Joint BAPEN and British Society of Gastroenterology Symposium on 'Coeliac disease: basics and controversies'. Dietitians are better than clinicians in following up coeliac disease.

It seems obvious to healthcare professionals that patients with coeliac disease should receive regular follow-up. Surprisingly, there is little evidence that patients benefit in terms of reduced morbidity or mortality. However, several authoritative bodies have published guidelines on the management of coeliac disease that recommend regular follow-up. There is good evidence that compliance with a gluten-free diet reduces the risk of complications such as osteoporosis or small bowel lymphoma. Compliance is enhanced particularly by education about the disease and the gluten-free diet and by supp

Inpatient management of diabetes and hyperglycemia: implications for nutrition practice and the food and nutrition professional.

Although numerous guidelines and standards address the management of diabetes in outpatient settings, only recently has evidence been provided to issue standards of care to guide clinicians in optimal inpatient glycemic control for hospitalized individuals with diabetes or illness-induced hyperglycemia. Both the American Diabetes Association and the American College of Endocrinology recommend critically ill patients keep their blood glucose level as close to 110 mg/dL (6.1 mmol/L) as possible. In the noncritically ill patient, the American Diabetes Association recommends to keep pre-meal blood

Understanding insulin resistance in osteosarcopenic obesity: Therapeutic approaches for prevention and management.

Osteosarcopenic obesity (OSO) is a multifaceted condition in which obesity is present along with reduction in bone and muscle mass. The coexistence of these conditions increases the risk of fractures, physical disability and insulin resistance. This article explains the mechanism of insulin resistance in individuals with OSO highlighting the role of adipokines, lipid metabolites, pro inflammatory cells like Tumor Necrosis Factor-alpha, Interluekin-6, Interleukin-1 in causing the defective insulin signaling and exacerbating insulin resistance. The pathophysiology involves muscle atrophy and bon

Long-term neuromuscular, cardiac and liver outcomes in an adult man affected with Chanarin-Dorfman syndrome.

Chanarin-Dorfman syndrome (CDS) is an ultra-rare autosomal recessive subtype of neutral lipid storage disorder (NLSD); it is characterised by ichthyosis and intracytoplasmic accumulation of lipid droplets containing triglycerides, most commonly in granulocytes, muscle fibres, skin and liver. Several pathogenic variants in the ABHD5/CGI-58 gene have been described. Clinical manifestations include steatohepatitis, myopathy, ophthalmic disease, developmental delay. Liver involvement is an important cause of morbidity and mortality. We present a case of a 26-year-old male diagnosed with ichthyotic

Congenital enteropathies involving defects in enterocyte structure or differentiation.

Congenital enteropathies (CE) are a group of rare inherited diseases with a typical onset early in life. They involve defects in enterocyte structure or differentiation. They can cause a severe condition of intestinal failure (IF). The diagnostic approach is based first on clinical presentation (consanguinity, prenatal expression, polyhydramnios, early neonatal onset, aspect of stools, persistence at bowel rest, associated extra-digestive manifestations….) and histo-pathological analyses. These rare intestinal diseases cause protracted diarrhea that might resolve, for a few, with a dietetic ap

Vitamin C Deficiency Presenting as an Acute Limp in Childhood.

A previously well three-year-old boy attended with right knee pain and an antalgic gait. There was no history of fever and bloods including inflammatory markers were normal. A diagnosis of transient synovitis (irritable hip) was made and managed conservatively. One month later, he represented with increasing pain, now localising to his left hip, waking him at night with difficulty weight-bearing. No effusion was seen on ultrasound and bloods remained normal. He was discharged home but came back a week later with worsening polyarticular pain, a new maculopapular rash, significant bruising and n

Antidiabetic potential of commonly consumed legumes: a review.

Over the last few decades, lifestyle changes have resulted in a drastic increase in the incidence of diabetes all over the world, especially in the developing countries. Oral hypoglycemic agents and insulin form the mainstay in controlling diabetes, but they have prominent side effects and fail to significantly alter the course of diabetic complications. Appropriate diet and exercise programs that form a part of lifestyle modifications have proven to be greatly effective in the management of this disease. Dietary therapy is showing a bright future in the prevention and treatment of diabetes. L

[Unspecified gain of weight?].

In the case of a 49 year old patient, a weight gain of 37 kg occurred during the first weeks of the year 2003, accompanied by a painless increase in the diameter of the upper arms and thighs. This process did spontaneously cease in the summer of 2003, but weight reduction could not be achieved. The diagnosis of multiple symmetrical lipomatosis, also known as Launois-Bensaude syndrome, the first authors to describe the condition in detail in 1898, could be established due to the unique appearance of the patient. Typical features of the disease are the accumulation of multiple lipomata in the sh

Interactive group education for refugees from the Former Yugoslavia to reduce their oil consumption.

