The cochlear amplifier: augmentation of the traveling wave within the inner ear.

PURPOSE OF REVIEW: There have been many recent advancements in our understanding of cochlear function within the past ten years. In particular, several mechanisms that underlie the sensitivity and sharpness of mammalian tuning have been discovered. This review focuses on these issues. RECENT FINDINGS: The cochlear amplifier is essentially a positive feedback loop within the cochlea that amplifies the traveling wave. Thus, vibrations within the organ of Corti are sensed and then force is generated in synchrony to increase the vibrations. Mechanisms that generate force within the cochlea includ

[The cochlear implant--evolution of hearing and language with an artificial inner ear].

One of the most spectacular progresses in modern medicine is the possibility to replace a deaf ear, a sensory organ in total by an implantable electronic prosthesis, a so-called cochlear implant (CI). The CI stimulates the auditory nerve by electrical pulses and thus generates the sensation of hearing along the auditory pathway. One of the most impressive aspects of cochlear implantation is the fact that small children with profound deafness who were able a few years ago to learn spoken language only to very limited extent may achieve nowadays an almost normal language development. Duration an

Surgery of the inner ear with hearing preservation: serial histological changes.

OBJECTIVES/HYPOTHESIS: Surgery of the inner ear can result in hearing preservation under certain conditions, but the mechanisms responsible for hearing preservation or loss are not well understood. The specific aim of the study is to examine histological sections obtained at different time intervals after varying degrees of surgical entry into the inner ear, to understand how the cochlea is protected. The hypothesis is that internal partitioning occurs. STUDY DESIGN: Histologic examination of guinea pig inner ears by light microscopy. METHODS: Guinea pigs underwent lateral semicircular canal

The effect of chronic otitis media on the immunoreactivity of human inner ear.

Twenty temporal bones (TBs) were removed from autopsy cases and prepared for immunohistochemical examination. Ten TBs were free of ear disease whereas the other ten TBs showed the signs of chronic otitis media. Expression of markers for monocyte-macrophages (25F9, 27E10) and natural killer cells (anti-Leu-11) was examined immunohistochemically. There were no specific positive stainings with 25F9 or anti-Leu-11 antibodies in any of the specimens. Staining for 27E10 was found to be negative in each section obtained from normal cochlea. However, 27E10 positivity was detected in three of ten TBs w

The vascular mechanism of action of betahistine in the inner ear of the guinea pig.

The aim of this study was to investigate the mechanism and site of action of betahistine dihydrochloride in the inner ear of the guinea pig. Betahistine-evoked increases in cochlear blood flow (CBF) have been presumed to be due to the drug effect on the later wall capillary bed or larger feeding vessels in the cochlea vascular system. As such, the mechanism of action could be due to inhibition of H3 receptors. Betahistine may also have a direct effect on postsynaptic H1/H2 receptors and/or an effect modulated by other autonomic receptors. Betahistine-evoked CBF responses were assessed by laser

Vasopressin and oxytocin receptor mRNAs are expressed in the rat inner ear.

The cause of endolymphatic hydrops, a characteristic finding in Menière's disease, is not known. To study the possible involvement of the neurohormones vasopressin and oxytocin in this condition, we investigated whether transcripts of the genes encoding the arginine vasopressin (AVP) and oxytocin receptors are expressed in the rat inner ear. Utilizing the reverse transcription-polymerase chain reaction (RT-PCR) method, primers specific for each receptor showed a single message band of the expected size in the rat inner ear. When the PCR products were cloned, the sequences were identical to tho

[Toxic inner ear damage in topical treatment of psoriasis with salicylates].

BACKGROUND: Whereas cochlear impairment after intravenous ingestions of salicylates is well known, reports of cochlear symptoms after topical application are quite rare. The extent of the ototoxic salicylate impact is increased by diabetes, renal insufficiency, and alcoholism. PATIENT: This study presents a case report of a female patient who suffered a repeated, symmetric, pancochlear, reversible inner ear impairment after two treatments with salicylate containing ointment for psoriasis. The correlation of the salicylate therapy with the observed inner ear lesions is obvious due to the close

The localization and specificity of guinea pig inner ear antigenic epitopes.

