Male sex reversal syndrome: a disorder of sexual differentiation (DSD) with infertility.

46,XX male sex reversal syndrome is a rare genetic disorder where individuals with a 46,XX karyotype present with male phenotypic characteristics despite the absence of an Y chromosome. We report a case of a male in his early 30s, presenting with infertility and azoospermia, with bilateral small testes, but no other sexual dysfunction or any signs of hypovirulisation. His hormonal evaluation revealed hypogonadism with normal gonadotropin levels. Genetic testing showed a 46,XX karyotype with the presence of the sex-determining region on the Y chromosome (SRY) gene and microdeletions in the azoo

Successful Pregnancy Outcome after In Vitro Fertilization in a Kidney Graft Recipient: A Case Report and Literature Review.

BACKGROUND: Successful spontaneous pregnancy in a kidney graft recipient is regarded as a sign of full recovery. The crucial factors determining positive outcome are optimizing time of conception and multidisciplinary team care. However, there are only a few reports dealing with in vitro fertilization (IVF) outcomes in organ recipients. CASE REPORT: A 34-year-old living donor kidney recipient with primary infertility due to bilateral tubal obstruction was referred to our clinic. Transfer of 2 embryos was conducted after a long stimulation protocol with GnRH and rFSH, and a viable singleton pr

Intracerebroventricular infusion of exercise donor plasma exosomes induces molecular changes indicating exercise-like adaptations in basal ganglia cells of older male rats.

Chronic moderate aerobic exercise promotes health in older adults and provides neuroprotection to patients with neurodegenerative diseases, most notably Parkinson's disease (PD). Exosomes are small extracellular vesicles that facilitate interorgan communication. During exercise, they are selectively packaged with bioactive molecules termed "exerkines" and released into the blood. Exosomes protect their exerkine cargo, including DNA, RNA, and proteins, and facilitate their interorgan transit. Since aging is the primary risk factor for common neurodegenerative disorders, including PD, in this st

Designing biomaterials for the modulation of allogeneic and autoimmune responses to cellular implants in Type 1 Diabetes.

The effective suppression of adaptive immune responses is essential for the success of allogeneic cell therapies. In islet transplantation for Type 1 Diabetes, pre-existing autoimmunity provides an additional hurdle, as memory autoimmune T cells mediate both an autoantigen-specific attack on the donor beta cells and an alloantigen-specific attack on the donor graft cells. Immunosuppressive agents used for islet transplantation are generally successful in suppressing alloimmune responses, but dramatically hinder the widespread adoption of this therapeutic approach and fail to control memory T c

Preclinical and clinical studies on kidney allograft tolerance via hematopoietic chimerism.

In current clinical practice, successful kidney transplantation requires lifelong immunosuppression to prevent graft rejection, which is unfortunately associated with significant morbidity, mortality, and reduced quality of life. Over the past 2 decades, there has been growing interest and encouraging progress in clinical therapeutic approaches to transplantation tolerance, defined as stable allograft acceptance in the absence of chronic immunosuppression. To date, these advances have largely been based on the concept of hematopoietic chimerism, achieved by bone marrow or hematopoietic stem ce

Three successful pregnancies following natural conception over an 8-year time span despite serum follicle stimulating hormone level greater than 15 mIU/ml.

PURPOSE: To demonstrate that the concept that a level of serum follicle stimulating hormone (FSH) of > 15 mIU/ml on day 3 of a younger woman's menstrual cycle suggests that the remaining eggs are of very poor quality consistent with a woman of very advanced reproductive age is a fallacy. METHODS: A woman with a serum FSH > 15 mIU/ml on day 3 was studied over an 8-year time period. RESULTS: Despite the absence of therapy with follicle maturing drugs, and at the latter part of the study the development of oligomenorrhea, the woman had three successful conceptions over eight years without ART t

Large Stokes shift fluorophores from meta-substituted zwitterions.

Many commonly used dye classes suffer from strong overlap of their absorption and emission spectra, favoring reabsorption and hampering the combination of several dyes for multi-analyte sensing with single wavelength excitation. This can be overcome by increasing the energy difference between absorption and emission using donor-acceptor dyes with charge-transfer processes. A neglected concept to finetune the Stokes shift is meta-substitution, which we exploited to design a single-benzene fluorophore exhibiting the largest Stokes shift of a zwitterionic compound. The meta-substitution of perman

Adaptive Co-Crystals with Switchable Host-Guest Stoichiometry for Dynamic Regulation of Photoluminescence.

The development of photoluminescence materials with tunable properties through structural and functional modifications remains a fascinating yet challenging task. In this work, the concept of adaptive co-crystals featuring switchable host-guest stoichiometry within the realm of pillararene-based supramolecular chemistry is proposed. This strategy allows for the rational design of a diverse range of through-space charge-transfer co-crystals, enabling dynamic modulation of photoluminescence in the solid state. Experimental and theoretical findings unveil two distinct phases within each set of co

Enantioselective Formation of Isoxazolidines via Brønsted-Base Catalyzed (3 + 2) Annulation of Cyclopropanes with Nitrosoarenes.

