Auricular cartilage grafting to correct lower conjunctival fornix retraction and eyelid malposition in anophthalmic patients.

PURPOSE: To evaluate the surgical outcomes of auricular cartilage grafts to correct inferior fornix retraction and eyelid malposition in anophthalmic patients. METHODS: We performed a retrospective review of consecutive anophthalmic patients whose posterior lamella was lengthened by an autogenous ear cartilage graft to correct inferior fornix retraction and eyelid malposition. Surgical and postoperative complications, additional procedures, recurrence of fornix retraction, eyelid malposition, and donor-site complications were documented. RESULTS: Fifty-four patients (21 men, 33 women) were i…

Quantitative Framework Fabrication with Autogenous Costal Cartilage in Microtia Reconstruction.

Hearing improvement is another basic requirement for microtia patients in addition to aesthetic needs. This quantitative framework fabrication method can reduce the learning curve, obtain satisfactory aesthetic results with few complications, and reserve a certain space for future canalplasty. Laryngoscope, 134:148-153, 2024.

A Modified Technique in Rhinoplasty: A Septal Extension Graft Complex Using Septal Cartilage, Ethmoid Bone, and Auricular Cartilage.

BACKGROUND: Due to the small and weak septal cartilage of Asians, it is a challenge to obtain ideal tip projection and nose lengthening in this population by performing septal extension grafts with only septal cartilage. OBJECTIVES: The aim of this study was to introduce a septal extension graft complex incorporating nasal septal cartilage, the perpendicular plate of the ethmoid bone, and bilateral auricular cartilage, and to examine its effectiveness in terms of morphological and mechanical support. METHODS: Septal cartilage was harvested under an endoscope according to standard techniques.

Three-Dimensional-Printed External Scaffolds Mitigate Loss of Volume and Topography in Engineered Elastic Cartilage Constructs.

OBJECTIVE: A major obstacle in the clinical translation of engineered auricular scaffolds is the significant contraction and loss of topography that occur during maturation of the soft collagen-chondrocyte matrix into elastic cartilage. We hypothesized that 3-dimensional-printed, biocompatible scaffolds would "protect" maturing hydrogel constructs from contraction and loss of topography. DESIGN: External disc-shaped and "ridged" scaffolds were designed and 3D-printed using polylactic acid (PLA). Acellular type I collagen constructs were cultured in vitro for up to 3 months. Collagen construct

Novel decellularized animal conchal cartilage graft for application in human patient.

Restoration of the external ear and nose in human patients, in either congenital deformity or acquired defects, is a challenge in reconstructive surgery. Optimization of the currently available materials is necessary for rhinoplasty and microtia correction to avoid intraoperative manoeuvring and early rejection. The aim of this study was to develop cross-linked decellularized caprine conchal cartilages as biocompatible, robust, and non-toxic matrix template. The characterization of the decellularized tissue encompasses in vitro lymphoproliferation assay, cytotoxicity test, agar gel precipitati

Hyperbaric oxygen in the treatment of perichondritis of the pinna.

Perichondritis is a condition characterized by pain, swelling and purulent discharge from the external ear, which may progress to a deformity of the pinna. The presence of co-morbidities such as diabetes mellitus may aggravate the situation. The main aetiological agent is Pseudomonas aeruginosa and treatment consists of antibiotics combined with surgical drainage of the ear. We present the case of a diabetic patient with recurrent perichondritis of the pinna treated with hyperbaric oxygen therapy, with successful healing. Hyperbaric oxygen therapy has proved beneficial as adjunctive therapy of

Extrusion of pyrophosphate into extracellular media by osteoarthritic cartilage incubates.

The distribution of calcium pyrophosphate mineral phase, almost exclusively confined to articular cartilage in chondrocalcinosis, and the high level of pyrophosphate (PPi) ion relative to serum in synovial fluid in patients with either chondrocalcinosis or advanced osteoarthritis led to an investigation of whether cartilage cells elaborate PPi ions. Incubates of articular cartilage from young rabbits but not from mature rabbits, as well as growth plates cartilage, released PPi into incubation media during a 4h period. Control rabbit ear cartilage and synovial membrane elaborated negligible amo…

[Application of the remnant in repairing the reconstructed ear in microtia].