In this study, we investigated the relative localization of some antigenic epitopes in the inner ear. The inner ear protein antigens were extracted from various parts of the guinea pig inner ear. Brain, kidney, lung, heart and liver extracts were also obtained. We found by SDS-polyacrylamide gel electrophoresis that total inner ear extracts separated into three high concentration polypeptide bands with molecular weights of approximately 30, 42, 58 kd and three low density bands of 20, 25 and 35 kd. The 30 kd band was found mainly in the extract of the spiral ganglion and the acoustic nerve in

3DFT-magnetic resonance imaging of the inner ear in Meniére's disease.

Three dimensional Fourier transformation constructive interference in steady state (3DFT-CISS) magnetic resonance imaging (MRI) allows a detailed visualization of the membranous labyrinth of the inner ear. In this study the endolymphatic duct and sac is investigated in patients with Menière's disease using 3DFT-CISS MRI. In addition, the distance between the vertical part of the posterior semicircular canal and the fossa posterior is quantified.

Differential diagnosis of virus-like particles in the human inner ear.

The entire endolymphatic duct and sac as well as the vestibular epithelia were obtained from four patients with Meniere's disease during translabyrinthine (TL) eighth nerve section and from 12 patients undergoing TL resection of acoustic schwannomas. After these specimens were processed for routine transmission electron microscopy (TEM), they were studied for morphologic evidence of viral infection. Although no virus particles were identified, numerous regularly occurring cell components and artifacts were found to morphologically mimic viruses. An atlas of these structures is presented.

The lateral-line and inner-ear afferents in larval and adult urodeles.

The inner-ear and lateral-line afferents were studied in members of almost all urodele families and in two primitive representatives of anurans and gymnophionans by means of transganglionically transported horseradish peroxidase. The lateral-line projection patterns are, when present, identical in all urodeles and gymnophionans. This is in agreement with the presence of ampullary organs in all urodeles and gymnophionans which possess a lateral-line neuromast system. In contrast, even the most primitive anurans lack both ampullary organs and the dorsal projection of afferents from these organs.

Alcohol and ultrastructural changes in the developing inner ear. An in vitro study.

Inner ear explants from the CBA/CBA mouse were used in an organ culture system. The explants were cultured from the 16th gestational day until one day post partum. They were exposed to 1.5% or 3% ethanol in organ culture medium in order to determine any possible toxic effects upon the differentiating sensory structures of the sensory epithelium of the inner ear, that could be correlated to fetal alcohol syndrome. The higher concentration of ethanol caused a general and possibly unspecific destruction of the sensory epithelium, while the lower concentration caused characteristic changes includi

Four variations of the Mondini inner ear malformations as seen in microdissections.

Four variations and degrees of severity of the Mondini malformation were found in the temporal bones from two neonates, one with congenital heart disease and the other with trisomy D, and from one teenager with leukemia: 1) short cochlea and normal vestibular organs; 2) short cochlea and persistent horizontal canal anlage; 3) markedly shortened cochlea with no modiolus, wide internal auditory meatus, and persistent horizontal canal anlage; 4) same as variation 3, but with persistent anlagen in all semicircular canals. Variations 3 and 4 were from the case of trisomy D, in which the left cochle

[Effect of oxygen in the tympanum on inner ear function (author's transl)].

Possible effects of extreme variations of oxygen concentration from 0% to 100% in the tympanic lumen on inner ear function were investigated through animal experiments. The experiments were performed with Ketanest anaesthetized cats under short-time acoustic irradiation (3 min 115 dB, pink noise). Inner ear function was measured by means of the stapedius reflex audiometry. On the nitrogen-flushed side (pO2 approx. 0%), the stapedius reflex threshold dropped significantly more after high-frequency irradiation than on the O2-flushed side. Even without acoustic irradiation the stapediusreflex thr
