An unusual case of bilateral vestibulopathy, chronic subjective dizziness and spondyloarthropathy.

OBJECTIVE: Vertigo and dizziness are two common symptoms seen in everyday practice. However, in some cases, making a diagnosis can be challenging. This case report shows the relevance of a careful psychiatric history, which led to the diagnosis of chronic subjective dizziness associated with bilateral peripheral vestibulopathy. METHODS: A case of a 33-year-old lady with multiple episodes of vertigo attack after a minor head injury is reported. RESULTS: A comprehensive audiometric and vestibular evaluation reviewed bilateral peripheral vestibulopathy. The frequency of the vertigo attacks decr…

The University of California Los Angeles Dizziness Questionnaire: advantages and disadvantages.

Dizziness is one of the most troublesome symptoms of vestibular disorders, although its description remains somewhat imprecise, encompassing vertigo, lightheadedness, disequilibrium and giddiness. Symptoms include sensitivity to motion stimuli and discomfort with open spaces, such as empty streets and bridges. It is accompanied by varying degrees of anxiety and phobic behaviors. Several tools have been developed in an attempt to quantify the degree of disability, self-perceived health status and quality of life in patients with dizziness. This review focuses on the University of California, Lo…

[Vertigo and dizziness. Diagnostic algorithm from the perspective of emergency medicine].

Vertigo and dizziness are common symptoms in the acute care setting and have a wide diagnostic range. The most deleterious diagnosis is vertebrobasilar disease with brain infarction in the posterior fossa. Therefore, every patient with acute vestibular syndrome needs to be evaluated by a structured interview and a systematic physical examination for red flag symptoms which indicate vertebrobasilar infarction. Routinely, the physical examination should cover the head impulse (Halmagyi) test, test for nystagmus as well as the test of skew and in cases of benign paroxysmal positional vertigo, by …

The epidemiology of vertigo, dizziness, and unsteadiness and its links to co-morbidities.

Vertigo, dizziness, and unsteadiness (VDU) are common symptoms traditionally considered to result from different kinds of vestibular and non-vestibular dysfunctions. The epidemiology of each symptom and how they relate to each other and to migraine, agoraphobia, motion sickness susceptibility (MSS), vaso-vagal episodes (VVE), and anxiety-depression was the object of this population-based study in north-eastern France. A self-administered questionnaire was returned by 2987 adults (age span 18-86 years, 1471 women). The 1-year prevalence for vertigo was 48.3%, for unsteadiness 39.1%, and for diz…

Rate and predictors of serious neurologic causes of dizziness in the emergency department.

OBJECTIVE: To describe the rate and predictors of central nervous system (CNS) disease in emergency department (ED) patients with dizziness in the modern era of neuroimaging. PATIENTS AND METHODS: We retrospectively reviewed the medical records of all adults presenting between January 1, 2007, and December 31, 2009, to an academic ED for a primary triage complaint of dizziness, vertigo, or imbalance. The final diagnosis for the cause of dizziness was independently assigned by 2 neurologists, with a third neurologist resolving any disagreements. The primary outcome was a composite of ischemic …

[The objective method for differential diagnostics of vestibular and psychogenic (phobic) dizziness].

The results of the study opening up new possibilities for differential diagnostics of vestibular and psychogenic dizziness are presented. A specially developed combined approach was used including a set of highly informative stabilometric tests for the verification of pathological changes in the vestibular analyzer, such as Romberg test with both open and closed eyes, tandem test, head turn and tilt tests. The groups of the subjected enrolled in the study were shown to possess statistically different stabilometric characteristics.

Utility of head CT in the evaluation of vertigo/dizziness in the emergency department.

Acute dizziness (including vertigo) is a common reason to visit the emergency room, and imaging with head CT is often performed initially to exclude a central cause. In this study, consecutive patients presenting with dizziness and undergoing head CT were retrospectively reviewed to determine diagnostic yield. Four hundred forty-eight consecutive head CTs in a representative sample of dizzy emergency room (ER) patients, including patients with other neurological symptoms, were reviewed to identify an acute or subacute cause for acute dizziness along with the frequency and modalities used in fo…

Which medication do I need to manage dizzy patients?

Vertigo and dizziness are not independent disease entities, but instead symptoms of various diseases. Accordingly, a variety of treatment approaches are required. Here we review the most relevant drugs for managing dizziness, vertigo, and nystagmus syndromes. It is important to differentiate symptomatic treatment of nausea and vomiting with, for example, dimenhydrinate and benzodiazepines, and prophylactic treatment of motion sickness with scopolamine from a causal therapy of the underlying disorders. Examples of such causal therapy include aminopyridines for downbeat nystagmus and episodic at…

[The 'dizziness handicap inventory': initial psychometric evaluation of the German version].