OBJECTIVE: To explore the application of the remnant in repairing the reconstructed ear in microtia. METHODS: From October 2000 to July 2006, 1,823 patients with reconstructed ear underwent the second operation for repairing. The remnants were used to reconstruct tragus, antitragus, crus of helix, concha cavum, or relax the adhesion between the reconstructed auricle and cranio wall, or insert into the reconstructed lobule or under the basement of the costal cartilage frame to restore the cranio-auricular angle or to the preauricular subcutaneous to correct the facial local depression. Onlay rib bone graft in elevation of reconstructed auricle: 17 years of experience.

BACKGROUND: A cartilage wedge block and covering flap are standard procedures for firm elevation of the ear in microtia correction. However, using costal cartilage for elevation of the reconstructed auricle can be insufficient, and the fixed cartilage wedge block may be absorbed or may slip out. Furthermore, elevating covering flaps is time-consuming and uses up fascia, a potential source of reconstruction material. Therefore, we propose an innovative method using autologous onlay rib bone graft for auricular elevation of microtia. METHODS: From February 1995 to August 2012, 77 patients recei

Current Status of Auricular Reconstruction Strategy Development.

Microtia has severe physical and psychological impacts on patients, and auricular reconstruction offers improved esthetics and function, alleviating psychological issues. Microtia is a congenital disease caused by a multifactorial interaction of environmental and genetic factors, with complex clinical manifestations. Classification assessment aids in determining treatment strategies. Auricular reconstruction is the primary treatment for severe microtia, focusing on the selection of auricular scaffold materials, the construction of auricular morphology, and skin and soft tissue scaffold coverag

Long-term results of otoplasty for microtia.

We report the results of otoplasty for microtia in 497 ears of 229 patients treated between January 1971 and March 1995. More than 80% of the cases showed satisfactory outcome. Seventy-six ears underwent minor additional surgery. When cartilage exposure occurred because of skin necrosis we used a free skin graft or a pedicled flap or sutured closely.

Integrated protocol of auricle reconstruction combined with hearing reconstruction.

CONCLUSIONS: Auricle reconstruction combined with hearing reconstruction can help patients with microtia to obtain practical hearing improvement as well as cosmetic improvement. The integrated procedures can facilitate the incision design and obtain a natural-looking three-dimensional shape of the reconstruction auricle. OBJECTIVES: To discuss the advantages of an integrated protocol for auricle reconstruction combined with hearing reconstruction. METHODS: We retrospectively analyzed the clinical information and follow-up results of 1300 cases (1460 ears) of congenital aural atresia with mic

Trachea Reconstruction with Single-Stage Composite Flaps in a Rabbit Model.

BACKGROUND: Trachea reconstruction requires creation of a functional lining, semirigid support, and vascularity. We aimed to design composite flaps with these three components in a rabbit model. METHODS: Circumferential (n = 9) and partial anterior (n = 8) tracheal defects were created in rabbits. A circumferential defect was reconstructed with a tubed ear flap incorporating cartilage for support and skin for lining. This was pedicled on the posterior auricular vessels and tunneled into the neck to bridge the defect. In the second experiment, a longitudinal anterior trachea defect was patched

Rethinking auricular trauma.

OBJECTIVES/HYPOTHESIS: An unrecognized auricular hematoma can lead to a disfiguring deformity, the cauliflower ear, but it can be prevented with prompt and comprehensive management. Fine needle aspiration with pressure bandages remains the mainstay treatment but will occasionally fail. We review our experience with recurrent or recalcitrant auricular hematomas in terms of their pathophysiology and revision surgery. STUDY DESIGN: Retrospective chart review. METHODS: A review of patients undergoing surgical incision, drainage, and debridement secondary to recurrent auricular hematomas was cond

Long-Term Clinical Results of Two-Stage Total Ear Reconstruction of Microtia Using Autologous Cell-Engineered Chondrocytes.

BACKGROUND: Microtia repair requires a large volume of reconstruction material. In pediatric patients, the collectable volume of autologous cartilage is limited, and the impact of surgical invasion and donor-site morbidity can be particularly severe. The authors developed a new treatment method using cultured autologous human auricular chondrocytes that provides a sufficiently large volume of reconstruction material. METHODS: Approximately 1 cm2 of auricular cartilage was collected from the affected site. Chondrocytes were isolated and cultured with autologous serum to accelerate cell prolifer
