Elective surgery for conservatively treated acute uncomplicated diverticulitis: a systematic review of postoperative outcomes.

INTRODUCTION: Elective surgery has been proposed, after at least two episodes of acute diverticulitis, initially treated conservatively, in order to prevent further episodes or chronic complaints. However, prophylactic surgery has been questioned, due to the associated risks of postoperative mortality and morbidity, as well as the risk of recurrent diverticulitis. This systematic review attempts to assess the role of prophylactic left colonic resection, after episodes of uncomplicated acute diverticulitis treated either conservatively with antibiotics and/or other supportive measures. EVIDENC

Laparoscopic diverticular resection with situs inversus totalis (SIT): report of a case.

Situs inversus totalis (SIT) is a rare condition where the abdominal and thoracic cavity structures are opposite of their usual position. Laparoscopic colonic surgery for this patient population is not well described, with only 2 reported cases. Our patient was a 62-year-old female with a history of SIT who underwent a laparoscopic sigmoid colectomy for recurrent diverticulitis. The procedure included the use of 4 ports. The sigmoid colon was noted on the right side. Laparoscopic resection with stapled anastomosis was performed. The patient tolerated the procedure well and was discharged home

[Computed tomography study of the sigmoid colon: discriminating diagnostic criteria and interobserver correlations].

PURPOSE: To assess the value of pericolonic findings at CT in the evaluation of the sigmoid colon. MATERIALS AND METHODS: A total of 210 CT examinations were retrospectively reviewed by 3 blinded radiologists. Data was analyzed to determine the interobserver correlation and the value of pericolonic and colonic wall findings in diagnosis of sigmoid colon pathology. RESULTS: The interobserver correlation for pericolonic findings was equal to or superior to that for colonic wall findings. The presence of abnormal pericolonic fat was the most sensitive (88%) and specific (93%) sign to differenti

Ureteral obstruction secondary to acute sigmoid diverticulitis.

Genitourinary manifestations and complications of sigmoid diverticulitis are well recognized. These consist mainly of bladder symptoms or fistula formation. Ureteral obstruction secondary to acute sigmoid diverticulitis is rare with only 3 cases documented and reported up to the present. All 3 cases were of left ureteral obstruction. Two patients are presented with acute sigmoid diverticulitis complicated by ureteral obstruction -- one of which was obstructed bilaterally. Both patients were admitted with left lower abdominal pain and tenderness and showed elevated temperatures with leukocytosi

Diverticulitis with perforation into the peritoneal cavity.

Sigmoid diverticulitis with free perforation or perforation through the mesentery results in generalized peritonitis. Emergency surgical treatment is mandatory, but the most efficacious procedure has not been clearly established. Ten consecutive patients were treated by removal of the perforated sigmoid colon, temporary end colostomy and peritoneal toilet. All but one patient survived the initial procedure, and there were only four minor complications. The preoperative diagnosis was correct in 8 of the 10 patients. Analysis of the preoperative clinical findings revealed that a decision in favo

Sigmoid resection for diverticulitis is more difficult than for malignancies.

PURPOSE: Sigmoid resection for diverticulitis is usually the first procedure performed when starting the learning process for laparoscopic colorectal surgery. The aim of this study is to evaluate the difficulty of laparoscopic sigmoid resection for diverticulitis in comparison to sigmoid malignancy in order to assess its role in the residents training program. METHODS: A cohort of patients was selected who suffered either from malignancy or recurrent diverticulitis in the sigmoid colon. Laparoscopic sigmoid resection was performed. The degree of difficulty was assessed by intraoperative compl

Incidence, Risk Factors, Clinical Presentation, and Treatment Outcomes of Acute Diverticulitis Requiring Hospitalization in Kidney Transplant Recipients.

Acute diverticulitis, a complication of diverticulosis, poses significant challenges in kidney transplant recipients who are often on immunosuppressive therapy. This retrospective single-center observational cohort study analyzes the incidence, clinical presentation, and management of acute diverticulitis in kidney transplant patients at University Hospital Center Zagreb over a 14-year period. Among 1400 kidney transplant recipients, 10 patients (0.7%) developed acute diverticulitis requiring hospitalization. The average age of these patients was 60.7 years, with abdominal pain as the predomin

Giant Colonic Diverticulum: A Rare Type of Diverticular Disease.

Giant colonic diverticulum (GCD) is a well-recognized but infrequently encountered disease in clinical practice. GCD is its own unique entity and differs from commonly seen diverticular disease in both size and management. Initial clinical presentation is typically associated with diverticulitis and symptoms such as abdominal pain, fever, nausea, vomiting, rectal bleeding, or even a palpable abdominal mass. Surgery is the recommended treatment option largely due to the risk of associated complications including colonic perforation. We describe the case of a 56-year-old female diagnosed with a

Early lap-band erosion associated with colonic inflammation: a case report and literature review.

INTRODUCTION: Laparoscopic adjustable gastric banding is an effective and safe surgical modality for the treatment of morbid obesity. Erosion of the band into the stomach has been reported. No reports are available on erosion of the Lap-Band following diverticulitis of the colon. CASE REPORT: A 31-year-old female with a body mass index (BMI) of 52 underwent an uneventful laparoscopic Lap-Band placement. Postoperative contrast study revealed good positioning of the band and no evidence of leakage. The patient's recovery was uneventful except for an elevated temperature of 101.5 degrees F that

Acute diverticulitis in young adults.

