A Follow-up Study of Electroconvulsive Therapy in Children and Adolescents.

OBJECTIVES: Electroconvulsive therapy (ECT) is an effective and a safe treatment for several severe psychiatric disorders across the age span. However, its use remains controversial and highly stigmatized especially among patients under 18 years. In this study, we examined current symptoms, attitudes, perception, and functioning of patients treated with ECT when they were less than 18 years old. METHODS: Participants had received ECT before age 18, between 1989 and 2015, at a tertiary medical center. Institutional review board-approval was obtained, and study documents (cover letter, consent,…

Electroconvulsive therapy and later stroke in patients with affective disorders.

The long-term effects of electroconvulsive therapy (ECT) on the risk of stroke are unknown. We examined the association between ECT and risk of incident or recurrent stroke. A cohort of 174 534 patients diagnosed with affective disorder between 2005 and 2016 in the Danish National Patient Registry were followed for stroke until November 2016. The association between ECT and stroke was analysed using Cox regression with multiple adjustment and propensity-score matching on sociodemographic and clinical variables. In 162 595 patients without previous stroke, 5781 (3.6%) were treated with ECT. The…

Association of Electroconvulsive Therapy With Psychiatric Readmissions in US Hospitals.

IMPORTANCE: Although electroconvulsive therapy (ECT) is considered the most efficacious treatment available for individuals with severe affective disorders, ECT's availability is limited and declining, suggesting that information about the population-level effects of ECT is needed. OBJECTIVE: To examine whether inpatient treatment with ECT is associated with a reduction in 30-day psychiatric readmission risk in a large, multistate sample of inpatients with severe affective disorders. DESIGN, SETTING, AND PARTICIPANTS: A quasi-experimental instrumental variables probit model of the associatio…

[Electroconvulsive therapy in the Netherlands: the practice in 2015 compared to that in 2008].

BACKGROUND: The most recent information about the use of ECT in the Netherlands dates from 2008. An update version of the guideline for the use of ECT in the Netherlands was issued in 2010. AIM: To obtain insight into the practice of ECT in the Netherlands five years after implementation of the guideline. METHOD: Our study is based on a questionnaire about the use of ECT by psychiatrists in all Dutch institutions (n=33). Questions concerned the use and availability of ECT as well as the implementation of the updated guideline; they also served as a check on the expertise and training of ECT-

Reexamining the Role of Electroconvulsive Therapy in Anorexia Nervosa in Adolescents.

Electroconvulsive therapy (ECT) is rarely, if ever, considered for the treatment of severe anorexia nervosa (AN) or other eating disorders comorbid with an affective illness. Mood disorders are common in these patients, and psychotropic medications, although frequently used, have limited evidence of benefit in AN or copresenting depressive symptoms. Even when the illness is life threatening, ECT as a treatment may be overlooked. We present a case of an adolescent girl with severe AN and comorbid major depressive disorder with suicidality. This patient failed multiple medication trials but went…

The impact of electroconvulsive therapy on the tryptophan-kynurenine metabolic pathway.

BACKGROUND: There is still limited knowledge about the mechanism of action of electroconvulsive therapy (ECT) in the treatment of depression. Substantial evidence suggests a role for the immune-moderated tryptophan (TRP)-kynurenine (KYN) pathway in depression; i.e. a depression-associated disturbance in the balance between the TRP-KYN metabolites towards a neurotoxic process. We, therefore, aimed to investigate the impact of ECT treatment on the TRP-KYN pathway, in association with ECT-related alterations in depressive symptoms. METHOD: Twenty-three patients with unipolar or bipolar depressio…

Electroconvulsive therapy in catatonic patients: Efficacy and predictors of response.

