Dermatomyositis (DM) in adults is frequently associated with cancer. In contrast, juvenile DM (JDM) is predominantly idiopathic and rarely reported with occult neoplasm. We describe a patient who presented with DM that was found to be a paraneoplastic manifestation of nasopharyngeal carcinoma. Although rare, paraneoplastic JDM must be suspected in the presence of unusual features such as elevated inflammatory markers or lymphadenopathy. Accurate clinical assessment including appropriate biopsies is needed before starting glucocorticoid therapy.

INTRODUCTION: An inflammatory myopathy, characterised by joint and muscle pain, chronic asthenia, with infiltration of peri-fascicular epimysium, perimysium and endomysium by cells of the macrophagic line, macrophagic myofasciitis is often associated with other, generally auto-immune, affections. However, the coexistence with another inflammatory myopathy is relatively rare.
OBSERVATION: A 29 year-old woman presented with 2 distinct inflammatory myopathies, a macrophagic myofasciitis and a dermatomyositis, which had appeared a few years after.
DISCUSSION: In the rare cases in which 2 inflamm…

PURPOSE OF REVIEW: Dermatomyositis is an inflammatory disorder of muscle affecting both children and adults. The pathogenesis of adult dermatomyositis is reviewed here, with particular attention to gaps and uncertainties in our current understanding of this disease, in order to focus attention on the outstanding research questions with regard to disease mechanisms.
RECENT FINDINGS: The conceptual model of the pathophysiology of dermatomyositis has been based on work extending back over at least 35 years, with important findings related to the deposition of membrane attack complex, the charact…

Juvenile dermatomyositis is a disease with similarities to chronic graft-versus-host disease. To identify whether chimerism is present in juvenile dermatomyositis, we investigated the families of 15 children with the disorder. Chimerism was identified by PCR in 13 of the 15 affected children, compared with five of 35 siblings (p<0.0001). Maternal cells among peripheral-blood mononuclear cells were detected in 11 of the 15 boys, compared with five of 17 unaffected controls (p=0.02), and in muscle tissue of 12 of 15 compared with two of ten unaffected siblings (p=0.005). These results suggest th…

PURPOSE: Polymyositis (PM) and dermatomyositis (DM) are inflammatory idiopathic myopathies of dysimmune origin. Studies have shown two different pathogenic mechanisms: a primitive vascular mechanism mediated mainly by a humoral mechanism responsible for a muscular ischemia in juvenile DM; and a primitive injury of muscle cells mediated by a cytotoxic cellular mechanism, directed against the myofibrillae, in PM.
CURRENT KNOWLEDGE AND KEY POINTS: Oral corticosteroids are the treatment of first choice in patients with PM/DM. This therapy has transformed the prognosis of these diseases. The effic…

Clinically apparent panniculitis is rare in dermatomyositis. The common histopathological findings are infiltration of lymphocytes, epithelioid cells and plasma cells in the fat lobules, along with varying degrees of fat degeneration and fibrosis. We report a 65-year-old woman with dermatomyositis who developed panniculitis with a characteristic histological change known as a membranocystic lesion. Although this change has been observed in various diseases affecting the subcutaneous fat tissue, it has rarely been reported in dermatomyositis. Dermatomyositis should be included in the diseases s…

OBJECTIVE: To measure calcium metabolism in 12 children with juvenile dermatomyositis (JDM).
METHODS: We used dual-tracer stable isotope studies to measure calcium metabolism in 12 children with JDM and a group of 43 healthy children (Group HC) of similar ages. Five of the JDM subjects were receiving steroids (Group JDM-ST) and 7 were not (Group JDM-NS).
RESULTS: The rate of calcium absorption in Group JDM-ST was lower than that in Group HC (19 +/- 10% vs 30 +/- 11%, p < 0.05). The lower rate of absorption was associated with a net loss of calcium each day (calculated calcium retention, Vbal…

PURPOSE: The mainstay of pharmacologic therapy in patients with dermatomyositis is corticosteroids. However, because patients sometimes become refractory to these drugs and because these drugs have potential short- and long-term toxicities, alternate therapy is highly desirable. Therefore, a pilot study was initiated using high-dose intravenous gammaglobulin (IVGG) in the treatment of dermatomyositis.
PATIENTS AND METHODS: IVGG was administered to five patients with juvenile dermatomyositis. Prior to IVGG treatment, all patients had persistent muscle weakness despite daily corticosteroids and…

Although substantial patients are dead in the course of PM/DM, little papers have precisely investigated the cause of death and factors which may relate to death in PM/DM. The purpose of the present study is to evaluate the prognosis for life, causes of death and risk factors for life in PM/DM. Clinical records of 76 adult PM/DM patients were retrospectively reviewed. Seven patients died of associated malignancy. Five-year survival rate was 69.7% in PM/DM patients without malignancy. During the last 24 years, there was no significant change in survival rate. Among causes of death in 19 cases w

Auto-immune mechanisms, mostly cell mediated, have been described as the basic abnormality in polymyositis and dermatomyositis. We report here the case of a patient affected with dermatopolymyositis, resistant to therapy, who was treated with corticosteroids and cyclosporin. She presented with an infiltrative, erythematous rash on the neck, eyelids and cheeks and weakness of the proximal limb and cervical muscles. The dramatic and persistent response to cyclosporin obtained in this case represents a clear demonstration of the action of this drug in cases of dermatomyositis.

This article reviews the clinical features and emphasises the treatment of polymyositis and dermatomyositis. Pharmacological intervention with corticosteroids and immunosuppressive agents is discussed. In addition, strategies regarding the initiation and subsequent tapering of these drugs are provided, and an algorithmic approach to the management of myositis is provided. Therapeutic modalities for patients with refractory disease are considered, and the potential adverse effects of such treatment are discussed. Since patients with myositis often have disease-related complaints other than musc…

Renal involvement is usually considered to be rare in dermatomyositis and polymyositis. The aim of this prospective study was to reevaluate the renal changes which may occur in these two conditions. Twenty patients, 12 with dermatomyositis and 8 with polymyositis of the adult type, were investigated to determine the incidence, the severity and course of renal disease. The 24 hour proteinuria was over 0.1 g in 14 cases and greater than 1 g/24 hours in 6 cases, including one patient with the nephrotic syndrome. Microscopic haematuria was noted in 3 cases and pyuria in 8 cases. Endogenous creatin…