Enantioselective transformations of donor-acceptor cyclopropanes have opened a new chemical space for the construction of enantioenriched molecules. This work presents the first catalytic enantioselective synthesis of isoxazolidines─a privileged key-structure in organic and medicinal chemistry─through a Brønsted-base catalyzed (3 + 2) annulation of donor-acceptor cyclopropanes with nitrosoarenes. The reaction concept is general, scalable to gram-scale, and enables the reaction between cyclopropanes, substituted with ketones, aldehydes, esters, thioesters, or sulfones, and nitrosoarenes with di

A myeloid trisomy 21-associated gene variant is protective from Alzheimer's disease.

Alzheimer's disease causes progressive cognitive decline, yet some individuals remain resilient despite developing hallmark pathology. A subset of people with Down syndrome (DS), the most common genetic cause of Alzheimer's disease, demonstrates such resilience. Given the elevated risk of hematopoietic mutations in DS, we hypothesize that certain variants may confer microglial resilience. Here, we introduce a myeloid DS-linked CSF2RB A455D mutation into human pluripotent stem cell-derived microglia from both donors with DS and healthy donors and study their function in 4-10-month-old chimeric

Novel ray of hope for diabetic wound healing: Hydrogen sulfide and its releasing agents.

BACKGROUND: Diabetes mellitus (DM) is a long-term metabolic disease accompanied by difficulties in wound healing placing a severe financial and physical burden on patients. As one of the important signal transduction molecules, both endogenous and exogenous hydrogen sulfide (H2S) was found to promote diabetic wound healing in recent studies. H2S at physiological concentrations can not only promote cell migration and adhesion functions, but also resist inflammation, oxidative stress and inappropriate remodeling of the extracellular matrix. AIM OF REVIEW: The purpose of this review is to summar

Activation and regulation of alloreactive T cell immunity in solid organ transplantation.

Transplantation is the only curative treatment for patients with kidney failure but it poses unique immunological challenges that must be overcome to prevent allograft rejection and ensure long-term graft survival. Alloreactive T cells are important contributors to graft rejection, and a clearer understanding of the mechanisms by which these cells recognize donor antigens - through direct, indirect or semi-direct pathways - will facilitate their therapeutic targeting. Post-T cell priming rejection responses can also be modified by targeting pathways that regulate T cell trafficking, survival c

Concept, Principles, and Objectives of Population Hematology

The article provides grounds for a new, expanded definition of a concept «population hematology». It is not only a hematological subsection of the traditional population and epidemiological medicine. It is focused on patients populations as its primary object; however, its uniqueness is in the fact that its primary object is the nested population hierarchy, such as blood cells, cell clones and human groups. The heterogeneity of the samples and changes in the heterogeneity with the course of time are most important. Taking into account the time factor, staging, and long-term observation are uni

Stem cell engraftment strategies.

The donor stem cell phenotype and host microenvironment determine the outcome of a stem cell transplant. In a series of transplant studies in syngeneic male to female or congenic Ly5.1/Ly5.2 models in which hosts have received no or minimal irradiation (100 cGy), evidence overwhelmingly supports the concept that syngeneic engraftment is determined by stem cell competition. These approaches can be extended to H-2 mismatched allogeneic mouse combination when antigen pre-exposure and CD40-CD40 ligand antibody blockage are employed. A human trial in patients with resistant neoplasia infusing phere

Immune maladaptation in the etiology of preeclampsia: a review of corroborative epidemiologic studies.

Genuine preeclampsia is a disease of first pregnancies. The protective effect of multiparity, however, is lost with change of partner. Also, exposure to semen provides protection against developing preeclampsia. Analogous to altered paternity, artificial donor insemination and oocyte donation are reported to result in a substantial increase of preeclampsia. Thus, epidemiologic studies strongly suggest that immune maladaptation is involved in the etiology of preeclampsia. Although the exact etiology of preeclampsia remains unknown, the conclusions derived from epidemiologic studies may have con

Australia antigen and liver function tests following infectious hepatitis. A study of 111 patients in quest of aids in blood donor screening.

An epidemic of infectious hepatitis involving 99 patients and employees of a state mental hospital revealed Australia antigen Au(1) to be absent from the blood of all but one of the subjects when tested at six weeks, three months, nine months and 12 to 18 months after onset of jaundice. The single patient with Au(1) at 12 months had no enzyme abnormality to indicate residual liver disease. If Au(1) is the virus of hepatitis these data would support the concept that persistent or long standing viremia is not a feature of epidemic hepatitis. Moreover, results of this study suggest that the Au(1)

[Organ donation in Germany under the new legislation].

The political debate concerning organ donations reached increased public awareness from 2012, despite the multiple scandals around organ allocation. The process of organ donation has been restructured since financing improved by the GZSO (Gesetz für bessere Zusammenarbeit und bessere Strukturen bei der Organspende). The focus should be reduction of deficits in donor identification and notification within hospitals. Hence, it is time to introduce an "opt-out" solution, a system already standard in other European countries. Finally, to increase in the much-needed organ donations within Germany a

Successful long-term management of recurrent focal segmental glomerulosclerosis after kidney transplantation with costimulation blockade.

Recurrence of primary focal segmental glomerulosclerosis (FSGS) occurs in up to 50% of patients after kidney transplantation and is associated with poor allograft outcome. Novel therapeutic concepts directly target podocyte function via B7-1 with inconsistent response. We present the case of a 19-year-old patient with recurrent primary FSGS early after living donor kidney transplantation. Plasmapheresis and rituximab did not induce remission. Repetitive abatacept administration was able to achieve partial remission. Maintenance immunosuppression was subsequently switched to a belatacept-based