Diverticular disease of the colon is generally considered a disease of older patients, rarely causing symptoms before age 40. Two recent cases of ruptured sigmoid diverticulitis in young female patients presenting to our Emergency Department prompted a review of the literature on this topic. Diverticulitis is more common in patients under 40 than previously suspected. Presented are the two cases and a brief summary of the literature review.

Pilot multicentre open-label randomised controlled trial assessing prevention of recurrent DIverticulitis through the use of pelvic floor PHYSIOtherapy: DIPHYSIO study protocol.

INTRODUCTION: Despite the ubiquity of diverticular disease, the options for reducing the risk of recurrent diverticulitis remain limited and the pathogenesis remains incompletely understood. While high intraluminal pressures within the distal colon and rectum have been proposed as a possible association with diverticular disease, studies on this relationship have been few, inconsistent and not generalisable. The investigators of this pilot study propose that the repeated transient high intraluminal pressures generated within the distal colon and rectum due to ineffective defecatory technique m

The role of heterotopic gastric mucosa with or without colonization of Helicobacter pylori upon the diverse symptomatology of Meckel's diverticulum in children.

The roles of heterotopic gastric mucosa either with or without colonization of Helicobacter pylori (HP) upon the diverse symptomatology of Meckel's diverticulum (MD) in children have been evaluated retrospectively. The medical records of 92 patients who underwent MD excision either incidentally or symptomatically between 1976 and 1997 were reviewed retrospectively. Age at admission and symptoms were recorded. The slides were stained with hematoxylin eosin and Giemsa to identify the presence of heterotopic tissue, ulceration, hemorrhage, inflammation and HP. Bleeding, obstruction and inflammato

Lower bowel disorders. 2. Diverticular disease.

Diverticula are thought to result from high intraluminal pressures in closed segments of the sigmoid colon, causing herniation of mucosa and submucosa through muscle along the paths of the nutrient vessels. Mild diverticular disease usually can be managed with bed rest and a controlled diet, plus administration of antibiotics. Acute diverticulitis, especially recurrent bouts, may call for colonic resection.

Imaging diverticular disease.

Barium enema remains the gold standard for demonstrating the extent and severity of colonic diverticular disease. As such, barium studies have a role in clarifying the differential diagnosis of patients with abdominal pain and altered gut function. In acute diverticulitis or suspected diverticular perforation water soluble contrast studies are preferred to barium. An alternative in this acute scenario is cross-sectional imaging by ultrasound, or more usefully computed tomography (CT). CT is especially helpful in complicated diverticular disease. Diverticular disease is a common finding at colo

Ultrasound of acute GI tract conditions.

Patients with acute abdominal pain are a great challenge to the radiologist. The clinical diagnosis is classically unreliable, resulting in both negative laparotomies as well as ill-advised surgical delay in a large number of patients. Ultrasound offers a non-invasive way to decrease both false-negative and false-positive diagnoses in this category of patients. This article focuses on the role of sonography in the diagnosis of acute conditions of the gastrointestinal tract tract such as appendicitis, sigmoid diverticulitis, Crohn's disease, colitis, infectious ileocecitis caused by Yersinia, C

Diverticular perforation of terminal ileum associated with chemotherapy for non-small cell lung carcinoma: a case report.

A 71-year-old man was diagnosed with advanced non-small cell lung carcinoma and treated with chemotherapy developed ileocecal diverticulitis three times over the last 2 months of receiving second-line treatment. During the fourth diverticulitis event, the patient presented with fever and abdominal pain, worsening after 5 days. Abdominal computed tomography showed ascites and intra-abdominal free air, suggesting bowel perforation with acute diffuse peritonitis. We performed emergency surgery; the surgical findings showed diverticulosis with perforated diverticula in the ileocecal region. We per

Computed tomography evaluation of diverticulitis.

Acute diverticulitis is a frequent and important diagnosis in gastrointestinal disease, most commonly involving the colon. It is estimated that approximately 15% to 30% of patients with diverticulosis develop symptomatic diverticulitis at some point in the natural history of the condition, often requiring medical and/or surgical therapy. The clinical diagnosis is often difficult to make, and several radiological studies have been used over the past decades to assist in the diagnosis of acute diverticulitis. These include barium enema, ultrasound, and computed tomography (CT). A number of studi

Patterns of pain in diverticular disease and the influence of acute diverticulitis.

BACKGROUND: While the association of recurrent bouts of abdominal pain with colonic diverticulosis is well recognized, the cause of this pain is obscure since in most cases it occurs without obvious diverticulitis or other potential causes. AIMS: To define the patterns of pain in diverticular disease and the influence of acute diverticulitis. METHODS: Two studies were undertaken to establish the relationship between bouts of prolonged abdominal pain (> 24 h) presumed to be due to inflammatory diverticulitis and recurrent short-lived pain. In Study 1, 261 patients with a barium enema showing
