might also elicit dermatitis. Environmenta

Atopic dermatitis in the first six months of life.

1,476/2,320 of our cases of atopic dermatitis (AD) start in the first six months of life. The diagnosis is usually easy, but at this age it is sometimes more difficult, mainly because of lack of or rare evidence of scratching, but also because of the brief clinical history that does not allow observation of the characteristic chronic and relapsing course. Moreover, the major atopic disorders-asthma, rhinitis-usually appears later in the natural history of atopic subjects. From a differential diagnosis point of view, AD is the most definite dermatological disorder at this age. Other not well de

Sleep and nighttime pruritus in children with atopic dermatitis.

The number of scratching episodes and average frequency with which they started during each sleep stage as well as the effects of nighttime pruritus on objective sleep parameters in nine children with atopic dermatitis were assessed in the sleep laboratory. Scratching episodes occurred during rapid eye movement (REM) sleep and non-REM (NREM) sleep. The largest average frequency corresponded to stage 1, followed by stage 2, REM sleep, stage 4, and stage 3. Sleep maintenance was markedly altered. Total sleep time decrease was related mainly to smaller amounts of stage 4 NREM sleep. REM sleep per

[Recommendations for the treatment and prevention of atopic dermatitis in children].

Atopic dermatitis (AD) is a chronically-relapsing disease of the skin caused by hereditary hypersensitivity to a lot of environmental substances. Disturbances of humoral and cellular immunity, abnormal vascular reactions and abnormal skin functions play a role in pathogenesis. AD shows different forms in different age-groups with pruritus as a commune feature. The treatment of AD is unsatisfactory and effective only by consequent cure. The principles are avoiding of pruritus and scratching, avoiding of chemical and mechanical irritants of the skin and avoiding of bacterial superinfections. Top

Increased prevalence of atopic dermatitis in Kawasaki disease.

Atopic dermatitis is associated with immunoregulatory abnormalities similar to those observed in acute Kawasaki disease. We investigated whether the prevalence of atopic dermatitis is increased among children who acquire Kawasaki disease. In a case-control telephone survey 83 Kawasaki disease patients and 83 children with innocent heart murmurs were matched for age and time between clinic visit and interview. The interviewer was blinded to the hypothesis of the study. Nine (11%) Kawasaki disease patients but only one (1%) unaffected child had atopic dermatitis; the incidence of atopic dermatit

Atopic dermatitis. An evaluation of clinical and laboratory findings.

These hundred seventy-two Chinese patients with atopic dermatitis were compared with a Caucasian population for the basic features. Significant findings occurred: personal or family history of atopy, early age of onset, xerosis, ichthyosis, palmar hyperlinearity, and facial pallor. Laboratory findings of immunologic alteration and altered pharmacologic reactivity are relatively more important features for the diagnosis of atopic dermatitis.

Skin reactions to foods in patients with atopic dermatitis.

Thirty-seven patients with moderate or severe persistent atopic dermatitis gave a history of skin reactions following the ingestion of certain foods. These reactions fell into three groups: immediate reactions (within one hour) which included angio-oedema, contact urticaria, generalised itching and erythema, or urticaria; late reactions, where patients experienced late urticaria, late angio-oedema, itching, or exacerbation of the dermatitis; dual reactions where immediate reactions were followed by exacerbation of the atopic dermatitis. The foods implicated were either potential allergens or r

The morphologic characteristics of dry skin in atopic dermatitis.

Uninvolved skin sites in 436 consecutive patients, 6 to 25 years old, with atopic dermatitis were observed during the winter months (from November to February). Ichthyosis vulgaris occurred in 133 patients. Of the 303 remaining patients, only 11 (4%) had generalized dry skin; 191 (63%) exhibited focal areas of dry skin; and 95 (33%) showed only normal-appearing skin. Microscopically, in 41 patients, dry skin associated with atopic dermatitis showed mild eczematous changes. Dry skin coexistent with ichthyosis in patients with atopic dermatitis revealed ichthyotic changes frequently superimposed

Atopic dermatitis. A review of diagnosis, pathogenesis, and management.

Atopic dermatitis, one of the common dermatologic abnormalities encountered by pediatricians in clinical practice, is a chronic pruritic skin disorder occurring in individuals with a personal or family history of atopic disease such as asthma or allergic rhinitis. Atopic dermatitis appears to be an inherited disease, although the mode of inheritance has not been established. Clinically, its course can be divided into stages: infantile, childhood, and adult. Recognition of atopic dermatitis in any of these stages should facilitate appropriate treatment. This general overview examines several as

IgE levels and PMN chemotaxis in atopic dermatitis.

