I read with interest the excellent report by Tolkachjhov et al1 that describes the delay in diagnosis of a scabies infestation in a 90-year-old woman. Her condition mimicked dermatitis not only on clinical presentation but also on evaluation of skin biopsies.

We report a 64-year-old man who presented with generalized erythroderma and erosions. The erythroderma improved generally as a result of systemic prednisolone treatment. After treatment, however, the patient developed annular erythema with tiny pustules. Histopathology, ELISA and immunoblot analysis showed the disease to be pemphigus foliaceus (PF) with prominent neutrophilic pustules. To our knowledge, this is the first known case of PF with prominent neutrophilic pustules presenting as erythroderma.

Psoriasis is a common chronic skin disorder estimated to affect about 2% of the Western population. The disease creates a significant stigma for patients and is a major economic burden. Psoriasis has a large spectrum of clinical features and evolution. Clinical features of chronic psoriasis are generally sufficient to make the diagnosis. Diagnostic doubts, however, may arise in several clinical variants and atypical cases or when the psoriatic lesions are localized in particular sites.

Neonatal and infantile erythroderma is a diagnostic and therapeutic challenge. Numerous underlying causes have been reported. Etiologic diagnosis of erythroderma is frequently difficult to establish, and is usually delayed, due to the poor specificity of clinical and histopathologic signs. Differential diagnosis of erythroderma is a multi-step procedure that involves clinical assessment, knowledge of any relevant family history and certain laboratory investigations. Immunodeficiency must be inspected in cases of severe erythroderma with alopecia, failure to thrive, infectious complications, or

We report the first Japanese patient with primary cutaneous histoplasmosis who was infected inside Japan. He was not an immunocompromised host but his cutaneous lesions developed on preexisting papuloerythroderma (Ofuji). The long-term topical corticosteroid treatment for papuloerythroderma, which could have suppressed the local immunological defence system of the skin, might have been responsible for the histoplasma infection in the present case.

A female child with congenital progressive erythrokeratodermia combined with sensory hearing loss observed through a period of 5 years is reported. She demonstrates symmetrical hyperkeratotic skin changes, verrucous plaques on her nose cheeks, ears, chin, knees, elbows, and heels. Electron microscopic studies of her skin did not reveal qualitative changes, her moderate to severe hearing impairment is of cochlear origin, moderately progressive, and particularly affecting the high frequencies. Up to now a vascularizing keratitis could not be detected. Her family history is not contributory. Twen

Sézary syndrome is a malignant form of cutaneous T cell lymphoma in which patients characteristically present with generalized pruritic erythroderma and large numbers of circulating Sézary cells in the peripheral blood. Several previous studies have proposed that viruses may play a role in the cause of cutaneous T cell lymphoma. This report describes a 68-year-old man with Sézary syndrome who received a seven-day course of intravenous acyclovir for treatment of disseminated herpes zoster and was noted to have almost complete disappearance of generalized erythroderma and pruritus. Since acyclov