BACKGROUND: Dizziness has a clear impact on quality of life of patients. Standardized instruments such as the "Dizziness Handicap Inventory" (DHI) help clinicians assess this impact systematically. The purpose of this study was to analyse the psychometric quality of a German version of the DHI. METHODS: One hundred and five patients with dizziness as their primary complaint have completed the DHI and undergone vestibular examination. The structure of the DHI was determined with a factor analysis, a principal component analysis with a Varimax rotation. To evaluate the reliability, internal con…

What is the effect of dizziness on the quality of life for patients with Meniere's disease?

OBJECTIVE: To evaluate the impact of dizziness on quality of life (QOL) in Meniere's disease (MD) patients. METHODS OF STUDY: Fifty definite MD patients were submitted to the Dizziness Handicap Inventory (DHI), in and out of crisis. The DHI scores were correlated to age, sex, race, disease duration, unilateral/bilateral labyrinth involvement, duration/frequency of vertigo attacks, vestibular and auditory function, aural fullness, tinnitus, imbalance and functional disabilities. Statistical analysis was performed by ANOVA. RESULTS: During crisis, all DHI aspects scores were statistically high…

Anxiety disorders and other psychiatric subgroups in patients complaining of dizziness.

Two hundred and two consecutive patients with dizziness were evaluated using blind neuro-otological testing and examination, blind psychiatric examination, including structured interviews (according to DSM-IV), the Symptom Check-List (SCL 90 R), and the State-Trait Anxiety Inventory (STAI). In 28% of the patients (N=50) dizziness was of organic origin (O group); in 55.3% (N=99) of psychogenic origin (P group) and in 16.8% comorbid psychiatric disorders were found (Mixed group). In 5.3% (N=10) neither organic nor psychiatric results could be found, which could explain the dizziness (Ideopathic …

Incidence of arrhythmias in normal pregnancy and relation to palpitations, dizziness, and syncope.

We assessed the relation between symptoms and cardiac arrhythmias in 110 consecutive pregnant patients without evidence of heart disease referred for evaluation of palpitations, dizziness, and syncope (study group) and in 52 consecutive patients referred for evaluation of an asymptomatic functional precordial murmur (control group). Both groups had a high incidence of arrhythmias on Holter monitoring with atrial premature complexes (APCs) of 56% in the study group and 58% in the control group, > 100 APCs in 7% and 4% of the patients, respectively, and isolated ventricular premature complexes (…

ENG in the emergency room: subtest results in acutely dizzy patients.

A retrospective analysis of electronystagmography (ENG) subtests performed on 93 acutely dizzy patients in the emergency room (ER) shows that "pendular tracking" has high sensitivity and specificity for determining central pathology. Small combinations of subtests altered sensitivity or specificity only slightly over pendular alone. Detection of spontaneous nystagmus was also helpful in ruling out "other" diagnoses such as hyperventilation syndrome, psychogenesis, or malingering. Popularity of the shortened ENG in the ER was evidenced by the high rate of charting of results and the frequency w

Autonomic nervous system disturbance as etiological background of vertigo and dizziness.

In order to investigate whether dysautonomia lies behind vertigo and/or dizziness in the neurotological clinic, an electrocardiogram (ECG) R-peak to R-peak test (R-R test) along with the Schellong test were simultaneously performed in 136 subjects. ECG data were analyzed by a microcomputer, and R-R intervals were calculated. In a dynamic test performed during postural changes from the supine to the standing position, there were 23 of 55 (41.8%) positive test results in patients with peripheral vestibular disorders, 28 of 51 (54.9%) with central vestibular disorders, and 15 of 18 (83.3%) with s…

Blood pressure abnormalities as background roles for vertigo, dizziness and disequilibrium.

In 3 years, a total of 1,291 patients have undergone equilibrium examinations because of vertigo, dizziness and disequilibrium in the Otoneurological Division of Toyama Medical and Pharmaceutical University. Abnormal blood pressures (hypertension, hypotension and orthostatic hypotension) were found in 18.67% (241 patients). Hypotension was found significantly more often in peripheral vestibular disorders and hypertension in central nervous system disorders. Furthermore, the incidence of orthostatic hypotension was statistically higher in hypertensive patients than in hypotensive patients. Ther…

Dizziness, falling, and fainting: differential diagnosis in the aged (Part I).

Lack of awareness of benign positional vertigo is a common cause of misdiagnosis of dizziness. Patients complain of brief bouts of vertigo, specifically provoked by turning the head or by arising in the morning. The Bárány's test reproduces characteristic vertigo and nystagmus. Postural hypotension, by far the most common cause of recurrent dizziness and falls in the elderly, is especially notable on arising after prolonged bedrest. Vasoactive drugs are the most common culprits, typically antihypertensive agents, vasodilators used for angina, and tricyclic antidepressants.

Dizziness, falling, and fainting: differential diagnosis in the aged (Part II).

Salicylates used for rheumatoid arthritis, or in inadvertently high dosages by patients themselves, often cause toxicity manifested by tinnitus, hearing impairment, and imbalance. Vestibulotoxic drugs should always be considered in older patients when the cause of imbalance is not apparent. Gait disturbance is an important presenting symptom of subdural hematoma, along with a fluctuating level of alertness. A high degree of suspicion is required to diagnose this treatable condition early.