In the recent years immunological abnormalities, including defective polymorphonuclear neutrophils (PMN) chemotaxis and both quantitative and functional defects in T-lymphocytes, have been reported in atopic dermatitis (AD). To date there is still a controversy regarding the causes of PMN chemotaxis defect and the possible mechanism underlying this abnormality in patients with AD. The aim of this study was to investigate the PMN chemotactic function in 18 children affected with AD and high levels of serum IgE, and in 4 children with Hyper IgE Syndrome (HIES). Our results showed a marked PMN ch

Hyperlinear palms: association with ichthyosis and atopic dermatitis.

The skin and palmar markings were examined in 178 patients with atopic dermatitis, 66 (37%) of whom had ichthyosis vulgaris. Increased palmar markings were found in 49 (28%) patients, while a normal pattern of palmar markings was present in 129 (72%) patients. Increased palmar markings were commonly observed in patients with atopic dermatitis and ichthyosis, while normal palms were found in almost all patients with atopic dermatitis alone. It is suggested that the increased palmar markings often seen in patients with atopic dermatitis are actually a manifestation of concomitant ichthyosis vulg

Immediate hypersensitivity to seminal fluid and atopic dermatitis.

A woman with atopic dermatitis experienced anaphylactic episodes following intercourse with her husband, with subsequent exacerbations of her atopic dermatitis. Skin testing and an in vitro leukocyte histamine release assay established the diagnosis of immediate hypersensitivity to her husband's seminal fluid; delayed hypersensitivity to seminal fluid could not be demonstrated. Antigen was found in the seminal fluid of nonrelated men. Radioallergosorbent testing detected the presence of circulating IgE antibodies specific for seminal plasma protein. Immunotherapy with seminal plasma may have l

Atopic dermatitis: clinical and immunologic aspects and treatment.

Atopic dermatitis is a chronic disease marked by exacerbations and remissions. It begins in early infancy and may persist into late adulthood. Flares of dermatitis may be precipitated by emotional stess, extremes or sudden changes in humidity or temperature, and other factors. Treatment consists primarily of the use of mild topical agents to reduce inflammation and pruritus. Long-term systemic therapy with corticosteroid is not recommended. Topical preparations containing agents that increase the local levels of cyclic adenosine monophosphate (eg, caffeine) may be useful.

Contact and bacterial allergy in children with atopic dermatitis.

Cell-mediated immunity to bacterial antigens was studied by intradermal testing, and to contact antigens (cosmetics, environmental chemicals and topical medicaments) by patch testing in 270 children with atopic dermatitis. The incidence of delayed-type immune cutaneous reactions in these patients was lower than in the controls. Contact allergy is a rare finding in the first 4 years of life but its incidence increases subsequently. In subjects with atopic dermatitis the incidence of sensitization by contact with allergens contained in topical medicaments proved to be lower than in subjects with

OBSERVATIONS ON THE ANTIPRURITIC EFFECT OF GUANETHIDINE IN ATOPIC DERMATITIS.

Guanethidine, 0.75 to 1.25 mg. per kg. per day, was found to alleviate the pruritus of acute atopic dermatitis in 19 out of 20 patients. It was not effective in patients with other dermatoses. An apparent partial tolerance to this treatment developed in six out of 10 patients re-treated. It had little or no therapeutic effect in 10 patients with a variety of other pruritic dermatoses, suggesting that this drug has a specific antipruritic action in patients with atopic dermatitis. In a double-blind study, 11 of 12 patients with atopic dermatitis treated with guanethidine experienced relief of p

Autologous Th2-polarized lymphocytes induce atopic dermatitis lesions in non-atopic human skin xenotransplants.

BACKGROUND: The key signals that suffice to induce atopic dermatitis (AD) in human skin remain incompletely understood. Also, current mouse models reflect human AD only unsatisfactorily. Therefore, we have asked whether a humanized AD mouse model can be developed that reflects human AD more faithfully and permit to identify key signals that suffice to induce AD lesions in previously healthy human skin in vivo. METHODS: Healthy human skin from non-atopic donors was transplanted onto SCID/beige mice. After xenotransplant reinnervation by mouse sensory nerve fibers had occurred, mixed autologous