In 1999 at an outpatient dietetic consultation in Geneva, Switzerland, 20% of the patients were refugees. Most were from the Former Yugoslavia. The main goal of dietary counseling was to decrease their fat consumption. As one-on-one counseling was unsuccessful, a pilot intervention was developed to explore the potential of participatory workshops for identifying and implementing appropriate behaviors. Four series of three workshops were held. Participants identified and practiced ways to reduce fat consumption. Knowledge skills were measured on completion of the workshops and 6 months later. O

Sperm disorders according to the Byzantine medical writers (4th - 14th centuries).

Research in the works of the Byzantine medical authors brought to light significant information concerning disorders of the sperm as causes of infertility. The eminent Byzantine physicians give detailed accounts about the anatomy of the genitals, the creation of the sperm and its disorders as regards its quantity, quality, appearance, consistency, colour, ejaculation etc. According to those authors, the disorders of the sperm are due to, dietetic reasons (a list of foods and drugs influencing the production of sperm is referred to by many Byzantine physicians); constitution and age of the pati

[Thromboses and haemorrhages in geriatrics (author's transl)].

In elderly patients thromboses are especially important because of their frequency and consequences (invalidity) often demanding measures of rehabilitation. In thrombophilia there are prophylactic measures necessary founding upon new perceptions on pathogenesis (vascular wall factors; rheologic and microcirculatory factors and factors of hemostasis: increasing of factor VIII; decreasing of antithrombin III; hypofibrinolysis; increased aggregation of thrombocytes). In prophylaxis you should influence the predisposing factors (hypertension, diabetes, arteriosclerosis, adipositas), use dietetic a

[Toxoplasmosis and pregnancy. Evaluation of 2 years of prevention of congenital toxoplasmosis in the maternity ward of Hôpital Saint-Antoine (1973-1974)].

The authors report the results of prophylaxis of congenital toxoplasmosis in a maternity hospital in Paris for a two years period (1973-1974). 6269 pregnant women were surveyed. 18 toxoplasmosis were detected in evolution at the first prenatal examination, 10 seroconversions were identified among the first examination antibody negative women, when re-examined during the pregnancy. 25 of these 28 women were treated regularly. The seroconversions of the 3 other women were detected only at delivery. In addition 25 women were treated because of high antibody titers (Dye-Test greater than or equal

COVID-19 and Virtual Nutrition: A Pilot Study of Integrating Digital Food Models for Interactive Portion Size Education.

Background and aims: Digital food viewing is a vital skill for connecting dieticians to e-health. The aim of this study was to integrate a novel pedagogical framework that combines interactive three- (3-D) and two-dimensional (2-D) food models into a formal dietetic training course. The level of agreement between the digital food models (first semester) and the effectiveness of educational integration of digital food models during the school closure due to coronavirus disease 2019 (COVID-19) (second semester) were evaluated. Method: In total, 65 second-year undergraduate dietetic students were

Urolithiasis Due to Adenine Phosphoribosyl Transferase (APRT) Deficiency: A Case Report and Practical Recommendations in Children.

BACKGROUND: The rare metabolic disease, adenine phosphoribosyl transferase (APRT) deficiency, is asymptomatic in many cases until it is discovered in terminal renal failure. The most frequent complication is 2,8-dihydroxyadenine (2,8-DHA) urolithiasis. Stones, which are frequently bilateral and recurrent, appear in childhood in one-third of cases. CASE REPORT: An 8-year-old girl presented with urinary tract infection and coralliform urolithiasis in the right kidney. Initial radiological examination showed emphysematous pyelonephritis stage 1 due to obstructive lithiasis. Stone analysis helped

Fatigue in Cirrhosis.

Fatigue is a common symptom in patients with liver disease and has a significant impact on the health-related quality of life (HR-QoL). Its pathogenesis is poorly understood and is considered multifactorial. The liver is central in the pathogenesis of fatigue because it uniquely regulates much of the production, storage, and release of substrate for energy generation. Also, the liver "cross-talks" with the key organs that are responsible for this symptom complex-gut, skeletal muscle, and brain. Fatigue can have both peripheral (i.e., neuromuscular) and central (i.e., resulting from changes in

Microbiota and aging.

PURPOSE OF REVIEW: This article summarizes our current knowledge of changes in the intestinal microbiota in elderly people and centenarians. RECENT FINDINGS: Age-related processes comprise specific changes in the intestinal microbiota and related metabolic alterations. They result in 'inflamm-aging', which is associated with age-related inflammatory processes and diseases, including cachexia, frailty, cancer, and metabolic as well as neurological diseases. Age-related microbial changes consist of an increase in proteolytic bacteria and a decrease in saccharolytic bacteria. These changes are a