Recent evidence favors the view of catatonia as an autonomous syndrome, frequently associated with mood disorders, but also observed in neurological, neurodevelopmental, physical and toxic conditions. From our systematic literature review, electroconvulsive therapy (ECT) results effective in all forms of catatonia, even after pharmacotherapy with benzodiazepines has failed. Response rate ranges from 80% to 100% and results superior to those of any other therapy in psychiatry. ECT should be considered first-line treatment in patients with malignant catatonia, neuroleptic malignant syndrome, del…

Preanesthesia medical evaluation for electroconvulsive therapy: a review of the literature.

OBJECTIVE: Electroconvulsive therapy (ECT) is widely used for the treatment of psychiatric disorders, yet there is few published literature to guide the practitioner in the preprocedural evaluation of patients. Based on a review of the literature, we sought to develop a concise, algorithmic approach to be used when evaluating patients for ECT, including those with underlying conditions, such as cardiovascular and neurological disorders. METHODS: The databases of Ovid MEDLINE, PubMed, the Web of Knowledge, and PsychINFO were searched from January 2000 through December 2011. All abstracts were …

Rapid antidepressant effect of ketamine in the electroconvulsive therapy setting.

OBJECTIVES: Studies now provide strong evidence that the N-methyl-D-aspartate receptor antagonist ketamine possesses rapidly acting antidepressant properties. This study aimed to determine if a low dose of ketamine could be used to expedite and augment the antidepressant effects of electroconvulsive therapy (ECT) treatments in patients experiencing a severe depressive episode. MATERIALS AND METHODS: Subjects with major depressive disorder or bipolar disorder referred for ECT treatment of a major depressive episode were randomized to receive thiopental alone or thiopental plus ketamine (0.5 mg…

Electroconvulsive therapy for catatonia in a boy with hydrocephalus and an arachnoid cyst.

We report on the successful use of electroconvulsive therapy in a 13-year-old boy with congenital hydrocephalus, a history of multiple shunt revisions, and a stable prepontine arachnoid cyst, who experienced profound catatonic deterioration. After initial shunt placement at age 20 months, the patient had followed normal motor, cognitive, and social developmental trajectories. Two uncomplicated shunt revisions were performed at ages 10 and 13 years. Three months after the last revision, the patient demonstrated multiple psychomotor signs, culminating in hospital admission for rigidity, posturin…

Electroconvulsive therapy in the treatment of mixed states in bipolar disorder.

INTRODUCTION: Mixed bipolar states are not infrequent and may be extremely difficult to treat. Lithium, anticonvulsants including valproate and carbamazepine, and antipsychotics such as olanzapine, ziprasidone, and aripiprazole have been reported to be at least partially effective in controlled clinical trials, but many patients do not respond to pharmacological approaches. Electroconvulsive therapy has been tested to be efficacious for the treatment of both manic and depressive episodes, but much less evidence is available with regards to mixed states. The aim of the review was to report the …

Neurobiological correlates of the cognitive side effects of electroconvulsive therapy.

Although electroconvulsive therapy (ECT) is a highly effective form of treatment, its use is limited by the emergence of cognitive side effects, notably anterograde and retrograde amnesia. Despite a large literature on the neurobiology of therapeutic mechanisms of ECT, very little is known about the neurobiological underpinnings of its cognitive effects. On theoretical grounds, structures within the medial temporal lobes, especially the hippocampus, are predicted to be critical regions mediating anterograde and, possibly, retrograde amnesia. However, functional neuroimaging studies in normal v…

The safety of electroconvulsive therapy in patients with severe aortic stenosis.

Little is known about the safety of electroconvulsive therapy (ECT) in patients with severe aortic stenosis and depression and other psychiatric syndromes. We conducted a retrospective review of the medical records of 10 patients with severe aortic stenosis who underwent ECT at Mayo Clinic, Rochester, MN, between January 1, 1995, and June 30, 2006. Of the 10 patients, 6 (60%) were women. The median age was 79.5 years (range, 65-93 years). All patients had an aortic valve area of 1.0 cm2 or less (median, 0.9 cm2). The median aortic transvalvular pressure gradient was 43.5 mm Hg (range, 32-58 mm…
